Sustained autoimmune mechanisms in dermatomyositis.

Dermatomyositis is an autoimmune disease predominantly affecting skin and muscle. Its poorly understood pathogenesis is increasingly being linked to the overproduction of type 1 interferon-inducible gene transcripts and proteins. Mechanisms have been identified by which chronic sustained accumulation of these proteins might occur and thereby injure tissue in dermatomyositis.

[Laboratory diagnostics of polymyositis/dermatomyositis].

Polymyositis/dermatomyositis is chronic systemic inflammatory disease that usually affects the skin and muscle tissue. The diagnosis of polymyositis/dermatomyositis isn't simple, and represents challenge for clinicians, it is based on the integration of results: medical history and clinical presentation, laboratory findings enzymes more or less specific to muscle tissue, myositis-specific autoantibodies and autoantibodies associated with myositis which can be found at other inflammatory connective tissue diseases without myositis, electromyography findings, and biopsy of muscle and skin. Labor…

Nephrotic syndrome and juvenile dermatomyositis.

Juvenile dermatomyositis (JDM) is a rare autoimmune disease characterized by proximal muscle weakness, skin lesions, gastro intestinal, pulmonary, cardiac and small nerve damage. Renal involvement has been rarely reported in JDM. This is the report of a 7-year-old boy presented with nephrotic syndrome (NS) and subsequent renal failure. Clinical manifestations of JDM appeared gradually. Renal manifestations could be considered as a rare initial presentation of JDM.

Hydroxyurea induced dermatomyositis-like eruption.

Medication-induced dermatomyositis (DM) is rare, but a recent review highlighted hydroxyurea (HU) as the most common inciting agent. To aid diagnosis, HU-induced DM-like eruption (HU DM-LE) forms a distinct dermopathy where the typical cutaneous features of DM are without systemic involvement and co-exist with other HU-induced cutaneous findings such as severe xerosis, atrophy, stomatitis, cutaneous and mucosal ulceration and melanonychia. On cessation of HU the DM-LE clears avoiding unnecessary immunosuppression and demonstrating the importance of consideration of medication aetiology in DM p…

Dermatomyositis and type 1 interferons.

Dermatomyositis is a poorly understood multisystem disease predominantly affecting skin and muscle. This review focuses on the potential role of a group of related cytokines, the type 1 interferons, in the pathogenesis of dermatomyositis. Type 1 interferon-inducible transcripts and proteins are uniquely elevated in dermatomyositis muscle compared with all other muscle diseases studied to date. The endothelial cell tubuloreticular inclusions present in affected dermatomyositis muscle are biomarkers of type 1 interferon exposure. The cell-poor lichenoid reaction in skin with predominant involvem…

Amyopathic dermatomyositis resembling stasis dermatitis.

This case describes an unusual presentation of dermatomyositis in a patient with ovarian carcinoma. The eruption appeared as a venous stasislike dermatitis. The temporal sequence of onset after chemotherapy administration suggested a possible drug-induced process. However, in the context of underlying ovarian carcinoma, a paraneoplastic process offered an alternative explanation for the dermatomyositis.

Juvenile dermatomyositis presenting with anasarca.

Generalized edema can occur during the course of juvenile dermatomyositis. In this article, a 4-year-old boy with generalized nonpitting edema and proximal weakness is reported. Characteristic cutaneous lesions, laboratory tests, results of electromyography, and magnetic resonance imaging findings resulted in a diagnosis of juvenile dermatomyositis. He was treated with corticosteroids and methotrexate. It is concluded that the generalized edema may appear as the presenting cutaneous manifestation of juvenile dermatomyositis.

[Lyme disease could mimic dermatomyositis].

UNLABELLED: We report a dermatomyositis associated with Lyme disease. OBSERVATION: A 73-years-old woman has developed for 5 months an asthenia, a periorbital oedema and a forearm's skin infiltration without other signs suggesting of dermatomyositis. Laboratory studies showed an elevation of muscular enzymes, and inflammation signs. The skin and the muscles biopsies were compatible with the diagnostic of dermatomyositis. The patient was seropositive for Lyme disease. The patient was efficiently treated with doxycycline. DISCUSSION: Lyme disease could mimic a dermatomyositis. Indeed, Lyme dise…

Recent advances in juvenile dermatomyositis.

This paper reviews the knowledge we continue to gain in the areas of pathogenesis, evaluation of disease activity, and interventional strategies with regards to idiopathic inflammatory myositis. Potential new diagnostic criteria incorporate the use of magnetic resonance imaging and other imaging modalities along with the use of muscle biopsy pathologic features including that of immunologic markers. Specifically, this criterion reflects the upregulation of major histocompatibility complex molecules on muscle tissue during acute inflammation.
