In a previous study which evaluated the BACTEC 9240 automated blood culture system (Becton Dickinson Diagnostic Instrument Systems, Sparks, Md.), we noted a 1.3% "instrument false-positive" rate. Haemophilus influenzae has been reported on rare occasions as the cause of prostatitis and urinary tract infections. Here, we report a rare case of chronic prostatitis in a 52-year-old male with benign prostatic hypertrophy and discuss the possible underestimation of the true incidence of H. influenzae in genitourinary infections. This organism was identified only by its growth on chocolate agar, a medium that is not commonly used for urine cultures.

We present a rare case of endometriosis of the cervix. Transvaginal sonography showed a 35-mm cyst in the cervix with diffuse, low-level internal echoes. Transvaginal sonography-guided aspiration with a 17-gauge needle was performed, yielding a very thick chocolate-colored fluid. Cytological examination of the fluid revealed the presence of endometrial cells. Symptoms resolved after aspiration and no recurrence has developed.

Serious sequalae to the ingestion of several foods such as egg, fish and milk--and the lack of profound reactions to chocolate--have been well documented. However, no comparable study exists in regard to peanut, which has long been known to be a patent antigen. This article explores the role of peanut with respect to its hazards as an antigen and as a foreign body in the upper respiratory tract, and documents a number of fatal and near-fatal reactions.

Susceptibility of 64 clinical isolates of Neisseria gonorrhoeae to metronidazole was determined by the agar dilution technique using Mueller-Hinton and chocolate agar media. All isolates were resistant to metronidazole at 12.5 mug/ml in either medium. This lack of activity of metronidazole against N. gonorrhoeae would suggest that the ability to recover N. gonorrhoeae in patients being treated with metronidazole for concurrent Trichomonas vaginalis infections should not be hampered.

A patient is described with a mortar shell shrapnel fragment in the anterior compartment of the right lower leg that had been in situ for 46 years without causing symptoms, when a large hematoma leading to a "chocolate cyst" developed around it. Initially, the patient was thought to have a malignant tumor. Roentgenograms and CT scans showed shrapnel fragments, and the cyst was removed surgically. A digital subtraction angiogram ruled out any other involvement.
