Seronegative Disseminated Cryptococcosis: A Case Report.

Cryptococcal infection is a major cause of morbidity and mortality in immunocompromised patients, especially those with HIV/AIDS. The cryptococcal antigen (CrAg) lateral flow assay (LFA) has become an essential diagnostic tool due to its high sensitivity, specificity, and ability to produce rapid results. However, this test is not without limitations. This case details a patient with disseminated cryptococcosis with a negative serum CrAg LFA and an unremarkable brain MRI to demonstrate the importance of cerebrospinal fluid testing in high-risk immunocompromised individuals.

Disseminated Skeletal Cryptococcosis: A Case Report.

INTRODUCTION: Cryptococcus is a rare cause of osteomyelitis, especially in immunocompromised individuals. This case report discusses a rare case of disseminated cryptococcosis with multiple bone lesions in a patient with isolated CD4 lymphocytopenia. CASE REPORT: A 31-year-old apparently normal Indian male presented with pain and swelling of his right proximal femur for 8 months without any history of trauma. He also reported a similar swelling in his chest wall with allergic respiratory symptoms for 8 years. Laboratory analysis revealed mild elevation in inflammatory markers. Magnetic resona…

Disseminated cryptococcosis presenting as lung mass.

Cryptococcosis is an opportunistic fungal infection in immunocompromised individuals, with a disease spectrum ranging from asymptomatic pulmonary infection to fatal meningoencephalitis. This case report describes the case of a man with diabetes in his late 40s who presented with chest pain and generalised weakness. Contrast-enhanced CT of the chest revealed a mass lesion in the left upper lobe. Ultrasound-guided biopsy of the mass revealed a diagnosis of cryptococcosis. Cerebrospinal fluid and blood samples tested positive for cryptococcal antigens. A final diagnosis of disseminated cryptococc…

Pulmonary cryptococcosis in chronic kidney disease.

Pulmonary cryptococcosis is a fungal infection caused by Cryptococcus species, with Cryptococcus neoformans being the most common agent, affecting the lungs. While it commonly occurs in immunocompromised individuals, such as those with HIV/AIDS or undergoing immunosuppressive therapy, its presentation in patients with chronic kidney disease (CKD) is relatively rare. However, it should be considered in the differential diagnosis of respiratory infections in patients with CKD, particularly in the context of immunosuppression.

Disseminated Cryptococcosis in Idiopathic CD4+ Lymphocytopenia.

INTRODUCTION: Idiopathic CD4+ Lymphocytopenia (ICL) is a rare entity grouped in non- HIV-related syndromes. ICL is characterized by a marked … tory. Based on clinical findings and investigations, she was diagnosed with ICL having disseminated cryptococcosis. Unfortunately, the patient did not undergo specific treatment as she was recognized late, and unfortunately, she died. CONCLUSION: It is of paramount importance to recognize the clinical entity as early as possible to start appropriate treatment, …

Gastrointestinal Cryptococcosis Associated with Intestinal Lymphangiectasia.

Intestinal lymphangiectasia is a pathological dilation of enteric lymphatic vessels resulting in lymph leakage to the intestinal lumen. This chronic lymph leakage leads to a state of immunosuppression secondary to the loss of humoral and cellular components of the immune system and represents a potential risk factor for opportunistic infections. We report a case of protein-losing enteropathy in a seemingly immunocompetent patient. An intestinal histopathological study revealed the unusual association of lymphangiectasia and intestinal cryptococcosis. Although cryptococcal infection is common i…

Cerebral cryptococcosis with pseudo-meningiomatous revelation?

We report the case of a 28-year-old woman with history of lupus treated by Mycophenolate Mofetil and admitted for febrile meningeal syndrome associated with diplopia. Both initial and control brain MRI showed pseudo-meningiomatous lesion while second CSF microscopy revealed characterizing images of Cryptococcus neoformans.

Cryptococcosis in solid organ transplant recipients.

PURPOSE OF REVIEW: Cryptococcosis among solid organ transplant (SOT) recipients is a source of significant morbidity. Its pathogenesis, the etiology of immune reconstitution syndrome, and the optimal therapy in this setting are still not well defined. Herein, we review the epidemiology, the latest findings on pathogenesis, unique clinical manifestations, and the treatment of Cryptococcosis in this specific vulnerable population. RECENT FINDINGS: Cryptococcosis is a common fungal complication among SOT recipients. It follows in frequency only to aspergillosis and candidiasis. Cryptococcal infe…

Pulmonary cryptococcosis in non-AIDS patients.

OBJECTIVE: To investigate the clinical features, management, and prognosis of pulmonary cryptococcosis in non-acquired immunodeficiency syndrome (AIDS) patients. METHOD: 24 cases of pulmonary cryptococcosis with accurate pathological diagnosis were retrospectively studied. RESULTS: 15 male patients and nine female patients were diagnosed at the first affiliated hospital of Sun Yat-sen University from November 1999 to November 2011. The mean age at the time of diagnosis was 44.2±11.3 years (range: 24 to 65 years). Among these patients, 13 had other comorbidities. 15 were symptomatic and the o…

Disseminated cryptococcosis presenting with generalized lymphadenopathy.

Cryptococcosis is a common opportunistic infection among immunocompromised individuals. Some of the commonly affected sites are respiratory and central nervous system. Lymph node is an unusual site of involvement which could mimic tuberculosis, as seen in our case. We report a 32-year-old male immunocompromised patient presenting with generalized lymphadenopathy who was clinically suspected to have tuberculous lymphadenitis. He was diagnosed to have disseminated cryptococcosis on fine needle aspiration cytology and fungal isolation on culture.

Induction of protective immunity against cryptococcosis.

Cryptococcus neoformans, the predominant etiological agent of cryptococcosis, is an encapsulated fungal pathogen that can cause life-threatening infections of the central nervous system in immune compromised individuals resulting in high morbidity and mortality. Consequently, several studies have endeavored to understand those mechanisms that mediate resistance and susceptibility to Cryptococcus infection. In this review, we will examine the contributions of various components of the innate and adaptive immune response toward protection against cryptococcosis. We will focus our discussion on s…

[Pulmonary cryptococcosis: analysis of 38 cases].

OBJECTIVE: To investigate the clinical features, radiology, diagnosis and treatment of pulmonary cryptococcosis. METHODS: A total of 38 cases of pulmonary cryptococcosis, confirmed by pathological examinations at Fuzhou General Clinical Medical College, Fujian Medical University from March 2003 to February 2010, were retrospectively studied. RESULTS: All of the cases were community-acquired. The patients consisted of 29 males and 9 females, aged from 21 to 70 years. There were no underlying diseases in 29 cases. The CD(4) cell numbers were normal in 20 patients. Radiological study showed tha…
