Alcohol-clozapine synergism triggering rhabdomyolysis at subtoxic levels: a case report and literature review.

BACKGROUND: While excessive oral intake of clozapine is known to cause severe complications, this case report highlights that even conventional doses of clozapine ingested after alcohol consumption-without reaching toxic concentrations-can lead to the rare complication of rhabdomyolysis when synergistic effects occur between the two substances. CASE PRESENTATION: We report a case of a 39-year-old male patient who presented with impaired consciousness 1 day after consuming 500 g liquor followed by ingestion of 8 clozapine tablets (200 mg total). Toxicology screening upon admission indicated su

Emergency superficial temporal artery-superior cerebellar artery bypass for hyperacute basilar artery occlusion following elective middle cerebral artery aneurysm clipping: illustrative case.

BACKGROUND: Basilar artery occlusion (BAO) is a threatening condition requiring rapid reperfusion to preserve brainstem function. Although endovascular thrombectomy is considered standard treatment, underlying pathology or limited resources may require surgical intervention. The authors performed an emergency superficial temporal artery-superior cerebellar artery (STA-SCA) bypass in the hyperacute setting of BAO following elective aneurysm surgery. The bypass prevented stroke progression and dramatically improved consciousness. LESSONS: This case demonstrates that with early recognition, prot

Responsiveness variability during anaesthesia relates to inherent differences in brain structure and function of the frontoparietal networks.

Anaesthesia combined with functional neuroimaging provides a powerful approach for understanding the brain mechanisms of consciousness. Although propofol is used ubiquitously in clinical interventions that reversibly suppress consciousness, it shows large inter-individual variability, and the brain bases of this variability remain poorly understood. We asked whether three networks key to conscious cognition-the dorsal attention (DAN), executive control (ECN), and default mode (DMN)-underlie responsiveness variability under anaesthesia. Healthy participants (N = 17) were moderately anaesthetize

Remifentanil Alleviates Propofol-Induced Burst Suppression without Affecting Bispectral Index in Female Patients: A Randomized Controlled Trial.

The bispectral index is affected by various factors, such as noxious stimuli and other drugs, such as muscle relaxants. The burst suppression ratio from bispectral index monitoring is correlated with electroencephalographic burst suppression, which is associated with deep anesthesia, metabolic disorders, and brain injury. We assessed patients undergoing total intravenous anesthesia and examined the effects of remifentanil on the bispectral index, burst suppression ratio, and hemodynamic changes immediately after loss of consciousness with propofol. Seventy American Society of Anesthesiologists

Brexanolone for Postpartum Depression: Clinical Evidence and Practical Considerations.

Our aim was to review the efficacy, safety, and pharmacology of brexanolone (Zulresso), a new antidepressant with a novel mechanism of action, in the treatment of postpartum depression (PPD). Pertinent data and information were obtained via PubMed (1993 to August 2018). Articles published in English that evaluated the safety and efficacy of brexanolone and other off-label PPD treatments were included. Literature regarding epidemiology and pathophysiology of PPD was also selected. Brexanolone, administered as an intravenous infusion over 60 hours, produced a statistically significant and clinic

A Fatal Case of Amlodipine Toxicity Following Iatrogenic Hypercalcemia.

Using calcium salts in management of amlodipine overdose is challenging. A 25-year-old male with known history of adult polycystic kidney disease presented with hypotension, tachycardia, and intact neurological status after ingestion of 450 mg of amlodipine. Immediately, normal saline infusion and norepinephrine were initiated. Two grams of calcium gluconate was injected, followed by intravenous infusion of 1.16 mg/kg/h. The patient was put on insulin-glucose protocol to maintain euglycemia and hyperinsulinemia. Electrocardiography demonstrated junctional rhythm. Serum creatinine was 2.5 mg/dL

Does yawning represent a transient arousal-shift during intravenous induction of general anesthesia?

UNLABELLED: Although yawning occurs frequently during the IV induction of general anesthesia, the significance of this response remains unknown. In this study, we induced 30 surgical patients with 4 mg/kg thiopental IV, and 30 patients with 2 mg/kg propofol IV. Thereafter, the occurrence of yawning was continuously assessed, as the only clinical end-point, for 1 min. The electroencephalographic bispectral index was monitored throughout the observation period. The criterion for an arousal response was a transient increase during a continuing decrease in the bispectral index value. On the basis

[Case of prolonged recovery from serotonin syndrome caused by paroxetine].

We report a case of serotonin syndrome in a patient being treated with paroxetine for depression. Despite prompt discontinuation of medication, his serotonin syndrome continued for 10 days before full consciousness was restored. The patient was a 48-year-old male with chief complaints of hypobulia and suicidal thoughts. He consulted as a psychiatric outpatient, and oral paroxetine 20 mg/day, etizolam 1.0 mg/day, and brotizolam 0.25 mg/day were immediately started. Upsurge of feeling and disinhibition state were noted the following day, then on treatment day 6 his condition deteriorated to subs

[Syncope as a cardiologic problem].

Syncope is a frequent clinical event. It is mainly caused by a suddenly reduced cerebral blood flow. There are two reasons for sudden cerebral underperfusion: cardiogenic - associated with cardiac disorders and neurocardiogenic - resulting from a sudden fall of arterial blood pressure due to impaired autoregulation of the circulation. Cardiogenic syncopes prevail in cardiac diseases associated with impaired blood flow and cardiac arrhythmias. They develop in aortic stenosis, hypertrophic cardiomyopathy, atrial myxoma, myocardial infarction, pulmonary embolism, cardiac tamponade. Cardiac arrhyt

Cerebral Oxygen Dynamics in a Patient After Subdural Haematoma Removal Surgery: A Case Report.

Near-infrared spectroscopy (NIRS) can be employed for the non-invasive real-time evaluation of cerebral oxygen dynamics. A portable wireless NIRS system has been developed to record measurements using a band worn around the head. In this case report, we describe the clinical findings and cerebral oxygen dynamics of a patient who underwent surgery to remove a left temporal subdural haematoma. The patient was a male in his 70s, who fell while riding a bicycle. Following the gradual onset of gait disturbance, the patient was admitted to our hospital for surgery. After surgical removal of the subd

[Usefulness of diffusion-weighted magnetic resonance imaging in diagnosis and therapeutic strategy of deep cerebral venous thrombosis: a case report].

We present the usefulness of Diffusion-weighted magnetic resonance images (DWI) in diagnosis and therapeutic strategy of deep cerebral venous thrombosis (VT). We report a 37-year-old man who suffered general convulsion and deep coma. Magnetic resonance imaging (MRI) showed high intensity in the right caudate nuclei and bilateral thalamus on T2-weighted images. DWI showed slightly high intensity in the same area and apparent diffusion coefficient (ADC) is normal or slightly high (0.748-1.100 x 10(-3) mm2/s). Cerebral angiography showed occlusion of straight sinus from basal veins and internal c

A therapy against vasospasm after subarachnoidal haemorrhage: clinical experience of balloon angioplasty.

Vasospasm is a phenomenon often present in human cases after subarachnoidal haemorrhage. Its aetiology and pathophysiology are unknown. As a result, cases of vasospasm do not respond to any known pharmacological therapy. We present in this paper, a practical application of intravascular surgery for vasospasm. In this procedure the requirement for a portable digital subtraction unit, transcranial Doppler sonography, and being able to determine quickly the neurological state of the patient are very important points. The use of monitoring equipment is especially indispensable during the carrying

A Clinical Exploration of One of the Longest Documented Ventricular Pauses in an Ambulatory Patient.

Cardiac conduction system abnormalities, such as sinus nodal dysfunction and AV block, can lead to significant bradyarrhythmias and pauses, potentially causing symptoms like dizziness and loss of consciousness. Monitoring these rhythm disturbances is crucial, as they may evolve and indicate underlying structural heart disease or lead to more serious arrhythmias. We present the case of a 76-year-old man with an exceptionally prolonged 62-second ventricular pause, followed by a single ventricular beat and an additional 24-second pause before normal rhythm resumes. He presented recurrent, intermi

[Aims of rehabilitation after successful resuscitation].

After successful resuscitation, cases of persisting neurological deficits or prolonged intensive care usually require inpatient neurological rehabilitation treatment. Depending on the severity this is either early neurological and neurosurgical rehabilitation in phase B as a continuation of acute hospital treatment or neurological rehabilitation in phase C or D. To relieve the burden on intensive care and monitoring wards in acute care hospitals, phase B often initially involves intensive care with weaning from ventilation and decannulation. In this phase, quantitative and qualitative disturba
