PURPOSE: The first mutation associated with nonsyndromic deafness has recently been identified in pedigrees with susceptibility to aminoglycoside ototoxicity and in a large Arab-Israeli pedigree. The mutation is maternally transmitted, and is a nucleotide substitution in the mitochondrial 12S ribosomal RNA gene. A different sequence change, in the mitochondrial tRNA(Ser)(UCN)/COI gene, has been proposed as a candidate mutation in a Scottish nonsyndromic deafness pedigree. We have now identified a family in New Zealand with maternally inherited nonsyndromic sensorineural deafness, and the purpo…

The paper reports the case of a young woman who presented with a serious bilateral sudden deafness, after the ingestion of a betalactam antibiotic (known as allergic for her) in association with a mucolytic. Account of the course of the clinical events noticed, which are compared with the findings observed in other scarce publications on sudden deafness linked with allergic reactions. Finally the AA. outline a pathogenetic hypothesis in order to explain these cases of deafness involved with digestive allergic reactions.

Autoimmune deafness represents a heterogeneous group of disorders in which there is though to be inner ear and/or middle ear damage mediated by various immunological abnormalities. We describe a patient with Wegener's granulomatosis in whom there was evidence of significant inner ear damage caused by vasculitis, which was completely reversed by cyclophosphamide and prednisolone. We propose that Wegener's granulomatosis is a cause of autoimmune deafness.

A prospective audiometric evaluation of 69 hospitalized febrile patients in Sierra Leone, West Africa, revealed a sensorineural hearing deficit (SNHD) in 14 (29%) of 49 confirmed cases of Lassa fever and in 0 of 20 febrile controls. An SNHD was present in nine (17.6%) of 51 people who had evidence of previous Lassa virus infection. Twenty-six of 32 local residents who had previously sustained a sudden deafness had antibody titers to Lassa virus of 16 or greater, compared with six of 32 matched controls. Lassa fever is associated with an incidence of SNHD, which considerably exceeds that previo…

In an examination of 624 prelingual deaf children in the Christian Institute for deaf children 'Effatha' in Voorburg special attention was paid to deafness caused by meningitis. This diagnosis was made in 70 persons. In 41 of these the pathogen of meningitis could be traced. Streptococcus pneumoniae is the main causative organism of meningitis and subsequent deafness (60%). Haemophilus influenzae, Escherichia coli and Neisseria meningitidis follow with 12, 9 and 7% respectively. We emphasize the timely audiologic control of all children who suffered a meningitis.

In 56 patients with senile deafness, the degree of bone atrophy was quantitatively assessed by microdensitometry (MD). Biochemical examinations were also made. The following results were obtained: 1. Abnormal bone metabolism is suspected in many patients with senile deafness. 2. Serum calcium level is significantly lower and BUN significantly higher in patients with senile deafness than in normal subjects. 3. A significant positive correlation is noted between hearing levels and serum alkaline phosphatase levels in patients with senile deafness. After treatment of 12 patients suspected of havi…

Today's otologic surgeon has the opportunity to restore normal serviceable hearing in patients with congenital conductive deafness using tympanoplasty with ossicular chain reconstruction, stapedectomy, or fenestration of the horizontal semicircular canal. The particular surgical approach used is determined by the abnormalities found in the congenitally deformed ear. I report a series of 18 patients who had operation for congenital conductive deafness during a four-year period at the Farrior Clinic. The study results show the efficacy of surgical reconstruction in producing an improvement in he…

Musical hallucinations associated with acquired deafness are rarely reported in the psychiatric literature. In the few cases studied no evidence of psychiatric disturbance is noted. The authors describe two patients with musical hallucinations while in a state of nonpsychotic depression. One of these suffered from acquired deafness and no organic pathology was demonstrable in the other. Cessation of musical hallucinations occurred concomitantly in both patients with improvement of the affective state. Depression might have a contributory role in the occurrence of such hallucinations.

While it is well known that mumps is a potential cause of deafness, it is not generally appreciated that it can produce total deafness. The damage is usually unilateral but bilateral disease has been recorded. A case is presented of severe bilateral hearing loss during a mumps epidemic in which, in spite of intensive conventional as well as lesser known therapy, no improvement occurred. In the absence of a successful therapeutic regime it is clearly advisable to intensify immunization in non-developed countries.

A patient presented with a bilateral profound hearing loss of sudden onset following a two-month neurologic illness. Microscopy and culture of cerebrospinal fluid revealed Cryptococcus neoformans. Treatment with amphotericin B and 5-fluorocytosine failed to restore hearing. Auditory brain stem response and electrical promontory stimulation suggest a profound deafness with poor neuronal survival. This is consistent with previous temporal bone histopathology reports in individuals dying of cryptococcal meningitis, suggesting a retrocochlear lesion. It is important to exclude this occult patholog…

Two patients with auditory hallucinations beginning after a long history of progressive bilateral hearing loss were studied. The hallucinations included both unformed (tinnitus and irregular sounds of varying pitch and timbre) and formed (instrumental music, singing and voices) components, and were repetitive. They were affected by ambient noise levels; their content and speed were influenced by attentional and intentional factors. There was no evidence of global dementia, nor of epileptogenic or psychiatric disturbance. A combination of peripheral and associated central "disinhibition" may be

Three cases of ipsilateral deafness occurred in association with herpes zoster ophthalmicus. Although our review of the literature did not disclose any identical cases, we believe that this complication may not be rare. If vertigo, tinnitus, abnormal auditory sensations, or ear pain occur with herpes zoster of any cranial nerve, an audiogram is indicated immediately to detect possible hearing loss. There is no known satisfactory treatment for sudden deafness due to herpes zoster infection.

Initial optimism about the successful outcome of treatment of congenital syphilitic deafness with ampicillin and steroids has always been tempered with an awareness of the uncertain long-term results. This paper is a review of 16 patients treated with various combinations of ampicillin and steroid; these have been followed up for periods of five to 14 years. Ampicillin alone does not prevent progressive hearing loss in the majority of patients. Ampicillin combined with prednisone, or with prednisone and ACTH, has preserved the hearing in all but two ears which had useful hearing before treatme…

Two brothers are described with renal tubular acidosis and nerve deafness: the elder also had rickets and hypokalaemia. The parents were unaffected. Studies of urinary acidification and bicarbonate excretion were consistent with a distal tubular abnormality. This report strengthens the view previously proposed in similar cases that nerve deafness and renal tubular acidosis constitute a genetic entity. Examination for nerve deafness is indicated in any child with renal tubular acidosis.
