Prognostic indicators in a Danish hospital-based population of colic horses.

A prospective survey of 528 colic horses, referred to the Large Animal Hospital at the Royal Veterinary and Agricultural University of Copenhagen, Denmark, during the period August 1994 to December 1998, was undertaken to develop a predictive model for application in the clinical assessment of prognosis. In the multivariate logistic regression analysis, 357 colic cases were used in the elaboration of a simple clinical-practical model consisting of degree of pain, packed cell volume, capillary refill time and rectal temperature. The relationship between rectal temperature and outcome (survival/…

Trends in the use of unenhanced helical CT for acute urinary colic.

OBJECTIVE: Unenhanced helical CT for urolithiasis detection is a limited CT examination that was designed specifically for the detection of urolithiasis. The purpose of this study was to repeat a prior study to assess whether clinicians had broadened the indications and changed the yield and findings of unenhanced helical CT. MATERIALS AND METHODS: One hundred consecutive patients with suspected renal colic or flank pain referred for unenhanced helical CT were selected for this study. We reviewed the original radiographic reports for each patient and recorded the presence of ureteral calculi.…

The best management for 'crescendo biliary colic' is urgent laparoscopic cholecystectomy.

Gallbladder disease due to stones is well recognised as falling into two categories, presenting with either chronic symptoms or developing acute cholecystitis or other complications. We describe an intermediate group of 14 patients (11 women, three men, median age 31 years) presenting with 4-14 days of at least daily attacks of resolving biliary colic, who underwent early laparoscopic cholecystectomy within 24 hours of presentation. None had any evidence of acute inflammation, either at laparoscopy or on histology. Their surgery was straightforward with operating times ranging from 35-80 minut…

Squamous cell carcinoma of the gallbladder presenting with a biliary-colic fistula.

Pure squamous carcinoma of the gall bladder is a rare tumour which may present diagnostic difficulty for the histopathologist. A case presenting as a biliary-colic fistula is described and the diagnostic histological features discussed. Theories regarding aetiology and the prognosis of this rare tumour are also discussed.

Normality: a clinically useless concept. The case of infant crying and colic.

To summarize, it has been argued that: 1. The assessment of complaints about crying and colic present particular diagnostic problems. 2. The crying brought as a complaint seldom indicates disease. 3. Once clinical disease has been ruled out, the clinical meanings of normality and abnormality no longer apply. 4. At that point, one should not try to determine a "cut-off" point for abnormal crying, because (a) it is unhelpful clinically, (b) it is wrong in principle, and (c) it is not likely that any specific amount of crying is normal or abnormal, independent of context. 5. As a possible alterna…

Intestinal carcinoid in a mare: an etiologic consideration for chronic colic in horses.

Intestinal carcinoid, or argentaffinoma, should be an etiologic consideration for horses with chronic colic. A mare was referred with a history of chronic colic. Previously, the signs of colic had subsided in response to impiric treatment, but recent episodes of colic did not. Clinical signs and results of physical examination supported the finding of strangulating obstruction of the small intestine. Exploratory celiotomy revealed internal herniation with simultaneous volvulus of the jejunum and accompanying carcinoid.

Cow's milk as a cause of infantile colic in breast-fed infants.

18 mothers of 19 breast-fed infants with infantile colic were put on a diet free of cow's-milk protein. The colic disappeared promptly from 13; in 12, it reappeared on at least two further indirect challenges (in the form of a diet containing cow's milk to the mother). Most infants became symptom-free at age 2 to 4 months; at 4 months, only 4 reacted with colic when the mother took cow's milk. 5 infants were directly challenged with cow's milk; 4 reacted promptly with colic. Other signs of intolerance to cow's-milk protein developed in 3 infants during weaning. The treatment of infantile colic…

Severe Renal Colic as a Trigger of Coronary Thrombosis on Plaque Erosion.

BACKGROUND: Acute myocardial infarction may result from the complex interplay between transient systemic triggers and vulnerable coronary substrates. CASE SUMMARY: A 37-year-old man presented with persistent chest pain after severe renal colic. Electrocardiography demonstrated lateral ST-segment elevation with diffuse ST-segment depression. Emergency coronary angiography revealed thrombotic occlusion of the proximal left anterior descending artery. Optical coherence tomography (OCT) identified extensive thrombus overlying a mildly atherosclerotic segment without evidence of plaque rupture, co…

Impact of evidence-based information on horse owners' misconceptions of colic.

BACKGROUND: Misconceptions can be defined as 'false, persistent beliefs' or 'inaccurate, prior knowledge' and can influence decision-making. OBJECTIVES: To investigate the impact of evidence-based information on UK horse owners' decision-making for colic. STUDY DESIGN: Mixed-methods cross-sectional study. METHODS: An online survey was distributed to UK horse owners, with four sections: owner demographics; views on decision-making and referral to an equine hospital facility for colic; current knowledge and approach to colic; impact of evidence-based information (including evidence on recogni…
