Pulmonary vein dilatation: another possible crosslink between left atrial enlargement and atrial fibrillation?

The interplay between left atrial (LA) dilatation and atrial fibrillation (AF) has been well established, but the underlying mechanisms of this vicious circle are not fully understood. Recent studies indicated that pulmonary vein (PV) dilatation is implicated in the development of AF. On the other hand, PV dilatation has been associated with LA dilatation. It is therefore reasonable to assume that PV dilatation represents a common pathophysiologic pathway between LA enlargement and AF.

Mixed laryngocele: a case report and review of the literature.

Laryngocele is a rare, benign dilatation of the laryngeal saccule that may extend internally into the airway or externally through the thyrohyoid membrane. Many laryngoceles are asymptomatic; sometimes they may cause a cough, hoarseness, stridor, sore throat and may present as a swelling on one or both sides of the neck. Laryngocele may be associated with supraglottic squamous cell carcinoma. Computed tomography scan is the most effective imaging method for diagnosis. Surgery is the treatment of choice. A case of large mixed laryngocele in a 75-year-old male is described together with surgical

Wandering spleen: a rare cause of mesenteroaxial gastric volvulus.

Gastric volvulus is a rare cause of acute abdomen in children. Usually it is associated with defects such as diaphragmatic hernia, hiatal hernia, eventration, and paralysis of the diaphragm. We report an extremely rare case presenting with acute intractable vomiting and abdominal distention. Passage of a nasogastric tube relieved the symptoms. A history of the ingestion of a large bolus of chewing gum and a suboptimal contrast study were misleading, and there was a delay in diagnosis of 3 days. Later, repeat upper GI fluoroscopy with contrast medium identified mesenteroaxial volvulus of the st

[Dilated vestibular aqueduct syndrome in children. Report of a case].

Enlarged vestibular aqueduct syndrome is a clinical condition characterized for a progressive perceptive deafness linked to a broadening of vestibular aqueduct greater than 1.5 mm without other otic abnormal structures. We report the case of a 2-year-old child with such congenital malformation. We have reviewed the actual literature and point out its clinical and physiopathologic features as well treatment possibilities of these cases.

Cowper's syringocele: diagnosis based on MRI findings.

An 8-year-old boy presenting with intermittent haematuria and dysuria was investigated with voiding cystourethrography (VCUG), urethroscopy and perineal MRI. VCUG and urethroscopy did not provide a definitive diagnosis, while MRI clearly demonstrated a homogeneous, oval cystic lesion extending from the bulbourethral glands and slightly impressing the bulbous urethra. The patient was found to have an imperforate Cowper's syringocele. Treatment consisted of marsupialisation and the patient became symptom free. To the authors' knowledge, this is the first description of the MRI characteristics of

Idiopathic enlargement of the right atrium.

A case of aneurysmal enlargement of the right atrium was found accidentally during a routine examination. The patient was asymptomatic and free of palpitations or proven rhythm disturbances. The results of echocardiography, magnetic resonance imaging, and cardiac catheterization excluded cardiac malformation, which could have caused secondary enlargement of the right atrium. A partial pericardial defect as a likely cause of herniation of the right atrium was ruled out by magnetic resonance imaging. In keeping with these results, isolated aneurysmal enlargement of the right atrium was diagnosed

An unusual case of segmental dilatation of the sigmoid colon.

An unusual case of segmental dilatation of the colon (SDC) with a broad membranous bridge in the distal part of the dilated bowel is presented. To our knowledge, this association has not been previously reported and might provide further insight into the underlying etiology of SDC.

[Laryngocele and pyolaryngocele].

Report of one case of laryngocele and an other one of pyolaryngocele, in two children with laryngeal papillomatosis. Review of the treatment and the origin of the process as well. The AA. point out that with regard to the cause there is an interrelation between a congenital predisposition, represented by a large ventricular appendix, and other postnatal acquired factors, for instance, the papillomatosis present in the two patients, which justify the paper.

Rudimentary dysplastic valvar tissue guarding the tricuspid orifice with dilatation of the right ventricle and a patent outflow tract.

Tricuspid regurgitation due to rudimentary and dysplastic tricuspid valvar tissue associated with a patent right ventricular outflow tract is described in a 10-year-old child presenting with cyanosis since birth. The diagnosis was confirmed by cross-sectional echocardiography, angiocardiography and subsequently by histopathological data. We discuss the differentiation of these findings from Uhl's anomaly and from the congenitally unguarded tricuspid orifice.

[Congenital cystic dilatation of the intrahepatic bile ducts (Caroli syndrome)].

Caroli's syndrome is a rare disease, six cases of which were under the authors' care between 1979 and 1986. None had associated congenital hepatic fibrosis; one had an associated choledochal anomaly. Endoscopic retrograde cholangiography proved to be the most sensitive diagnostic method. Liver abscess and secondary biliary cirrhosis of the liver developed as complications. Partial liver resection was possible in two patients in whom only one lobe had been affected.

Endoscopic microsurgical excision of internal laryngocele.

Internal laryngoceles are relatively rare, and a variety of surgical treatments have been proposed. A case is presented which was treated by endoscopic microsurgical excision.

Endoscopic dilation of late stomal stenosis. Its use following gastric surgery for morbid obesity.

We have observed 16 cases of stomal stenosis occurring late after gastric surgery for morbid obesity. In one patient no stoma was apparent in the pouch on radiographic or endoscopic examination and reoperation was required for complete obstruction. We dilated stenoses in the remaining 15 patients. Stenosis in the three earliest patients in our series were dilated with Eder-Puestow dilators. All subsequent patients have received endoscopic dilation with pneumatic balloon catheters. Ten patients remain asymptomatic following dilation. Two patients have occasional vomiting, which is ameliorated b

[Laryngocele. Possibilities and limitations of surgical treatment under direct microlaryngoscopy].

Three cases of laryngocele are reported, which were managed by surgery under direct microlaryngoscopy with excellent clinical and functional results, both at short and long term. According to the classification of Y. Lacomme, two patients had an internal or vestibular laryngocele, which was of the low type in one of them and of the middle type in the other. In the first case the pouch and the false vocal fold lying over it were completely removed under direct microlaryngoscopy. In the second case the laringeal cavity was marsupialized after extensive resection of the false fold and part of the
