Beyond coincidence: a case series on the clinical course of coinciding cluster headache and short-lasting unilateral neuralgiform headache attacks.

INTRODUCTION: Limited research has been conducted on co-occurring cluster headache and short-lasting unilateral neuralgiform headache attacks (SUNHA). OBJECTIVES: This case series aims to explore whether the observed overlap of cluster headache and SUNHA reflects a coincidental co-occurrence or suggests a shared underlying mechanism while also providing insights to improve diagnosis and management of these patients. METHODS: A retrospective case series of 10 patients with a diagnosis of both cluster headache and SUNHA. RESULTS: Two patients exhibited simultaneous onset of cluster headache a…

Tooth pain accompanying cluster headache in a middle-aged female: A case report.

Some orofacial pains (OFP) resemble primary headache disorders, but involve the trigeminal nerve V2 or V3 dermatome. The International Class … tonomic signs and symptoms. The pain persisted despite dental treatments, leading to a diagnosis of cluster headache. The OFP initially matched the Type 1 description, but later changed to match the Type 2 description. This case highlights the need to differentiate nonodontogenic from odontogenic pain for accurate diagnosis and treatment. Key words:cluster headach …

Neurotransmitter Imbalance in Cluster Headache: A Systematic Review of Mechanisms and Therapeutic Targets.

INTRODUCTION: Cluster headache (CH) causes brief but extremely severe head pain, yet we still do not fully understand the molecules that trigger these attacks. Many studies have looked at changes in neurotransmitters and neuropeptides, but their results do not always agree. This systematic review brings together all the available data on neurochemical changes in CH, with the goal of clarifying current knowledge gaps and highlighting promising targets for future research. METHODS: We searched PubMed, Embase (Ovid), and Scopus for studies reporting quantitative measurements of neurotransmitters…

Uncovering the neurological substrates underlying restlessness in cluster headache - A functional MRI study.

BACKGROUND: Restlessness or agitation is one of the core symptoms of cluster headache (CH). However, the neurological substrate underlying this phenomenon has not been thoroughly analyzed. Whether they are attributed to the core aggression circuit or other CH-related structures remains unclear. The aim of this study is to use functional neuroimaging to elucidate the underlying mechanism of restlessness or agitation in CH. METHODS: We prospectively recruited consecutive patients with CH from the Headache Clinic of Taipei Veterans General Hospital between Jan 2022 and July 2025. Patients who co…

Noninvasive Vagus Nerve Stimulation for Cluster Headache and Migraine: A Health Technology Assessment.

BACKGROUND: Cluster headache and migraine are 2 distinct types of primary headache that can cause substantial pain, disability, and decreased quality of life. Noninvasive vagus nerve stimulation (nVNS) is a treatment option that delivers a mild electrical stimulation to a nerve in the neck. nVNS is intended to reduce the pain and duration of a headache attack, and to prevent headaches from occurring. We conducted a health technology assessment of nVNS for the acute treatment and prevention of cluster headache or migraine, which included an evaluation of effectiveness, safety, cost-effectivenes…

Cluster Headache Presenting As Sinonasal Pathology in a Young Adult: A Diagnostic Odyssey.

Cluster headache is a debilitating primary headache disorder marked by severe, unilateral pain often accompanied by autonomic symptoms. We describe the case of a 20-year-old student who presented with excruciating peri-orbital pain localized to the right side, accompanied by ipsilateral nasal obstruction, a nasal spur, and a deviated nasal septum (DNS). The initial clinical picture strongly suggested sinonasal pathology, leading to investigations and treatments aimed at this presumed diagnosis. However, as the patient's symptoms persisted and evolved over time, with episodes of recurrent and i…

Galcanezumab effectiveness on comorbid cluster headache and chronic migraine: a prospective case series.

BACKGROUND: Cluster headache (CH) and migraine are recurrent painful primary cephalalgies, typically with different clinical appearance and some shared features, such as unilateral pain, common triggers and response to triptans and/or monoclonal antibodies against the calcitonin gene-related peptide (CGRP) pathway. Common pathophysiological mechanisms are proposed in CH and migraine, including the role of the trigeminal-vascular system with its most representative neuropeptide, CGRP. Very few case series have been conducted so far investigating anti-CGRP treatments in patients with comorbid CH…

Management of cluster headache and other trigeminal autonomic cephalalgias in pregnancy and breastfeeding.

Many clinicians lack experience in managing trigeminal autonomic cephalalgias (TACs) in pregnancy and lactation. In addition to cluster headache, TACs include hemicrania continua, paroxysmal hemicrania, and short-lasting unilateral neuralgiform headache with conjunctival injection and tearing/autonomic symptoms (SUNCT/SUNA). Treating these rare, severe headache conditions often requires off-label drugs that have uncertain teratogenic potential. In the last few years, several new treatment options and safety documentation have emerged, but clinical guidelines are lacking. This narrative review …

Changes in grey matter volume and functional connectivity in cluster headache versus migraine.

Cluster headache (CH) shows a more severe clinical picture than migraine (Mig). We tested whether brain changes can explain such difference. Multimodal MRI was acquired in attack-free patients with CH (n = 12), Mig (n = 13) and in normal controls (NC, n = 13). We used FSL for MRI data analysis and nonparametric permutation testing for voxelwise analyses (p < 0.01, corrected). CH showed lower grey matter (GM) volume, compared to Mig and NC, in frontal cortex regions (inferior frontal gyrus and frontal pole [FP], respectively) and, only compared to Mig, in lateral occipital cortex (LOC). Functio…

Altered Hypothalamic Region Covariance in Migraine and Cluster Headache: A Structural MRI Study.

OBJECTIVES: The hypothalamus plays a key role in both migraine and cluster headache (CH). As brain region-to-region structural correlations are believed to reflect structural and functional brain connectivity patterns, we assessed the structural covariance patterns between the volume of the hypothalamic region and vertex-by-vertex measurements of cortical thickness in patients with migraine and in those with CH relative to healthy controls (HC). METHODS: T1-weighted images were acquired on a 3T MRI scanner for a total of 59 subjects including 18 patients with CH (age: mean = 43.8, SD = 12.4),…

Pupil-sparing cranial nerve III palsy after intranasal cocaine treatment for cluster headache.

This case report discusses a cranial nerve III palsy in a 47-year-old, type II diabetic man that originally presented with a cluster headache that was treated with 100% oxygen by nasal cannula, 975 mg Tylenol, and 100 mg of Imitrex without any symptom relief. He then received a sphenopalatine fossa block using 1 ampule of 4% cocaine. Three days after medicinal, intranasal cocaine for treatment of a cluster headache, the patient presented with a cranial nerve III palsy with spontaneous resolution in 4 months without any intervention. Previously, intranasal cocaine had been standard treatment fo…

Spatial Distribution of Temporalis Pressure Pain Sensitivity in Men with Episodic Cluster Headache.

(1) Background: Spatial changes in pressure sensitivity have been described in migraine and tension-type headaches. Our aim was to determine differences in the spatial distribution of pressure pain sensitivity of the temporalis muscle between cluster headache (CH) patients and headache-free controls; (2) Methods: Pressure pain thresholds (PPTs) were determined over nine points covering the temporalis muscle in 40 men with episodic CH and 40 matched headache-free controls in a blinded fashion. Topographical pressure pain sensitivity maps were constructed based on interpolation of the PPTs. Pati…

Cluster-like headache and a cystic hypothalamic tumour as first presentation of sarcoidosis.

INTRODUCTION: Sarcoidosis is a granulomatous, multisystem inflammatory disease of unknown cause, which presents with a wide variety of symptoms. We describe a rare case of a newly diagnosed sarcoidosis, with cluster-like headache as a presenting symptom. CASE: A 31-year-old man presented with cluster headache with a cystic lesion in the hypothalamus. A non-caseating granuloma consistent with the diagnosis sarcoidosis was found at biopsy. Pulmonary involvement was confirmed on positron electron tomography-computed tomography (PET-CT). Treatment with prednisone led to regression of the hypothal…

Greater occipital nerve blocks in chronic cluster headache: a prospective open-label study.

BACKGROUND AND PURPOSE: Greater occipital nerve blockade (GONB) has been shown to be effective in episodic cluster headache. However, its use in chronic cluster headache (CCH) is less certain. The study aims to prospectively assess the efficacy and consistency of response to GONB in a large series of CCH patients. METHODS: CCH patients who had a unilateral GONB and were referred to the National Hospital for Neurology and Neurosurgery were studied prospectively. Headache characteristics were collected using headache diaries. Responders were considered to be patients with a complete or partial …

Aggressiveness in different presentations of cluster headache: results from a controlled multicentric study.

BACKGROUND: The hypothalamus has been discussed as a pivotal structure for both cluster headache (CH) and aggressiveness, but little is known about the extent of self-reported aggressiveness in patients with CH. PATIENTS AND METHODS: Twenty-six patients with chronic, 25 with active episodic and 22 with episodic CH outside the active period were examined interictally with a validated questionnaire quantifying factors of aggression and compared with 24 migraine patients and 31 headache-free volunteers. RESULTS: The ANOVA was significant for the subscale 'self-aggression/depression' (F(4, 123) …
