We report a case of an exophytic benign prostatic hyperplasia presenting as a polycystic pelvic mass. A 69-year-old man presented with an incidental finding of a pelvic mass of over 18 years. The authors determined serum PSA levels in combination with digital rectal examination (DRE) and evaluated their role in the differential diagnosis of prostate diseases with special reference to cancer. The possible causes of differences between the observed cut-off level of PSA and the standard level PSA were analyzed. We report the first case of acquired immunodeficiency syndrome (AIDS) presenting as prostate nodularity secondary to mycotic granulomatous prostatitis which mimicked prostatic cancer on digital rectal examination. Transrectal ultrasonography revealed hypoechoic areas in the peripheral zone, which on biopsy specimen were found to represent cryptococcal infection. Subsequent serologic evaluation of the patient confirmed human immunodeficiency virus (HIV) positivity and later AIDS developed in the patient.

Routine testing for prostatic carcinoma by digital rectal examination, transrectal ultrasonography and prostate specific antigen determination has been proposed to reduce deaths by earlier diagnosis. The questionable reliability of results, cost of screening, and inability to establish a balance between the benefits of treatment and the adverse effects on the quality of life of the men screened make screening experimental until controlled studies prove its value.
