BACKGROUND: In experimental lung injuries, improvement of lung function after treatment with surfactant/polymer mixtures may depend on both type of polymer and the specific surfactant. In vitro studies suggest that dextran is more effective when mixed with Curosurf, and polyethylene glycol (PEG) is more effective when mixed with Survanta. We therefore wanted to find out whether these results held true in an animal model of acute lung injury.

A case of dextran induced adult respiratory distress syndrome is used to illustrate the most serious complication of dextran usage during intracoronary stent placement. This reaction occurred despite premedication with the hapten dextran 1. Mechanisms of dextran induced complications are discussed. A mechanism for hapten resistant dextran reactions is postulated. Suggestions to prevent such reactions are made.

This review describes the properties and side effects of Hyskon and the implications for the patient and anaesthetist during hysteroscopy. By means of light and electron microscopy tissue reactions produced by repeated use of polyvinylpyrrolidone (PVP) and dextran plasma substitutes are studied on the material of 20 autopsies and 4 biopsies as well as in 65 experimental cases. Pharmacologic treatments for otopathologies of possible vascular origin are aimed at enhancing vascular transport and are intended to increase cochlear blood flow (CBF). In the current study, normotensive Wistar-Kyoto (WKY) and spontaneously hypertensive rats (SHRs) were given intravascular infusions of mannitol (20%), dextran 70 (6%), or control saline. In general, the results clearly indicate that both osmotic agents effectively increase CBF, as measured by laser Doppler flowmetry. However, the mechanism of action of these agents is less clear.

High-output left ventricular failure occurred in a patient after a difficult case of hysteroscopic lysis of adhesions using dextran as a distension medium. The excessive dissection in the uterine wall, the long duration of the operation, and the large volumes of dextran probably caused intravasation of dextran into the systemic circulation inducing a significant shift of fluids from the third space. This was possibly assisted by the large volume of fluids given intravenously in a 45-kg patient initiating the reported sequence of events.

The effect of intraperitoneal injection of Dextran Sulfate 500 (DS-500, 500,000 Mr) on the intravenous clearance and host resistance to Rhodococcus equi in mice was studied. In the present study, we compare resuscitation of bled sheep with hypertonic saline/dextran or hypertonic saline/hetastarch. Unanesthetized sheep were subjected to 2 h of hemorrhagic hypotension and then resuscitated with 200 ml of 7.5% NaCl solution made up to include either 6% dextran 70 (Macrodex) or 6% hetastarch (Hespan).
