We report the case of a 10-year-old boy with prolonged fever who was found to have tularemia of the middle ear. Otolaryngologic cases including oropharyngeal and glandular or ulceroglandular forms of the head and neck region are estimated to account for 12% of all tularemia cases, but to date we have not seen a report of tularemia in the middle ear. The possibility of tularemia may not occur to a physician because of the wide variation of clinical manifestations.

Knowledge of the development of the peripheral auditory system, especially the middle ear and inner ear (cochlea), contributes to meaningful interpretation of audiologic measures infants, including newborn screening outcome. An understanding of the effects of noise on neonatal auditory system and hearing function is also dependent on maturational factors. This paper is a brief overview of principles and trends in the development of the middle and inner ear. The emphasis is on maturation of human peripheral auditory system structure and function, although reference is also made to animal resear…

The authors report their experience in the surgical treatment of 96 patients with prominent ear. Most of the patients are less than 30 years old--75.3%. Various operation techniques are used. We noticed that the best cosmetic result was obtained by combination of two or more basic otoplasty techniques and that approach was applied in 60.5%. There were no major complications. Three patients have undergone second operation.

AIMS AND BACKGROUND: Melanoma of the external ear is a rare disease, and its management is controversial. To address this problem, we reviewed the data concerning the patients observed at our Institution.
METHODS: We retrospectively reviewed the clinical records of the 20 patients bearing primary ear melanoma observed over a period of about 20 years at the Istituto Nazionale Tumori of Milan.
RESULTS: Initial evaluation of the patients revealed 7 stage I, 12 stage II and 1 stage III. The thickness of the tumors varied from 0.39 to 6.62 mm. Fourteen patients underwent a wedge resection of the …

New computed tomography (CT) and magnetic resonance (MR) techniques allow more detailed anatomic studies of the inner ear. CT is still the best technique to study patients with fractures, congenital malformations and otodystrophies involving the inner ear. During recent years MR imaging has emerged as an excellent method to detect pathology in the internal auditory canal, membranous labyrinth and bony labyrinth and to characterize petrous apex lesions. MR has even proved its value in patients with fractures and congenital malformations making the diagnosis of, for instance, labyrinthine concus…

Choristoma is a mass of tissue histologically normal for an organ or part of the body other than the site at which it is located. A rare case of ectopic salivary gland choristoma in the middle ear is described in a 4-year-old boy whose only symptom was a 50 dB conductive hearing loss in the presence of a normal tympanic membrane.

Since the beginning of the war, during the last year, we have examined 255 patients who were exposed to shelling of different origin and caliber. Our pilot study included 197 of patients. We have excluded patients having serious head and brain injuries, as well as cases previously having chronic ear infection, hearing damage and disturbances of vestibulary functions. Patients over 55 years of age were also not included in our study. Our paper presents the consequences of blast injuries to the ear, as well as type and degree of hearing damage. Acquired data are based on anamnesis, ENT examinati…

Keloid formation is a rare complication of normal scar tissue development, also to the surgeon who performs surgery on the external ear. We are presenting 4 cases of keloid formation which occurred after ear piercing in 2 cases and after otoplasty in another 2 cases. The treatment of keloid formation caused difficulties and is not unequivocal. Therefore we wish to present our experiences as we have seen no cases of keloid formation after otoplasty described in the literature.

Ear injuries occur in people of all ages but predominate in active people such as wrestlers, boxers, and bike riders. The types and extent of injury are a function of the force causing the injury. Shearing forces of moderate intensity cause hematoma formation, whereas greater force causes lacerations or even amputation. Sharp objects cause lacerations determined by the force, direction, and point of impact. The high ratio of surface area to mass makes the auricle vulnerable to extremes of temperature. People participating in high-risk activities should wear protective headgear. The goal of tre…

Actinomycosis is an uncommon infection of the middle ear. Only 21 cases of actinomycosis of the middle ear have been reported in the English literature prior to this paper. The offending organism is Actinomyces israelii, which is an anaerobic, filamentous organism that is difficult to grow in culture. The infection is chronic and is seldom diagnosed prior to tympanomastoidectomy. The identification of small, yellow, glue-like masses, which are called sulfur granules, is often the key to making the diagnosis of actinomycosis of the middle ear. Following tympanomastoidectomy, penicillin is given…

Ear pain is a diagnostic problem when examination of the ear shows no pathology. Referred otalgia may be caused by neoplasms; dental abnormalities or infections; temporomandibular joint dysfunction; sinus, pharyngeal or salivary gland infections; temporal arteritis; cervical arthritis, or one of the neuralgias. Ear pain may also be psychogenic.

The prominent ear was first noted in the medical literature to be a social and psychological disturbance in 1881, when Ely suggested the first technique for the correction of this condition. Since Ely many surgeons have tried to find the best solution for this deformity; however, because of the specific cartilagenous architecture of the auricle, no one has yet designed an acceptable and secure technique. The successful reconstructured auricle should be the result of tackling all components of the deformity, namely, the absence of the antihelix and the aloofing of the concha from the mastoid. M…

High-resolution CT has proved to be a reliable and accurate method for evaluating the postsurgical ear. The presence or absence of cholesteatoma can usually be determined. Because of this, CT is a viable alternative to the "second look" or staging surgical procedure. Methods, techniques, and case examples are discussed.

Although microtia occurs in only 1:7000 to 8000 births, this major congenital ear deformity can result in significant psychological trauma for the affected child. Reconstruction of this complex deformity represents one of the greatest challenges to the reconstructive surgeon. Based on the pioneering work of Tanzer and Brent, the techniques have matured to the point that consistently good results can be obtained and many of the resultant problems associated with the deformity avoided. In classic microtia, a sausage-shaped vestige is made up of a rudimentary lobule and various additional remnant…

A seven year old boy was admitted with what appeared to be polyp of the external auditory canal, otitis media and Bell's palsy. It was subsequently diagnosed as rhabdomyosarcoma of the middle ear. This rare tumor must not be out of mind of practitioners, and especially pediatricians and E.N.T. specialists, when they have to examine a child with what looks like chronic otitis media, and aural polyp, or idiopathic facial paralysis. The diagnosis is histological and must be quickly made for early and aggressive multimodal antitumor therapy, which seems to be improving the hitherto gloomy prognosi…
