Severe chemical burns can damage the periocular area causing exposure keratopathy and resulting in ocular pain, foreign body sensation, epiphora, photophobia, and vision loss. A custom fit large diameter therapeutic scleral lens can protect the ocular surface by preventing desiccation and providing immediate improvement in comfort. This case describes the management of a patient with extensive chemical burn with complete lid loss and exposure keratopathy who has maintained excellent visual acuity and comfort with therapeutic scleral lens use.

The aim of tympanoplasty graft preparation is to stiffen the fascia or perichondrium and thereby to optimise ease of manipulation. We report 39 cases utilising a novel technique in which the graft is prepared in ear drops containing polyethylene glycol, flumetasone pivalate (0.02 per cent) and clioquinol (1 per cent). This technique is useful in reducing the risk of desiccation if placement is delayed, and may pose less risk of infection and mechanical damage than alternative methods.

We used wide-field color specular microscopy of the human corneal surface to study the morphologic changes that occur under extended-wear contact lenses. There was a shift in the frequency distribution of surface cells by size to larger cells, suggesting a prolonged residence time on the ocular surface. Other abnormalities included retained desiccated mucin, coarse mucus plaques, and palisading of cells.
