Encephaloceles are pathological herniations of brain parenchyma through congenital or acquired osseus-dural defects of the skull base or cranial vault. Although encephaloceles are known as rare conditions, several surgical reports and clinical series focusing on spontaneous encephaloceles of the temporal lobe may be found in the otological, maxillofacial, radiological, and neurosurgical literature. A variety of symptoms such as occult or symptomatic CSF fistulas, recurrent meningitis, middle ear effusions or infections, conductive hearing loss, and medically intractable epilepsy have been desc

A 13-year-old girl was admitted for headache, right periorbital swelling and erythema. CT imaging demonstrated right orbital preseptal cellulitis, severe pansinusitis and suspected epidural abscess. Brain MRI and sinus CT confirmed an epidural abscess in the right middle cranial fossa and a second extra-cranial abscess in the right infratemporal fossa along with large right sphenoidal emissary foramen. Drainage from sinus surgery confirmed allergic fungal rhinosinusitis. She was treated with prednisone and voriconazole.

A detailed understanding of the dural anatomy of the trigeminal nerve, particularly the Meckel's cave, and the petrous apex is essential to accomplish an interdural approach in dumbbell-shaped trigeminal schwannomas (TSs) from the middle fossa side. A 34-year-old gentleman with left hemifacial pain and masseter weakness on examination was found to have a dumbbell-shaped TS around the petrous apex occupying the adjacent middle cranial fossa, posterior fossa, and the cavernous sinus. We performed an extended middle fossa approach (EMFA), taking an interdural route, and with the addition of an ex

CASE SUMMARY: An 11-year-old neutered male Maine Coon cat was presented for investigation of anisocoria and depression. Neurological examination was consistent with a lesion at the level of the middle cranial fossa, and biochemistry was indicative of moderate renal functional impairment. MRI of the brain identified an extra-axial mass lesion at the level of the middle cranial fossa, T2-weighted hyperintense and strongly homogeneously contrast enhancing with dural tail. The cat was euthanased after 6 weeks of palliative treatment with corticosteroids. Histopathology and immunohistochemistry of

OBJECTIVE: Cholesteatoma of the petrous bone extending into the intracranial region is an unusual occurrence. Most cases have been attributed to secondary extension of a primary epidermal blastomatous malformation of the temporal bone into the middle or posterior fossae. Within the past two and a half decades, intracranial extension of acquired aural cholesteatoma has been recognized as a likely alternative to this mechanism. Recent literature has rejoined this observation by considering both primary and secondary cholesteatoma of the petrous bone as a single group, petrosal cholesteatoma. The

BACKGROUND AND OBJECTIVES: The intricate anatomy of the posterior fossa comprises layered muscles, fasciae, and both extracranial and intracranial neurovascular structures. Surgical intervention in this region requires a precise and comprehensive understanding of these structures, which can be difficult to achieve with only 2-dimensional materials. Traditional cadaver dissections provide basic 3-dimensional anatomic insight but are increasingly constrained by cost, ethical concerns, and difficulties in cadaver sourcing. Advances in 3-dimensional (3D) modeling, particularly through photogrammet

BACKGROUND AND OBJECTIVE: Trigeminal schwannomas (TSs) are rare, mostly benign tumors that exhibit an indolent clinical course. The lateral orbitotomy approach (LOA), with or without endoscopic assistance, has emerged as a novel skull base route, particularly for accessing lesions confined to the middle cranial fossa. Herein, we elucidate the advantages and limitations of the LOA for the treatment of TSs.
METHODS: All TS cases that underwent LOA through a lateral canthus incision during the past decade in our department were retrospectively reviewed. The operative technique was detailed, and

BACKGROUND: Dural arteriovenous fistulas (DAVFs) are rare in pediatrics. A case of DAVF diagnosed because of a slight conjunctival hyperemia and endovascular coil embolization at 2 years old is reported.
CASE DESCRIPTION: The 2-year-old boy presented with a slight conjunctival hyperemia of the left eye for 1 month. Magnetic resonance imaging (MRI) examination of the head showed abnormal blood flow in the left middle cranial fossa. On digital subtraction angiography, a DAVF with a dural feeder shunt and a venous varix at the middle cranial fossa was confirmed. After transarterial coil emboliza

BACKGROUND: Extrdural granulation or abscess is a possible complication of chronic suppurative otitis media (CSOM) with cholesteatoma. However, due to development of newer antibiotics and advances in bacteriology and imaging techniques, the incidence of extradural granulation or abscess has significantly decreased. The present study analyzed the clinical presentation, imaging findings, and surgical treatment of eight patients with CSOM with cholesteatoma extending to the cranial fossa.
PATIENTS AND METHODS: From 2000 to 2012, 1,010 patients were surgically treated for CSOM with cholesteatoma.

The foramen spinosum is one of the foramina of the skull that lies bilaterally close to the spine of the sphenoid in the middle cranial fossa. The recurrent meningeal branch of the mandibular nerve, middle meningeal artery, and vein usually passes through it. The hypoglossal canal, situated superior to the occipital condyles at the skull base, is a paired bony canal directed anterolaterally and serves as a conduit for the hypoglossal nerve, meningeal branch of the ascending pharyngeal artery, and emissary veins communicating with the basilar venous plexus. Duplication of the foramen spinosum a

AIM: In this study, we aimed to provide our initial experience with a novel combined approach resection of chondrosarcomas of the skull base and petrous apex (ChPA) and posterior cranial fossa by employing middle cranial fossa and retrosigmoid (RS) approaches with an augmented reality and virtual reality (AR/VR) system.
CASE PRESENTATION: A 66-year-old female patient was referred to our department owing to the growth of a left petrous apex lesion noted in computed tomography (CT) and magnetic resonance imaging (MRI) scans 10 years earlier. Her symptoms included headache, imbalance, and left-s

Transpalpebral or trans-eyelid approach is a modified trans-orbital access to lesions of anterior cranial fossa and sellar region. But whether this approach is also suitable for tumors extending laterally to the temporal lobe or middle cranial fossa is not clarified. We would like to share our experiences from the cadaveric anatomy study to clinical operations. We used 5 cadavers to study trans-eyelid approaches in a step-by-step fashion. And then assisted by an experienced ophthalmologist for incisions, we treated 3 female patients via this approach: One with spheno-orbital meningioma, one wi

OBJECTIVE: Temporal lobe encephaloceles are characterized by protrusion of brain parenchyma through a structural defect in the floor of the middle fossa. They have been reported to cause cerebrospinal fluid (CSF) leaks, conductive hearing loss, meningitis, and seizures. The association between temporal encephaloceles and epileptiform activity is particularly rare.
PATIENTS: All patients who presented to a single tertiary referral center between 2011 and 2014 with intractable seizures and radiographic evidence of a middle cranial fossa encephalocele were evaluated. Five patients from this subs

The ninth case of condylar dislocation into the middle cranial fossa is reported. The previously reported cases are reviewed and the clinical findings which they have in common are pointed out. Because difficulty in establishing the diagnosis seems to be the most serious aspect of this clinical entity, it is advised that films of the temporomandibular joint be taken more frequently. Finally, although treatment varied from physical therapy to intracranial condylectomy, our case of closed withdrawal of the condyle from the cranial fossa is the first case on record so treated.

OBJECT: As in patients with vestibular schwannomas, advances in surgical procedures have markedly improved outcomes in patients with trigeminal schwannomas. In this article the authors address the function of cranial nerves in a series of patients with trigeminal schwannomas that were treated with gross-total surgical removal. The authors emphasize a technique they use to remove a dumbbell-shaped tumor through the expanded Meckel cave, and discuss the advantage of the extradural zygomatic middle fossa approach for total removal of tumor and preservation or improvement of cranial nerve function
