Atypical fibro-osseous lesions, usually in the proximal femur, are a common consultative diagnostic problem. This is due to the fact that they contain a variety of patterns individually reminiscent of fibrous dysplasia, fibroxanthoma (non-ossifying fibroma), myxofibroma, lipoma, cyst, bone infarct, Paget's disease, and, occasionally, chondroma. Eight cases of primary mucocele or tumor in the sphenoid sinus treated in our hospital from 1965 to 1988 are reported. Six were male, 2 female. The ages ranged from 18 to 33 years. The course of disease was 20 days to 4 years with an average of 16 months. The common symptoms were headache and serious visual disturbance or involvement of cranial nerves III, IV, V and VI. Malignancy occurs more often in multiple enchondromas than in solitary enchondromas. In the hands the rate is about 14%. We amputated the third ray of the hand in a young man with recurrence of an enchondroma. The histology showed a low grade chondrosarcoma. The patient had no tumour recurrence or metastasis in the following 5 years. We describe the evaluation as well as the treatment and outline the literature.

Coccidioidomycosis, an infection caused by the fungus Coccidioides immitis, rarely affects the hand, but we report an unusual case which mimicked the radiological appearance of an enchondroma in the metacarpal. Curettage and bone grafting in combination with long-term antifungal therapy are necessary for successful treatment of coccidioidomycosis of the hand.

A thirteen-year old boy fell on his outstretched left palm, resulting in hyperextension. X-rays showed a fracture of the scaphoid through a cystic appearance in the center of the scaphoid. For two years, the patient was treated conservatively without any sign of healing. After operation, where curettage and autogenous-bone grafting were performed, the scaphoid healed completely. Biopsy revealed enchondroma.

Osteochondroma is about 50% of benign bone neoplasms and up to 15% of all primary tumours of the skeleton; metaphysis of long bones are more frequently involved with the spine being interested in 2.5-5% of all cases, usually pertaining to multiple osteochondromatosis, in which many areas of the skeleton are affected. A solitary osteochondroma of the right pedicle of L4 causing sciatalgic pain and difficulty warling and treated by surgery is presented. Only two other cases of solitary osteochondroma involving the lumbar spine have been previously reported.

A histopathological analysis of 119 surgically excised loose bodies revealed that the cases could be separated into three categories: (1) loose bodies due to synovial osteochondromatosis; (2) loose bodies due to osteochondral fracture; (3) loose bodies due to joint surface disintegration. Chondrosarcomas are the most common malignancy of cartilage and are associated with somatic mutations in isocitrate dehydrogenase 1 (IDH1) and IDH2 genes.
