Treatment needs of overdenture patients in a longitudinal study: five-year results.

This study evaluated the treatment needs of 44 overdenture patients who had worn their dentures for at least 5 years. There were 135 abutment teeth involved, 35 in the maxillae and 100 in the mandible. Recall rates varied from a low of 27.6% to a high of 73.7%. The attrition rate was 20.5% after 5 years. The caries rate varied from a low of 2.7% to a high of 20.6%, and periodontal treatment was required by 94% of the subjects after 5 years. Only 39.4% of the subjects required adjustments, repairs, relines, or remakes of their overdentures during this time. The study showed that the success of

Effect of implant number, attachment type, and framework material on stress distribution in mandibular overdentures: a finite element analysis.

BACKGROUND: Using finite element analysis, we evaluated the effects of implant number, attachment type, and framework material on stress distribution in bone and in implant and prosthetic components of mandibular overdentures in completely edentulous mandibles. METHODS: Straumann implants (4.1 mm diameter and 12 mm length) were placed in a completely edentulous mandible in two configurations. For the two-implant models, implants were positioned axially in the lateral–canine region while for the three-implant models, one implant was placed at the midline and two were positioned bilaterally in t

Mandibular overdenture with a single implant in the canine region (c-SIMO): a feasibility study.

OBJECTIVES: The aim of this multi-center pilot study was to assess the viability and feasibility of a novel treatment concept - the canine-positioned single implant mandibular overdenture (c-SIMO), with the single implant placed on the patient's preferred chewing side instead of the midline. MATERIALS AND METHODS: Participants received a single implant in the canine region of their preferred chewing side, based on an Asymmetry Index observed during mastication. The pre-existing mandibular denture was transformed into a c-SIMO on a spherical attachment. The primary outcome was oral health-rela

Enhancing oral function: A case report on mandibular overdenture utilization with custom-made subperiosteal implant.

Subperiosteal implants, previously set aside because of complications, are now emerging again as effective treatments for severe mandibular atrophy, aided by recent improvements in digital dentistry. Traditional dentures in such cases often face challenges with support and retention, necessitating complex regenerative procedures. This paper presents a case report of a 54-year-old male patient with significant mandibular atrophy who received a custom-made subperiosteal implant, showcasing promising results. The implant was precisely designed utilizing computed tomography (CT) scans, a 3D-printe

Dental rehabilitation of the patient with severe caries after radiation therapy.

Dental management of the patient with severe caries after radiation therapy can be accomplished by the general practitioner in an office setting. Because of the decrease in vascularity that results from radiotherapy, the general practitioner must keep the following points in mind when treating these patients: Extraction of teeth that were in the field of radiotherapy is contraindicated. If there is a question about a possible extraction, consultation should be made with the patient's radiotherapist and a hospital-trained dentist (preferably, an oral surgeon). Endodontic treatment is the only t

Use of an esthetic overdenture as an alternative treatment in a patient with bilateral cleft lip and palate.

Stable occlusion and a pleasing esthetic appearance are often difficult to achieve in patients with congenital defects and severe midfacial deficiencies. Conventional therapy, such as orthodontic treatment followed by orthognathic surgery, is often not sufficient to fully correct the dental and esthetic problems. An interdisciplinary approach for these patients should include prosthodontic treatment that will assist in establishing a harmonious occlusion and improve facial appearance. This clinical report describes the interdisciplinary approach for a young patient with a history of bilateral

Edentulous patients' experiences with two-implant overdentures - A qualitative exploration.

INTRODUCTION: The two-implant overdenture is widely recognized as the minimum standard of care for completely edentulous patients. However, limited research has explored patients' lived experiences with this treatment. This study aimed to gain an in-depth understanding of the experiences of individuals treated with this modality. METHODS: An interpretive phenomenological approach was employed to explore participants' perspectives. Completely edentulous patients treated with a mandibular two-implant overdenture opposing a maxillary complete denture at McGill Dental Clinic, Montreal, Canada, we

The role of attachment type and bone height in modulating stress distribution in mandibular overdentures: Insights from finite element analysis.

BACKGROUND: Mandibular two-implant overdentures are considered the standard of care for edentulous patients. The attachment system and the shape of the bone crest can significantly influence biomechanical behaviours. This study aimed to evaluate stress distribution in mandibular implant-supported overdentures using locator and bar attachments across various bone crest designs. MATERIALS AND METHODS: FEA was performed on mandibular overdentures supported by two implants. Three crest configurations (flat, convex, and irregular) were modelled with either bar or locator attachments. A vertical fo

Evaluation of locator versus TITACH attachments for mandibular 2-implant overdentures: a one-year randomized controlled clinical trial.

Implant overdenture attachments serve as the crucial link between dental implants and removable dentures, significantly enhancing the stability, retention, and function of the prosthesis. This study was designed to assess the effectiveness of locator and TITACH attachments in retaining mandibular two-implant overdentures, maximum bite force, and marginal bone loss. The study involved 36 edentulous patients, each receiving new complete dentures (CDs) designed with the bilateral balanced occlusal concept. Two implants were positioned in the mandibular canine regions. Participants were randomly a

[Zirconia in removable prosthodontics. A case report].

Zirconia as a framework material is well established in fixed prosthodontics. However, for its application for removable dentures little experience exists. Zirkonzahn® has developed a copy-milling unit, that is a manually operated machine for the manufacture not only of frameworks but also of complete removable dentures. The aim of this case report is to show the step-by-step clinical and technical fabrication of a zirconia bar on implants and of a corresponding zirconia complete denture. The advantages and disadvantages of the system are presented and problems are critically discussed.

Hypohidrotic ectodermal dysplasia with bilateral impacted teeth at the coronoid process: a case rehabilitated with mini dental implants.

Bilateral migration of teeth into the coronoid process in a patient with ectodermal dysplasia has not been reported in the literature except one report in which severe hypodontia and bilaterally ectopic impacted teeth in the coronoid processes of a nonsyndromic patient occurred. This article presents a 15-year-old female with hypohidrotic ectodermal dysplasia who had surgical removal of bilaterally impacted teeth in the coronoid process and was rehabilitated with a dental implant-retained fixed prosthesis in the mandible and over-denture in the maxilla.
