Clinical review of non-invasive ventilation.

Non-invasive ventilation (NIV) is the mainstay to treat patients who need augmentation of ventilation for acute and chronic forms of respiratory failure. The last several decades have witnessed an extension of the indications for NIV to a variety of acute and chronic lung diseases. Evolving advancements in technology and personalised approaches to patient care make it feasible to prioritise patient-centred care models that deliver home-based management using telemonitoring and telemedicine systems support. These trends may improve patient outcomes, reduce healthcare costs and improve the quali

Controlled trial of a collaborative primary care team model for patients with diabetes and depression: rationale and design for a comprehensive evaluation.

BACKGROUND: When depression accompanies diabetes, it complicates treatment, portends worse outcomes and increases health care costs. A collaborative care case-management model, previously tested in an urban managed care organization in the US, achieved significant reduction of depressive symptoms, improved diabetes disease control and patient-reported outcomes, and saved money. While impressive, these findings need to be replicated and extended to other healthcare settings. Our objective is to comprehensively evaluate a collaborative care model for comorbid depression and type 2 diabetes withi

Treatment barriers and preferences among veterans with chronic musculoskeletal pain and alcohol use in primary care.

Chronic pain and alcohol use commonly co-occur and are associated with considerable functional impairment. Many patients with chronic pain present to primary care, and integrated primary care may be well-suited to provide brief, behaviorally focused treatment. Little is known about behavioral health treatment barriers and preferences among primary care patients with chronic pain and alcohol use. Veterans enrolled in Veterans Health Administration primary care with a chronic musculoskeletal pain diagnosis and past-year alcohol use were identified via electronic medical record review and mailed

Challenges in the clinical implementation of a biopsychosocial model for assessment and management of orofacial pain.

Distress, suffering and care-seeking behaviour are characteristics of pain-related disease and illness. Pain that transitions from an acute to a chronic phase carries with it the potential of further effects: these include a worsening of the disease or illness; high-impact chronic pain; and substantial personal, societal and economic burden. The biopsychosocial model directly addresses these multiple processes, yet clinical frameworks supporting this model are not universally implemented. This paper explores barriers to clinical implementation of a full biopsychosocial framework for temporoman

[Defining interfaces to improve quality of care: the example of anorexia nervosa in acute somatic hospitalisation].

Specialisation in medicine requires multidisciplinary approaches, and hence coordination in collaborations of the different partners involved. These integrated approaches, sometimes called "disease management", fit particularly well to chronic diseases. Our institution introduced an integrated approach for taking care of the acute somatic hospitalisation of patients suffering from anorexia nervosa. Interfaces with the different partners were defined, specifying tasks, rights, and duties of each person, care givers or patients. This initiative allows now to identify any deviation occurring in t

A Person-Centered Hepatitis B Approach Using the COM-B Model to Understand Barriers and Facilitators of Hepatitis B Testing, Management and Treatment.

Over 250 million individuals live with chronic hepatitis B worldwide. More work is needed to address care gaps associated with chronic hepatitis B. Behavioural theories can assist in understanding gaps and aid in the development of effective interventions to reach elimination goals. Using the COM-B model, a sample of HBV-related direct messages from social media platforms from 2021 to 2023 was collected from individuals directly impacted by hepatitis B. Qualitative analysis was employed for social media messages using a guided codebook and the COM-B model. A sample of 168 unique data points we

Developing an interprofessional collaboration for COPD patients in primary care: a participatory action research approach.

Chronic obstructive pulmonary disease (COPD) is a respiratory disease which may significantly impact health status. To reduce symptoms and improve quality of life, pharmacological treatment should be complemented by addressing extrapulmonary traits and lifestyle- and psychosocial factors, such as physical deconditioning, decrease in muscle mass, smoking or depression. Treatment of these non-pharmacological traits is commonly conducted in a primary care setting and often requires multiple healthcare providers (HCPs). To provide complementary care, high quality interprofessional collaboration (I

Development of a prognostic model for predicting depression severity in adult primary patients with depressive symptoms using the diamond longitudinal study.

BACKGROUND: Depression trajectories among primary care patients are highly variable, making it difficult to identify patients that require intensive treatments or those that are likely to spontaneously remit. Currently, there are no easily implementable tools clinicians can use to stratify patients with depressive symptoms into different treatments according to their likely depression trajectory. We aimed to develop a prognostic tool to predict future depression severity among primary care patients with current depressive symptoms at three months. METHODS: Patient-reported data from the diamo

Derivation and validation of a predictive model for chronic stress in patients with cardiovascular disease.

BACKGROUND: Chronic stress in patients with cardiovascular disease (CVD), including peripheral artery disease (PAD), is independently associated worse outcomes. A model that can reliably identify factors associated with risk of chronic stress in patients with CVD is needed. METHODS: In a prospective myocardial infarction (MI) registry (TRIUMPH), we constructed a logistic regression model using 27 patient demographic, socioeconomic, and clinical factors, adjusting for site, to identify predictors of chronic stress over 1 year. Stress at baseline and at 1-, 6- and 12-month follow-up was measure

Team-based Care for Cardiac Rehabilitation and Exercise Training in Heart Failure.

The estimated cost of treating patients with HF in the United States is expected to more than double by 2030.65 This forecast of the impact of HF in the United States should serve as a call to action. Despite well-documented benefits, participation in exercise training and CR programs by patients with HF remains low. In this article, standards and guidelines for exercise and CR in HF were reviewed. Although traditional CR had core components, it lacked care management specific for HF. Chronic stable HF patients can safely exercise; however, there are many unique needs that are not currently ad

An institutional ethnography of chronic pain management in family medicine (COPE) study protocol.

BACKGROUND: Patients with chronic conditions and multiple comorbidities represent a growing challenge for health care globally. Improved coordination of care is considered essential for providing more effective and cost-efficient care for these patients with complex needs. Osteoarthritis is one of the most common and debilitating chronic conditions, is the most frequent cause of chronic pain yet osteoarthritis care is often poorly-coordinated. Primary care is usually the first contact for patients requiring relief from chronic pain. Our previous work suggests discordance between the policy goa

Delirium and neuropsychological recovery among emergency general surgery survivors (DANE): study protocol for a randomized controlled trial and collaborative care intervention.

BACKGROUND: Delirium is a complex neuropsychiatric syndrome which consists of acute and varying changes in cognition and consciousness. Patients who develop delirium are at increased risk for a constellation of physical, cognitive, and psychological disabilities long after the delirium has ended. Collaborative care models integrating primary and specialty care in order to address patients with complex biopsychosocial needs have been demonstrated to improve outcomes in patients with chronic diseases. The purpose of this study is to evaluate the ability of a collaborative care model on the neuro

Self-care in adults with sickle cell disease.

This article summarizes the psychometric evaluation of the Chronic Illness Assessment Interview for Sickle Cell Disease (CIAI-SCD), an instrument based on a model of self-care for adult patients with chronic medical conditions. The CIAI-SCD was administered to 104 adults with sickle cell disease. A factor analysis identified three factors that reflected the psychological constructs of Personal Satisfaction and Perceived Control (Factor 1), Feeling Concerned and Worried (Factor 2), and Feeling Supported (Factor 3). Preliminary evidence for the internal consistency, test-retest reliability conte

Consensus Statement for Clinical Pathway Development for Perioperative Pain Management and Care Transitions.

The perioperative surgical home (PSH) model has been created with the intention to reduce costs and to improve efficiency of care and patient experience in the perioperative period. The PSH is a comprehensive model of care that is team-based and patient-centric. The team in each facility should be multidisciplinary and include the input of perioperative services leadership, surgical services, and support personnel in order to provide seamless care for the patient from the preoperative period when decision to undergo surgery is initially made to discharge and, if needed after discharge from the

Protocol to Create Chronic Wounds in Diabetic Mice.

Chronic wounds develop as a result of defective regulation in one or more complex cellular and molecular processes involved in proper healing. They impact ~6.5M people and cost ~$40B/year in the US alone. Although a significant effort has been invested in understanding how chronic wounds develop in humans, fundamental questions remain unanswered. Recently, we developed a novel mouse model for diabetic chronic wounds that have many characteristics of human chronic wounds. Using db/db-/- mice, we can generate chronic wounds by inducing high levels of oxidative stress (OS) in the wound tissue imm
