Choroidal vascularity index as an indicator of vascular status of choroid, in eyes with nanophthalmos.

PURPOSE: To investigate choroidal vascular index (CVI) in eyes with nanophthalmos (NO) with the use of optical coherence tomography (OCT). METHODS: Macular enhanced depth imaging OCT scans of 25 eyes of 25 patients with NO and age-gender-matched 25 eyes of 25 control subjects were analysed. Images were binarized using the ImageJ software, and total choroid area (TCA), luminal area (LA) and stromal area (SA) were acquired. The main outcome measure was CVI, defined as the ratio of LA to TCA. RESULTS: Twenty-five eyes of 25 patients with NO and age-gender-matched control subjects were enrolled.…

Intravitreal Bevacizumab for Choroidal Neovascular Membrane at the Edge of a Healed Choroidal Tuberculoma.

The authors present a 36-year-old female with pulmonary tuberculosis who developed a choroidal tuberculoma in the left eye. The choroidal tuberculoma successfully resolved with visual gain following oral anti-tubercular and oral steroid therapy leaving behind a chorioretinal scar. One year after the completion of anti-tubercular treatment, she developed visual loss due to the development of a secondary choroidal neovascular membrane at the fovea. This was treated successfully with one intravitreal injection of bevacizumab in the left eye. The fovea remained free of fluid until the last follow-…

The anterior choroidal artery syndrome. I. Microangiography of the anterior choroidal artery.

The distribution of the anterior choroidal artery (AChA) was analysed on coronal, sagittal and axial microangiograms of cadaver brains. On axial sections, the territory of the AChA was confined to an arcuate zone between the striatum anterolaterally and the thalamus posteromedially. On sagittal sections, fine vessels from the cisternal part of the AChA were seen to extend upwards and somewhat posteriorly into the region of the posterior limb of the internal capsule. On coronal sections, ascending branches of the AChA coursed laterally and then upwards to penetrate the globus pallidus and the i…

Functional anatomy of the choroidal circulation: methyl methacrylate casting of human choroid.

The functional anatomy of the normal choroidal circulation in man is described from scanning electron microscopic examination of methyl methacrylate microvascular casts. Distal and para-optic short posterior ciliary arteries supply wedge-shaped areas of choroid. Regional variations in choriocapillary morphology are well recognised; the lobules are densely packed at the posterior pole with a high capillary to inter-capillary ratio. Choroidal capillaries are flattened providing a large surface area for metabolic exchange with the retinal pigment epithelium. In acute choroidal ischaemia (from a v…

Malignant melanoma of the choroid presenting as choroidal detachment following cataract surgery.

The progression of a monocular nuclear cataract was observed for thirty-two months in an eye which was apparently otherwise normal. Three weeks after uncomplicated cataract surgery the patient presented with an apparent detachment of the choroid which enlarged dramatically within a relatively short time and which was further complicated by a retinal detachment. The subsequent development of rubeosis iridis and increased intraocular pressure suggested some other etiology. Ultrasonography revealed a malignant melanoma which was confirmed following enucleation. Transillumination of all suspected …

Choroid plexus: Insights from distinct epithelial cellular components.

The choroid plexus (ChP) serves as a vital interface between blood and cerebrospinal fluid (CSF), playing a pivotal role in central nervous system (CNS) development and communication with the body. This review mainly summarizes how the ChP epithelial cells respond to physiological and pathological stimuli, emphasizing the role of distinct organelles and key molecular signaling pathways. Additionally, we discuss the roles of ChP cilia, an evolutionary conserved organelle whose function is still under investigation. Understanding these processes is essential for elucidating how ChP function modu…

Identifying Angiogenic Factors in Pediatric Choroid Plexus Papillomas.

(1) Background: Choroid plexus papillomas (CPPs) are rare brain tumors that tend to occur in very young children. Mechanisms of CPP development remain unelucidated. Separately, the process of angiogenesis has been implicated in other primary brain tumors. We hypothesize that angiogenesis is a hallmark of CPP biology. This study aims to identify and validate angiogenic factors in CPPs. (2) Methods: Cerebrospinal fluid (CSF) and CPP tumor samples are collected. A multiplex immunoassay panel is used to identify differentially expressed cytokines in the CSF samples. Concurrently, patient-derived p…

The structure and function of the human choroid.

In this manuscript, the structure of the human choroid is reviewed with emphasis of the macro- and microscopic anatomy including Bruch's membrane, choriocapillaris, Sattler's and Haller's layer, and the suprachoroid. We here discuss the development of the choroid, as well as the question of choroidal lymphatics, and further the neuronal control of this tissue, as well as the pathologic angiogenesis. Wherever possible, functional aspects of the various structures are included and reviewed.

Choroid plexus volume differentiates MS from its mimics.

This study aimed to determine whether choroid plexus volume (CPV) could differentiate multiple sclerosis (MS) from its mimics. A secondary analysis of two previously enrolled studies, 50 participants with MS and 64 with alternative diagnoses were included. CPV was automatically segmented from 3T magnetic resonance imaging (MRI), followed by manual review to remove misclassified tissue. Mean normalized choroid plexus volume (nCPV) to intracranial volume demonstrated relatively high specificity for MS participants in each cohort (0.80 and 0.76) with an area under the receiver-operator characteri…

Molecular genetics and diversity of choroid plexus tumors.

Choroid plexus tumors are rare intraventricular brain tumors predominantly arising in children but also affecting adults. Chromosome-wide copy-number alterations and TP53 mutations do occur, but in most choroid plexus tumors, driver mutations have not been identified. Here we give a brief overview of the histopathological and clinical diversity of choroid plexus tumors and their genetic and epigenetic heterogeneity. Preliminary data indicate that choroid plexus carcinomas comprise at least 2 epigenetic subgroups, one of which is associated with TP53 mutation status. These findings strongly enc…

Light-sensitive Ca2+ signaling in the mammalian choroid.

The choroid is the thin, vasculature-filled layer of the eye situated between the sclera and the retina, where it serves the metabolic needs of the light-sensing photoreceptors in the retina. Illumination of the interior surface of the back of the eye (fundus) is a critical regulator of subretinal fluid homeostasis, which determines the overall shape of the eye, but it is also important for choroidal perfusion. Noted for having some of the highest blood flow rates in the body, the choroidal vasculature has been reported to lack intrinsic, intravascular pressure-induced (myogenic) autoregulator…

Aquaporin-4 expression in the human choroid plexus.

The choroid plexus (CP) consists of specialized ependymal cells and underlying blood vessels and stroma producing the bulk of the cerebrospinal fluid (CSF). CP epithelial cells are considered the site of the internal blood-cerebrospinal fluid barrier, show epithelial characteristics (basal lamina, tight junctions), and express aquaporin-1 (AQP1) apically. In this study, we analyzed the expression of aquaporins in the human CP using immunofluorescence and qPCR. As previously reported, AQP1 was expressed apically in CP epithelial cells. Surprisingly, and previously unknown, many cells in the CP …

Regulation of choroid plexus development and its functions.

The choroid plexus (ChP) is an extensively vascularized tissue that protrudes into the brain ventricular system of all vertebrates. This highly specialized structure, consisting of the polarized epithelial sheet and underlying stroma, serves a spectrum of functions within the central nervous system (CNS), most notably the production of cerebrospinal fluid (CSF). The epithelial cells of the ChP have the competence to tightly modulate the biomolecule composition of CSF, which acts as a milieu functionally connecting ChP with other brain structures. This review aims to eloquently summarize the cu…

Experimental approaches for manipulating choroid plexus epithelial cells.

Choroid plexus (ChP) epithelial cells are crucial for the function of the blood-cerebrospinal fluid barrier (BCSFB) in the developing and mature brain. The ChP is considered the primary source and regulator of CSF, secreting many important factors that nourish the brain. It also performs CSF clearance functions including removing Amyloid beta and potassium. As such, the ChP is a promising target for gene and drug therapy for neurodevelopmental and neurological disorders in the central nervous system (CNS). This review describes the current successful and emerging experimental approaches for ta…
