Chylothorax after adult cardiac surgery: a comprehensive narrative review.

BACKGROUND: Chylothorax is a rare but serious complication of adult cardiac surgery, associated with respiratory compromise, metabolic derangements, and prolonged hospitalization. Its management remains challenging due to the lack of standardized guidelines and the complex anatomical variability of the thoracic duct. METHODS: This narrative review synthesizes available literature on postoperative chylothorax in adults. A total of 58 published cases were analyzed to identify patterns of presentation, treatment strategies, and clinical outcomes. Descriptive and comparative statistical analyses w…

Chylothorax in thoracic oncology: diagnostic challenges and management strategies.

Chylothorax is a distinctive form of pleural effusion characterized by the accumulation of chyle within the pleural space. We conducted an analysis of published evidence concerning oncological chylothorax, encompassing complications following thoracic surgery or spontaneous occurrences directly linked to thoracic malignancies. Diagnosis can be established based on clinical features of the pleural effusion and through analysis of pleural fluid. The presence of chylomicrons, measured by lipoprotein electrophoresis, triglyceride levels, and cholesterol content are indicative factors. In cases of …

Left Chylothorax Without Ascites in a Liver Transplant Patient.

Chylothorax is a rare condition that results from thoracic duct disruption with malignant and nonmalignant etiologies manifesting as a pleural effusion. Typically, chylothorax in the setting of cirrhosis is associated with the migration of chylous ascites. We present the case of a 64-year-old male with prior liver transplant who presented with new-onset transudative chylothorax without chylous ascites who responded to transjugular intrahepatic portosystemic shunt revision, diuresis, and serial thoracentesis.

Disseminated tuberculosis presenting as bilateral chylothorax: A case report.

Tuberculosis, being an airborne disease with a broad spectrum of symptoms and signs, is a rare cause of chylothorax. Mortality due to tuberculous chylothorax is 6%, and bilateral chylothorax has a higher mortality with a poor prognosis. A previously well 28-year-old male with a household contact history of tuberculosis exposure presented with a 1-month history of shortness of breath, fever, pleuritic chest pain, and constitutional symptoms. He was found to have bilateral chylothorax, generalized lymphadenopathy, hepatosplenomegaly, and miliary nodules in chest x-ray. Sputum acid-fast bacilli, …

A case report on chylothorax post penetrating chest trauma.

INTRODUCTION AND IMPORTANCE: Chylothorax is a rare condition secondary to a chyle leak from the thoracic duct. The most common cause is after thoracic and cardiac surgery. Other causes include malignancy, non-iatrogenic trauma, and miscellaneous disorder - tuberculosis, filariasis and idiopathic conditions. The incidence of chylothorax post non-iatrogenic trauma is low; however, it does occur. Therefore, clinicians managing trauma patients should be aware of chylothorax as a differential and understand how it can be managed. PRESENTATION OF CASE: Our patient presented in respiratory distress …

Chylothorax in newborns after cardiac surgery: a rare complication?

UNLABELLED: The aim of this study was to analyze patients diagnosed with chylothorax after congenital heart disease surgery among a cohort of neonatal patients, comparing the evolution, complications, and prognosis after surgery of patients who were and were not diagnosed with chylothorax, and to analyze possible risk factors that may predict the appearance of chylothorax in this population. Retrospective and observational study included all neonates (less than 30 days since birth) who underwent congenital heart disease surgery in a level III neonatal intensive care department. We included inf…

High-Output Chylothorax: Novel Imaging Techniques Provide Specific Treatment.

Postesophagectomy chylothorax is a therapeutic dilemma. Understanding anatomy and leakage site is essential, although imaging techniques for thoracic lymphatics are challenging and not always available. We describe a patient with a left-sided thoracic duct and chylothorax after esophagectomy. We performed intranodal Lipiodol lymphangiography, computed tomography lymphangiography, and fluorescence-guided thoracic duct ligation with robot-assisted thoracic surgery. These techniques are useful in the postoperative setting but may also be used as preventive strategies to reduce morbidity and morta…

Dietary treatment of congenital chylothorax with skimmed breast milk.

BACKGROUND: Congenital chylothorax (CC) is a rare but potentially life-threatening condition in newborns. It is defined as an accumulation of chyle in the pleural cavity. The few publications regarding medical management and therapeutic dietary intervention motivated us to share our experience. METHODS: Neonates diagnosed with congenital chylothorax and treated at Innsbruck Medical University Hospital between 2013 and 2020 (n = 6, gestational age: 36 3/7, 32 5/7, 36 4/7, 35 0/7, 35 4/7, 37 3/7 weeks) were eligible for this report. The cornerstones of treatment for chylothorax conventionally c…

CD56-positive Angioimmunoblastic T-cell Lymphoma Complicated by Chylothorax.

A 77-year-old woman presented with systemic lymphadenopathy and bilateral pleural effusion. Angioimmunoblastic T-cell lymphoma (AITL) was diagnosed based on the results of a lymph node biopsy. AITL cells expressed the aberrant antigen of CD56. The bilateral pleural effusion was attributed to chylothorax, not the infiltration of lymphoma cells into the pleura, as determined by the pleural fluid analysis. We therefore diagnosed her with CD56-positive AITL complicated by chylothorax. She achieved complete remission by multidrug chemotherapy. AITL is commonly complicated by pleural effusion, but r…

Unusual lung involvements of invasive mucinous adenocarcinoma with chylothorax.

A 77-year-old man who had a persistent productive cough for one month was admitted to our hospital. Chest computed tomography (CT) revealed subpleural nodular opacities, irregular pleural thickening with bilateral basal predominance, and a small right pleural effusion. Aspirated fluid was exudative and had the appearance of chylothorax without malignant cells. Surgical lung biopsy specimen showed focal proliferation of neoplastic epithelial cells with lepidic-predominant pattern and abundant mucus in the alveolar spaces, consistent with invasive mucinous adenocarcinoma (IMA). The results of PD…

Ten-Year Clinical Experience on Chylothorax after Cardiovascular Surgery.

BACKGROUND: Chylothorax or pseudo-chylothorax is a serious complication after adult and pediatric cardiac surgery. This study presents our 10-year clinical experience of chylothorax after cardiac surgery. METHODS: Between January 2008 and February 2019, 4896 cardiovascular surgeries were performed in 2 tertiary clinics, with 416 patients in the pediatric age group (8.4%). Chylothorax and pseudo-chylothorax were detected in 47 patients (22 adult and 20 pediatric patients, 4.8%). Pseudo-chylothorax was seen in 5 adult patients. In 27 patients, a pleural effusion developed on the left side (64.2…

Pregnancy Complicated by Gorham-Stout Disease and Refractory Chylothorax.

Introduction Gorham-Stout Disease (GSD) is a rare disorder of bony destruction due to lymphangiomatosis, and is often triggered by hormones. One complication of GSD is the development of chylothorax, which carries a high mortality rate. Very little experience has been published to guide management in GSD during pregnancy to optimize both fetal and maternal health. Case Study A 20-year-old woman with known GSD presented with shortness of breath at 18 weeks of pregnancy, due to bilateral chylothoraces which required daily drainage. To minimize chylous fluid formation, she was placed on bowel res…

Bilateral chylothorax: an unusual complication of cervical rib resection.

Bilateral chylothorax is a rare cause of pleural effusions. Here we report an unusual acute presentation of bilateral chylothorax following thoracic outlet surgery. Unique to this case was the disparate characteristics of pleural fluid analyses with an exudate on the left and a transudate on the right. This report describes the recognition and management of bilateral chylothoraces, an uncommon but potentially serious complication of this frequently performed surgical procedure.

Fluid overload deteriorate chylothorax: evaluation in a canine model.

No conservative treatments for chylothorax have yet been established, and surgical ligation of the thoracic duct is required in many cases. In the present study, we investigated the management of body fluid in a canine chylothorax model. Twelve beagle dogs were divided evenly into three groups: A, B, and C. Under general anesthesia, the thoracic duct was cut and opened, and the amount of lymph fluid leakage was measured. Intravenous extracellular fluid infusion was started at 5mL/kg/h for the first 2h, and then between 2 and 4h, the infusion rate was increased to 10 mL/kg/h in group A and to 2…

Early surgical management of chylothorax complicated by Gorham's disease.

Gorham's disease is a rare disorder and its etiology remains unknown. It is characterized by the proliferation of vascular structures in bone, and results in progressive osteolysis. Chylothorax is a severe complication of Gorham's disease with a mortality of 50%. We report a 14-year-old boy with Gorham's disease complicated by chylothorax who underwent early surgical management followed by radiotherapy.

Bilateral chylothorax after blunt thoracic trauma: a case report.

Traumatic chylothorax other than iatrogenic thoracic duct injury is extremely rare in children. Chylothorax can cause cardiopulmonary abnormalities and significant nutritional, metabolic and immunologic consequences. The management of chylothorax ranges from conservative treatment to surgical intervention. We present a four-year-old boy who presented with respiratory difficulty due to multiple rib fractures and bilateral chylothorax, which developed after a blunt chest trauma. The patient was successfully treated through a conservative approach with total parenteral nutrition, nothing by mouth…

Report of three cases: congenital chylothorax and treatment modalities.

Chylothorax is the most common cause of pleural effusion in the newborn. We report three patients with congenital chylothorax and discussed the clinical course and treatment options. Cases 1 and 2 with congenital chylothorax were treated by chest tube placement and total parenteral nutrition (TPN), and were fed a formula rich in medium-chain triglyceride. They were discharged home without any sequelae. Our 3rd case with chylothorax did not respond to the conventional therapies. Octreotide infusion was tried without any benefits and necessitated surgical intervention, but the infant developed c…
