INTRODUCTION: Human chorionic gonadotropin (hCG) is a heterodimeric hormone regulated in part by the maternal neuroendocrine-placenta axis. Serum levels of the beta subunit (hCGβ) are higher in Black vs. White pregnancies. The underlying causes of the difference remain unclear. The placental expression of human chorionic gonadotrophin (HCG) I- and beta-subunits was investigated in eight pregnancies presenting with trisomy 18 and in 30 normal pregnancies at 11-15 weeks gestation. In the control group, the median densitometric scores of placental beta-HCG and I-HCG mRNA were 1.23 and 1.74 respectively. Free beta-HCG is a new analyte that has been suggested to be superior to total HCG when used in combination with alpha-fetoprotein (AFP) for Down syndrome risk screening in early pregnancy. We report here the use of an alternative carrier diphtheria toxoid (DT) in human subjects to overcome antigen-specific unresponsiveness upon immunization with a hapten/ligand-carrier conjugate. In the phase I clinical trial of a birth control vaccine using gonadotrophin subunits linked to tetanus toxoid, some of the subjects failed to evoke a booster antibody response to human chorionic gonadotrophin (hCG). Presentation of the ligand on DT instead in subsequent immunizations restored anti-hCG response.

A 26 year old patient, who underwent an ovulation stimulation therapy (Clomiphene /UH-FSH), monitored only by ultrasound presented a severe Ovarian Hyperstimulation Syndrome (OHS). beta-HCG values confirmed conception, that outcame in an abortion. Ninety-one pregnancies were monitored prospectively with serial human chorionic gonadotropin (hCG) determinations and real-time sonography. The monitoring process included an initial hCG doubling time (DT) followed by sonographic examination for fetal heart motion at 7 to 8 weeks in the asymptomatic patient.
