Jejunal Leiomyosarcoma Diagnosed via Anterograde Enteroscopy: Case Report of a Rare Presentation of Obstructive Small Bowel Tumor.

Primary leiomyosarcoma (LMS) of the small intestine is an uncommon and aggressive malignant tumor. We report the case of a 61-year-old male who presented with postprandial abdominal pain, recurrent vomiting, transfusion-dependent anemia, and significant weight loss. Contrast-enhanced CT showed a large circumferential mass in the proximal jejunum, mesenteric thrombosis, and a hepatic lesion suggestive of metastasis. Anterograde double-balloon enteroscopy revealed an ulceroinfiltrative lesion approximately 40 cm distal to the ligament of Treitz, with friable mucosa and prominent vasculature. Tar…

Solitary Variceal Rupture in the Small Intestine.

A 70-year-old man was referred to our hospital to undergo treatment for hepatocellular carcinoma. In hospital, he complained of hematochezia and a laboratory analysis revealed a decreased level of hemoglobin. Abdominal computed tomography revealed a tumor in the small intestine, with slow enhancement of the dorsal region. Double-balloon enteroscopy revealed a submucosal tumor with a depression in the jejunum. Partial enterectomy was performed and a pathological examination demonstrated the presence of a solitary varix. Solitary varix in the small intestine has not been reported previously. We …

Endoscopic Retrograde Cholangiopancreatography in Surgically Altered Anatomy.

ERCP in surgically altered anatomy requires the endoscopist to fully understand the procedural goals and the reconstructed anatomy before proceeding. Altered anatomy presents a variety of challenges unique to enteroscopy, and others related to accessing the biliary or pancreatic duct from unusual orientations. Both side-viewing and forward-viewing endoscopes, as well as single and double balloon techniques, are available for ERCP in these settings. Endoscope selection largely depends on the anatomy and length of reconstructed intestinal limbs. Endoscopist experience with performing ERCP in sur

Biliary drainage in patients with altered anatomy: Literature review of different endoscopic approaches.

Endoscopic retrograde cholangiopancreatography (ERCP) in patients with surgically altered anatomy remains a challenging field in therapeutic endoscopy due to the complex anatomical reconstructions that limit access to the biliary tree. Over the past two decades, device-assisted enteroscopy (DAE), including single-balloon, double-balloon, and motorized spiral enteroscopy, has expanded the feasibility of ERCP in this population, with overall technical success rates generally reported between 70% and 90%. Nevertheless, these techniques are technically demanding, time-consuming, and frequently aff

Jejunal hemolymphangioma: A case report.

RATIONALE: Hemolymphangioma is a benign tumor comprised of the newly-formed lymph spaces and blood vessels, which can usually be found in th … ion in proximal jejunum on computed tomography (CT). Eventually, the lesions were confirmed through double-balloon enteroscopy (DBE) to be located in the jejunum 60 cm away from the Treitz ligament. DIAGNOSE: Subsequently, the small intestine was partially resected, and postoperative pathology had confirmed the diagnosis of small intestinal hemolymphangioma. INTERVENTIONS …

Jejunal Metastasis from Hepatocellular Carcinoma.

A 76-year-old man with hepatocellular carcinoma associated with alcoholic cirrhosis was hospitalized for lightheadedness and melena. He had undergone multiple surgeries and had been treated with transcatheter arterial chemoembolization and sorafenib. Neither upper nor lower gastrointestinal endoscopy detected the source of bleeding. Oral double-balloon enteroscopy revealed a mass lesion in the upper jejunum, 20 cm from the Treitz ligament on the anal side, which was identified as the source of bleeding. Subsequently, a biopsy was performed. A histopathological examination detected a hepatocell…

Massive lower gastrointestinal bleeding in a young adult due to Meckel's diverticulum without ectopic gastric mucosa.

Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract, occurring in approximately 2% of the general popu … luation and contrast-enhanced computed tomography failed to identify the bleeding source. Transanal double-balloon enteroscopy revealed a tubular structure 35 cm proximal to the ileocecal valve, with a circumferential ulcer at its opening. Although active bleeding was not observed, contact with the endoscope induced oozing. Laparoscopic segmental resection was performed, an …

A case of small intestinal neuroendocrine carcinoma diagnosed using double-balloon endoscopy with long-term survival.

Neuroendocrine neoplasms, including neuroendocrine tumors (NETs) and neuroendocrine carcinomas (NECs), are rare epithelial tumors with a predominant neuroendocrine differentiation. Compared with NETs, NECs have been reported to be rarer and have a poorer prognosis. We present a rare case of small bowel NEC diagnosed using double-balloon endoscopy (DBE) and the long-term survival accomplished via intensive therapy. DBE revealed an ulcerative tumor in the deep jejunum, and biopsy specimens showed large and highly dysplastic tumor cells; immuno-histological synaptophysin and chromogranin A tests

Obscure Gastrointestinal Bleeding Caused by a Small Intestinal Lymphatic-venous Malformation: A Case Report with a Literature Review.

A 44-year-old woman presented with severe anemia. We strongly suspected gastrointestinal bleeding; however, esophagogastroduodenoscopy, colonoscopy, and computed tomography showed no bleeding sources. Video capsule endoscopy revealed an actively bleeding submucosal lesion within the jejunum. Double-balloon enteroscopy revealed a 20-mm continuously bleeding submucosal lesion in the distal jejunum. We suspected small intestinal vascular malformation and performed surgical resection. The resected specimen pathologically comprised dilated, thin-walled lymphatic channels and blood vessels involving…

Laparotomy-Assisted Endoscopic Injection of Jejunal Varices for Overt Small Bowel Bleeding.

A 54-year old male liver transplant recipient with Roux-en-Y choledochojejunostomy was admitted for symptomatic anemia. Despite endoscopies and a bleeding scan, active bleeding was not visualized. Angiography and abdominal computed tomography demonstrated possible jejunal varices at the choledochojejunal anastomosis. Double-balloon enteroscopy demonstrated varices with overlying clots in the Roux limb at the anastomosis. Due to the extensive loop formation and unstable position of the endoscope, therapeutic intervention could not be performed. Operative enterotomy and intraoperative endoscopy …

Jejunal lipoma concealing intestinal adenocarcinoma: don't always regard CT imaging features like lipoma as benign or pseudomalignant features.

Lipomas are benign submucosal tumors composed of mature adipose tissue. Some authors have described patients with persistent abdominal pain who were misdiagnosed as intestinal adenocarcinoma and the diagnosis turned out to be a lipoma on histological examination. We discuss the case of a 57-year-old woman presenting with abdominal pain for 4 weeks. Intestinal adenocarcinoma was found by double-balloon enteroscopy (DBE), but at first only lipoma was found by CT scan. Therefore we want to emphasize that lipoma should not always be regarded as pseudomalignant features, sometimes lipoma conceals i…

Vascular malformation of the jejunum presenting as obscure gastrointestinal haemorrhage: detection with multidetector CT angiography.

Vascular malformation of the small bowel is an uncommon cause of gastrointestinal haemorrhage. Phlebectasia or venous ectasia is a rare beni … as detected using multidetector computed tomography (MDCT) angiography and was further confirmed by double-balloon enteroscopy. This report emphasises the potential of MDCT angiography in the diagnosis of small intestine lesions presenting as obscure bleeding.

[Adult ileal duplication diagnosis using combined capsule endoscopy and computed tomography enterography with amidotrizoic acid meglumine solution: a case report].

A 67-year-old man with complaints of dark red stools was admitted to the hospital. Capsule endoscopy, computed tomography enterography (CTE) with amidotrizoic acid meglumine solution (AM), and transanal double-balloon enteroscopy revealed a diverticulum in the ileum, which was accompanied by an ulcer in the vicinity. A partial ileal resection was performed laparoscopically, and the intraoperative and pathological findings indicated that the patient had a duplicated intestinal tract. Intriguingly, we identified a few reports on preoperative diagnosis of ileal duplication;however, we experienced…

A Rare Cause of Small Bowel Intussusception: Non-Meckel Diverticula in an Adult Patient.

Intussusception involves telescoping of a proximal bowel segment into a distal segment. Non-Meckel small bowel diverticula are rare and typically asymptomatic. We report a 37-year-old man with Crohn's disease presenting with persistent abdominal pain despite normal colonoscopies and inflammatory markers. Magnetic resonance enterography and capsule endoscopy revealed concentric small bowel thickening; double-balloon enteroscopy identified a large diverticulum in the ileum. Surgical resection was performed, with pathology revealing non-Meckel diverticula, and symptom resolution postoperatively. …

An Endoscopic View of Multiple Hemangiomas of the Small Intestine.

Small intestinal hemangiomas may present as a severe gastrointestinal hemorrhage associated with a hematologic emergency. In the emergent setting, this may result in more extensive intestinal resection than would otherwise be necessary with elective intervention. The widespread application of capsule endoscopy and double-balloon enteroscopy presents an opportunity to diagnose small bowel hemangiomas prior to symptomatic onset. In one of the first published cases of multiple small intestinal hemangiomas, we highlight the importance of maintaining a broad differential and pursuing a thorough wor…

Large Gallstones Stacked Together in the Common Bile Duct after Choledochojejunostomy.

A 65-year-old man had a history of cholecystectomy and treatment for cholelithiasis with a common bile duct incision. Owing to frequent cholangitis, he underwent choledochojejunostomy. Twenty years after the surgery, he was hospitalized for cholangitis and was suspected of having hilar cholangiocarcinoma based on imaging findings. Percutaneous transhepatic cholangioscopy using a SpyGlass™ DS (Boston Scientific, Marlborough, USA) showed gallstones and bile sludge in the bile ducts, but no tumors were noted. Electrohydraulic shockwave lithotripsy with double-balloon enteroscopy enabled complete …

Gastrointestinal bleeding 30 years after a complicated cholecystectomy.

Gastrointestinal bleeding from small-bowel varices is a rare and difficult to treat complication of portal hypertension. We describe the case of a 79-year-old female patient with recurrent severe hemorrhage from small-bowel varices 30 years after a complicated cholecystectomy. When double balloon enteroscopy was unsuccessful to reach the site of bleeding, a rendezvous approach was favored with intraoperative endoscopy. Active bleeding from varices within a biliodigestive anastomosis was found and controlled by endoscopic injection of cyanoacrylate. Intraoperative endoscopy should be considered
