BACKGROUND: Internal jugular vein stenosis (IJVS) is an underrecognized cause of cerebral venous outflow obstruction, commonly presenting with nonspecific symptoms such as head-noise, tinnitus, dizziness, headaches, and visual or auditory disturbances. Extrinsic compression by bony structures, particularly transverse process of the atlas (C1), has emerged as a significant but frequently ignored etiology. This study presents the first case series demonstrating that isolated resection of the C1 transverse process can restore venous outflow and provide durable relief of head-noise-dominant sympto

PURPOSE: Double crush syndrome involves two distinct compressive lesions along a single peripheral nerve. Patients with compressive neuropathies at the wrist and elbow may experience exacerbated symptoms from cooccurring cervical radiculopathy. Surgical management aims to decompress at either or both proximal and distal sites. This study compares outcomes of anterior cervical discectomy and fusion (ACDF) alone versus ACDF with subsequent peripheral nerve decompression.
METHODS: This retrospective study evaluated patients with double crush lesions, diagnosed with magnetic resonance imaging-con

CASE: A 33-year-old woman with a 1-year history of right gluteal and pelvic pain due to entrapment of the pudendal nerve by a bifurcate piriformis muscle underwent piriformis tenotomy, pudendal neurolysis, and sciatic neurolysis by an open posterolateral hip approach. At 32-week follow-up, the patient was free of deep gluteal pain and all pudendal neuralgia symptoms.
CONCLUSION: Pudendal nerve entrapment by the piriformis is an uncommon cause of pudendal neuralgia that can be effectively treated with surgical intervention. We demonstrate an open posterolateral hip approach may provide signifi

OBJECTIVE: Interlaminar endoscopic spine surgery has been introduced and utilized for lumbar lateral recess decompression. We modified this technique and utilized it for bilateral lateral recess stenoses without significant central stenosis. Here we present the surgical details and clinical outcome of ligamentum flavum sparing unilateral laminotomy for bilateral recess decompression (ULBRD).
METHODS: Prospectively collected registry for full-endoscopic surgeries was reviewed retrospectively. One hundred eighty-two consecutive cases from a single center between September 2015 and March 2021 we

Bow Hunter's syndrome is a rare cause of posterior circulation ischemia, produced by the mechanical and reversible occlusion of the vertebral artery during cephalic rotation. Diagnosis requires clinical suspicion and careful inspection of images with three-dimensional reconstruction. The study of choice is dynamic digital subtraction angiography (DSA). Treatment alternatives are: medical, surgical or endovascular. We report the case of an 8-year-old boy with recurrent infarctions of the posterior circulation secondary to the dissection of the vertebral artery, in association with an occipital

BACKGROUND: Microvascular decompression of the trigeminal nerve is the most effective treatment for trigeminal neuralgia. However, when encountering classical trigeminal neuralgia caused by venous compression, the procedure becomes much more difficult, and failure or recurrence because of incomplete decompression may become frequent. This study aimed to investigate the anatomic variation of the culprit veins and discuss the surgical strategy for different types.
METHODS: We performed a retrospective analysis of 64 consecutive cases in whom veins were considered as responsible vessels alone or

OBJECTIVE: Facet joint cysts (FJCs) of the lumbar spine are an increasingly reported cause of radiculopathy, lower back pain, and neurologic deficits. Currently, there is a lack of conclusive evidence outlining when a particular treatment should be undertaken and what patient indications suit a particular approach. The present systematic review and meta-analysis aims to evaluate the efficacy of percutaneous treatment and surgical decompression with or without fusion.
METHODS: A systematic literature search of scientific databases from their inception to February 2016 was performed by 2 review

Microvascular decompression (MVD) is effective for the relief of symptoms, but little is known about the impact of the MVD procedure on patient's quality of life (QoL) or which QoL factors are important. The surgical results of MVD and the impact of this procedure were evaluated on patient's QoL in 139 patients, 74 with hemifacial spasm (HFS) and 65 with trigeminal neuralgia (TN), who underwent MVD between 2004 and 2011 using the 36-Item Short Form Health Survey questionnaire. Symptoms had resolved in approximately 95% of patients after MVD. The QoL questionnaire was completed by 54 HFS patien

CONCLUSION: Lateral canal plugging with endolymphatic sac decompression (LCPESD) is a new therapeutic approach to intractable Meniere's disease. Further clinical data will be required.
OBJECTIVE: The objective of this paper is to present a new surgical procedure, i.e. lateral canal plugging (LCP) with endolymphatic sac decompression (ESD), which we defined as LCPESD. Two cases of intractable Meniere's disease, a 48-year-old male and a 48-year-old female, were successfully treated by this method.
METHODS: LCPESD was performed under general anesthesia. First ESD was undertaken followed by LCP.

INTRODUCTION: Quadratus femoris tear is an uncommon injury, which is only rarely reported in the literature. In the majority of cases the correct diagnosis is delayed due to non-specific symptoms and signs. A magnetic resonance imaging scan is crucial in the differential diagnosis since injuries to contiguous soft tissues may present with similar symptoms. Presentation with sciatica is not reported in the few cases existing in the English literature and the reported treatment has always been conservative.
CASE PRESENTATION: We report here on a case of quadratus femoris tear in a 22-year-old G
