Uterus didelphys, characterized by two separate uterine cavities and cervices, is associated with increased obstetric complications. Twin pregnancy with one fetus in each horn is exceptionally rare, often managed by cesarean or resulting in preterm birth. BACKGROUND: Uterine didelphys is a very rare Mullerian duct anomaly resulting from complete failure of ductal fusion, leading to duplication of uterine horns, cervix, and sometimes the proximal vagina. Although many of them are asymptomatic, it can present as poor reproductive outcomes or, rarely, pelvic organ prolapse. A 42-year-old woman with invasive lobular carcinoma of the left breast underwent staging with 68 Ga-FAPI PET/CT, which demonstrated intense uptake in the primary breast lesion and ipsilateral nodal metastases. Mullerian duct anomalies are congenital abnormalities involving the female genital systems. Anti-bothropic complex (ABC) was isolated from the serum of the South American opossum (Didelphis albiventris) by single-step affinity chromatography using a Sepharose-immobilized metalloprotease (BaP1) from Bothrops asper as the binding protein. UNLABELLED: The Herlyn-Werner-Wünderlich syndrome is a rare congenital anomaly of the Müllerian ducts. Müllerian malformations result from defective fusion of the Müllerian ducts during development of the female reproductive system. These malformations are associated with an increased probability of adverse obstetric events. The syndrome is characterized by uterus didelphys with obstructed hemivagina, and ipsilateral renal agenesis. Patients with this syndrome are asymptomatic until menarche, and can follow other complications.

Asymmetric obstructed uterus didelphys (Herlyn-Werner-Wunderlich syndrome-HWWS) is a rare congenital Müllerian anomaly consisting of uterus didelphys, hemivaginal septum and ipsilateral renal agenesis. We present a case of an incomplete HWWS diagnosed by 3D transvaginal ultrasound in a 22 year old patient with absence of the hemivaginal septum. The most contributive diagnostic factors as well as the appropriate therapeutic management in such cases are discussed.

We report a case of uterus didelphys with blind hemivagina and ipsilateral renal agenesis. This is an uncommon anomaly of the development of the Mullerian ducts in which a defect in one of the Wolffian ducts leads to failed induction in kidney formation and in the fusion of the Mullerian ducts. MRI plays a decisive role in the diagnosis and characterization of the malformation.
