Granulomatosis with polyangiitis: potentially lethal gingival lesions presenting to the dentist.

Gingival pathology is a daily presentation, however a small number of systemic conditions can manifest similar to a common gingival conditio … on biopsy. With 5% of cases showing intraoral lesions as the primary feature, it is essential that dentists have the knowledge of this rare disease to refer and not to treat as a common gingival condition.

Perception of Dental Students, Dentists and Patients on the Esthetics of Gingival Recession.

BACKGROUND: This cross-sectional study verified the self perception of different observers on gingival recession. METHODS: Areas of gingival recession were digitally modified using software simulating different clinical conditions in the esthetic area. A total of 180 individuals (60 dentists, 60 patients, and 60 dental students) participated in this study. From an original photograph of the smile of a volunteer, five cases of gingival recession (GR), including unilateral recession (lateral incisor and canine), bilateral recession (lateral incisor and canine), and generalized recession in uppe…

The Effect of New Oral Care Technologies on the Need for Dentists in 2040.

This article addresses changes in technology of oral self-care or professional care that may increase or decrease the demand for dentists by 2040. The focus is on dental caries, periodontitis, and temporomandibular joint disorders (TMD), as the first two are the main areas of current practice and because TMD is an area for growth. To address this question, the authors examined the scientific literature and government registries to identify recent or soon-to-be-available technologies. They also examined the state of translational efficiency, dissemination, and adoption of advances into dental p…

Concomitant sensitization to glutaraldehyde and methacrylic monomers among dentists and their patients.

BACKGROUND: A multitude of methacrylic monomers is used in dentistry. Glutaraldehyde (G) is used in dental practice and consumer products as a broad-spectrum antimicrobial agent. The purpose of our study is to evaluate the frequency and the risk of concomitant sensitization to some methacrylic monomers (methyl methacrylate (MMA), triethyleneglycol dimethacrylate (TEGDMA), ethyleneglycol dimethacrylate (EGDMA), 2,2-bis-[4-(2-hydroxy-3-methacrylo-xypropoxy)phenyl]-propane (Bis-GMA), 2-hydroxy-ethyl methacrylate (2-HEMA) and tetrahydrofurfuryl methacrylate (THFMA)) and glutaraldehyde in students

Premature loss of primary teeth with gingival erythema: An alert to dentist.

Premature exfoliation of primary teeth is an important diagnostic event warranting urgent investigation. The majority of conditions presenting with early loss of teeth are serious and in some cases could be fatal. The most common causes of premature tooth loss are Papillion-Lefevre syndrome, Chediak-Higashi syndrome, hypophosphatasia, neutropenia, leukemia and in some cases Langerhans cell histiocytosis (LCH). LCH is a disorder of unknown cause, characterized by abnormal proliferation of histiocytes. The disease has a predilection for children, although LCH may occur in adults. Owing to the re

Musculoskeletal health of the woman dentist: distinctive interventions for a growing population.

Female dentists face unique musculoskeletal demands and inherent gender differences that may place them at higher risk for occupational pain and injury than their male counterparts. By familiarizing themselves with unique muscle imbalances, specific pain syndromes, and positioning challenges to which they are predisposed, female dentists may take pre-emptive action and initiate ergonomic intervention in the operatory and specific exercise at home to reduce the risk of developing musculoskeletal disorders.

The role of dentists in diagnosing osteogenesis imperfecta in patients with dentinogenesis imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI), also known as "brittle bone disease," can be difficult to diagnose in its mild form. The authors describe a clinical case of a diagnosis of dentinogenesis imperfecta (DI), in which a literature review combined with an analysis of dental alterations led to indications of OI involvement. CASE DESCRIPTION: Since DI can be associated with OI, the authors reviewed correlated studies and obtained a new medical history from the patient. They then conducted a radiographic and clinical examination of the dentition and submitted an affected third molar to scann

A survey of knowledge, attitudes, and behavior of Italian dentists toward immunization.

This study assessed the knowledge, attitude, and compliance to immunization guidelines among dental health-care professionals in Italy. A questionnaire was mailed to a random sample of 1000 dentists on demographic and professional characteristics; knowledge regarding infectious diseases that can be acquired/transmitted by the dentist and the vaccinations recommended; attitudes regarding the vaccinations; self-assessed information about previous vaccinations. A total of 369 dentists responded. Only 44.1 and 32.4% correctly indicated all infections that can be acquired or transmitted during thei…

['Emily goes to the dentist'. Oral care for individuals with Down syndrome].

This article is primarily based on an editorial letter in the Journal of Oral Health and Disability that describes the visits of a patient with Down syndrome named Emily. Oral health care for individuals with Down's syndrome and other people with learning disabilities in The Netherlands is discussed. Due to the syndrome related oral aspects and specificity, the authors argue strongly in favour of working multidisciplinary within oral health care centres. The dependency of persons with Down's syndrome necessitates an appeal to parents, relatives and carers to maintain oral health. Client-centre

Genetic mutations in certain head and neck conditions of interest to the dentist.

This article identifies certain syndromes of the head and neck, which a dentist may see in clinical practice, and relates these syndromes to their sites of mutation on involved genes. This paper is timely with the near completion of the Human Genome Project, the mapping of the entire human genetic material. Knowing the site of the genetic lesion is important in helping clinicians understand the genetic basis for these conditions, and may help in our future understanding of remedies and treatments.

What the dentist should know about a patient with HIV/AIDS.

Chances are good that oral health care providers will treat someone with diagnosed or undiagnosed HIV during their careers. The Centers for Disease Control and Prevention estimate that 650,000 to 900,000 U.S. residents are living with HIV infection, more than 200,000 of whom are unaware of their infection. As with any medical condition, it is possible for a dentist to do great harm by ignoring systemic manifestations of HIV. On the other hand, dentists who are ignorant of modern HIV disease management often request unnecessary medical consultations resulting in dental treatment delays. Since 1…

[Rational use of antibiotics by the dentist can prevent microbial resistance].

Worldwide resistance of microorganisms to antibiotics is becoming a problem of major concern. An antibiotic policy which tries to control the development of resistance by rational use is needed in all fields where antibiotics are prescribed. In dental practice, apart from endocarditisprophylaxis, prophylaxis is leukopenic patients and prophylaxis after dental implants, antibiotics can be used to support local therapy of an dentoalveolar abscess or refractory periodontitis. In the majority of these cases, a small spectrum penicillin is therapy of choice.

Views of cardiac-valve prosthesis patients and their dentists on anticoagulation therapy.

By administering a questionnaire to 253 patients with cardiac-valve prostheses (89.3% responding), and another to 136 of their attending dentists (79% responding), the level of knowledge among both groups of anticoagulant therapy in connection with dental treatment was investigated. The cardiothoracic department monitored all anticoagulation therapies. Of the anticoagulated patients, 96.6% were able to state their medication (94.1% received phenprocoumon); and of 86 dentists with patients on anticoagulation treatment, 94% were aware of their patients' medication. All 20 dentists stating that t…

Latex allergies in children with spina bifida: relevance for the pediatric dentist.

Latex is ubiquitous in pediatric dentistry and medical practice. Children with spina bifida and other urogenital abnormalities are at great risk for hypersensitivity reactions during dental treatment. Four representative cases of children with latex allergies at one institution are presented. A latex-avoidance protocol is presented with suggested instrument and equipment alternatives.

Current strategies for dentist-applied treatment in the management of hypersensitive dentine.

Few of the many dentist-applied treatments for dentine hypersensitivity meet the requirements of an ideal desensitizing agent'. This paper, although not exhaustive, covers methods that may be classified from simplest to most complex. Topical methods are simple, but they are often uncomfortable and the effects usually quite short-lasting, and many fall short of the 'ideal'. Fluorides and oxalates appear to be the best topical methods. Iontophoresis of fluoride represents an intermediate stage of complexity and it meets most of the criteria of an ideal desensitizing agent. Newer restorative mate

Elder abuse and the dentists' awareness and knowledge of the problem--a national survey.

A national sample of 1,775 Canadian dentists, representing 13.2 per cent of all Canadian dentists, were evaluated via a mailed questionnaire about their knowledge, awareness of, and experiences with victims of elder abuse and/or neglect. In all, 83.3 per cent of respondents were aware, to varying extents, of elder abuse or neglect. Of the dentists observing a patient suspected to have been a victim of neglect or abuse, 40.6 per cent reported incidents of neglect, while physical abuse was seen 59.4 per cent of the time. The types of neglect most frequently observed included: neglect of personal…

The impaired dentist. Recognition and treatment of the alcoholic and drug-dependent professional.

An attempt has been made to focus on the gravity of the consequences of alcoholism and/or drug dependence within the dental profession. The … nized our own denial and enabling and accept our responsibilities to help our suffering colleagues. Dentists are just as susceptible to disease as other humans, and the tragedy of a wasted life as a result of alcoholism is inexcusable with our level of knowledge of alcoholism programs today.
