Lorazepam was compared with diazepam for the treatment of status epilepticus in a double-blind, randomized trial. Seventy-eight patients with 81 episodes were enrolled. Patients received one or two doses of either 4 mg of lorazepam or 10 mg of diazepam intravenously. Seizures were controlled in 89% of the episodes treated with lorazepam and in 76% treated with diazepam. The times for onset of action of the medications did not differ significantly. A patient with muscle spasms and rigidity associated with tetanus was successfully treated with a continuous infusion of diazepam. This beneficial response was associated with a minimal serum diazepam concentration of 500 ng/ml. The major metabolite of diazepam, DMD, did not appear to exert substantial activity. Under GABA-receptor blockade bicuculline prevented or substantially reduced the depressant effects of phenazepam and diazepam on the nerve control of cerebral circulation. Simultaneously, the depressant effects of the drugs on tonic activity and reflex responses in the sympathetic nerves were substantially decreased. Bilateral antagonism between phenazepam and diazepam, on the one hand, and bicuculline, on the other, was found. The data obtained indicate the contribution of GABA-ergic mechanisms to the development of phenazepam and diazepam cerebrovascular effects.

In a study of 52 consecutive out-patients the effects of diazepam and dixyrazine were compared with each other and with placebo as premedication for elective gastroscopy. All patients also received topical lidocaine and intravenous administration of propantheline bromide immediately before the examination. No difference was found between diazepam and dixyrazine, but they were both significantly superior to placebo in relieving anxiety, retching, and eructations.

Misuse and abuse of diazepam among addiction-prone individuals is reported. The most common pattern of abuse appears to be periodic ingestion of 30 to 80 mg of diazepam in one dose, either alone or in conjunction with methadone or other narcotics. Two cases of physical dependency to diazepam have been observed. Many addict patients using diazepam are buying it on "the streets". All physicians should know that diazepam abuse and misuse is occurring, and careful attention should be given to prescribing, transporting and storing this drug.
