Identification of hierarchical factors to guide clinical decision making for successful long-term pulp capping.

OBJECTIVE: Clinicians have few quantitative studies that rank the in vivo pulp capping effects of commonly used restorative materials. These factors were investigated to provide guidance to clinicians. METHOD AND MATERIALS: One hundred sixty-one standardized pulp-exposed cavities were prepared in nonhuman primate teeth. Exposed pulps were capped with calcium hydroxide, resin-modified glass-ionomer cements, and resin composites. Teeth were collected from 7 to 720 days to observe a full range of responses. Pulpal reactions were categorized according to the standards set by the International Sta

[Clinical thinking and decision making in practice: a full-term neonate with misunderstood respiratory insufficiency].

A fullterm newborn boy developed severe respiratory insufficiency, multiple air leaks and severe pulmonary hypertension, leading to his death on the third day of life. Family history revealed that a sister of the patient had died earlier after a similar course with respiratory problems. The most common causes of respiratory insufficiency could be subsequently excluded. After extensive postmortem investigation alveolar proteinosis was found in the lung tissue. DNA investigation was then performed in the parents, and both appeared to be heterozygotic for the 121ins2 mutation. This finding sugges

Capsulectomy Following Breast Implant Removal: Evidence-Based Indications and a Framework for Clinical Decision-Making.

The question of when capsulectomy is genuinely indicated at the time of breast implant removal has important implications for clinical practice, surgical risk, and patient counselling. This review defines the evidence-based indications for capsulectomy, evaluates the evidence for each of the additional contexts in which it is commonly proposed, and provides a clinical decision framework to assist surgeons in counselling patients and documenting clinical reasoning. No published study directly compares outcomes of explantation with capsulectomy versus explantation alone; the most rigorous prospe

The patient perspective in benign foregut surgery guidelines: bridging evidence, values, and clinical decision-making.

BACKGROUND: Patient values influence preference-sensitive decisions in benign foregut (upper gastrointestinal) surgery, but their incorporation into guideline development is unclear. This systematic review evaluated how patient perspectives are included in clinical practice guidelines (CPGs) and whether involvement is associated with quality. METHODS: Databases and society websites were searched for CPGs addressing endoscopic or surgical management of benign foregut disease from January 2010 to March 2026. Two reviewers screened records, extracted data, mapped involvement across the 10-stage

Impact of blood sampling time deviations on vancomycin therapeutic drug monitoring and clinical decision-making.

OBJECTIVE: To evaluate the impact of deviations in blood sampling time on the interpretation of measured trough concentrations in vancomycin therapeutic drug monitoring; to explore a reference time window for early blood sampling in clinical settings where trough concentration monitoring remains the primary approach. METHODS: A retrospective analysis was conducted on 160 therapeutic drug monitoring (TDM) records from inpatients receiving vancomycin TDM in our hospital between March 2024 and March 2026. Among these, 137 early-sampled cases were included for analysis, with predicted trough conc

The use of pharmacodynamic results for recommended phase II decision making in oncology clinical trials.

INTRODUCTION: Pharmacodynamic analyses are increasingly important to early-phase oncology drug development, however these analyses use invasive methods to collect patient material. Yet, their role in guiding recommended phase II dose (RP2D) decisions remains unclear. Therefore, this review evaluates how pharmacodynamic analyses assisted the determination of the recommended phase II dose (RP2D) in first-in-human cancer trials of small-molecule agents. METHOD: First-in-human dose finding clinical trials of single-agent small-molecule anticancer therapies published between 2022 and 2025 that inc

Treatment effects prediction and clinical decision-making system for retinal vein occlusion by artificial intelligence.

Retinal vein occlusion (RVO) is a chronic retinal vascular disease that often requires repeated anti-VEGF injections and long-term follow-up. However, predicting treatment responses across different follow-up timepoints remains clinically challenging. To address this issue, we developed an AI system integrating generative adversarial networks (GANs), UNet + +, and ResNet-101 to generate post-treatment OCT and fundus images and support clinical decision-making. A total of 2304 OCT and 576 fundus images from 576 RVO patients were collected at baseline and at weeks 4, 12, and 24 after treatment. …

Artificial intelligence for predicting perioperative anaesthetic complications and supporting clinical decision-making: a scoping review.

Perioperative anaesthetic complications are a major public health concern, as they are associated with increased mortality, prolonged hospital stay, higher healthcare costs, and compromised patient safety. Despite advances in perioperative monitoring and risk stratification, early and personalised identification of patients at risk remains limited. Artificial intelligence (AI) has emerged as a promising approach to improve prediction of perioperative anaesthetic complications, although current evidence is heterogeneous. To synthesise the available evidence on the use of artificial intelligence

Urinary Biomarkers in Urological Oncology: From Biological Origin to Clinical Decision-Making-A Narrative Review.

Urinary biomarkers are promising tools in urological oncology because urine can be collected non-invasively and repeatedly during diagnosis, treatment, and surveillance. Despite extensive biomarker discovery, their adoption in routine practice remains limited. Biomarker performance depends not only on analytical accuracy but also on the clinical decision, anatomical route into urine, biological source of the signal, specimen fraction, and timing and conditions of sampling. This review evaluates urinary biomarkers across bladder cancer, upper-tract urothelial carcinoma, prostate cancer, and ren

Interpreting acute postoperative MRI after lumbar spinal fusion: a practical framework for clinical decision-making.

INTRODUCTION: Magnetic resonance imaging (MRI) is frequently performed in the acute postoperative period following lumbar spinal fusion in patients with neurological deterioration or unexpected pain. However, interpretation is often challenging due to expected postoperative changes and metal artifact, which may obscure or mimic clinically significant pathology. In the absence of a structured interpretive approach, this can lead to diagnostic uncertainty and potentially unnecessary reoperation. METHODS: A narrative review of the literature was performed focusing on acute postoperative MRI find

ELN vs. IPSS-M in MDS/AML: Which Prognostic System Should Guide Clinical Decision-Making?

The recognition of myelodysplastic syndrome/acute myeloid leukemia (MDS/AML) as a distinct overlap entity by the International Consensus Classification (ICC) has introduced new challenges in prognostication and therapeutic decision-making. Defined by 10-19% blasts in the absence of AML-defining genetic abnormalities, MDS/AML occupies a biological continuum between myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML) and is characterized by unique molecular features, patterns of clonal evolution, and clinical behavior that are not fully captured by prognostic systems originally deve
