[Correcture of congenital defects of the wing and the tip of the nose (author's transl)].

Congenital hypoplasia of the nasal wing can appear as a defect (congenital notch) or as a flattening and deficiency of the cartilagineous support. The combination with other deformities is seen frequently, 2 own cases: 1. Congenital notch of the left ala nasi. Operation: forming a window in the wing by incision, implantation of a composite graft from the ear, undisturbed and complete healing. 2. Hypoplasia of both cartilages of the ala nasi, flatness and dropping of the nasal tip, combined with deformities in both middle ears. Operation: Lifting the nasal tip, median suture between the cartila

Vertical mattress suture applied as "tension band fixation" for the antihelix creation and prominent ear treatment: 104 case reports.

BACKGROUND: Prominent ears were the most common auricular deformity. Different surgical techniques, such as cartilage-cutting techniques and suturing techniques, are available for treatment. The horizontal mattress suture technique, represented by the Mustard technique, is widely accepted, while the vertical mattress suture technique is rarely discussed in otoplasty. METHOD: A total of 104 cases of prominent ear deformities were treated from January 2017 to December 2020. Several hypothetical "tension bands" perpendicular to the natural curvature of the antihelix were introduced for the surgi

Otoplasty: critical review of clinical results.

Although it is generally believed that otoplasty provides uniformly gratifying results, thorough patient follow-up reviews are quite rare. Otoplasty surgery performed on 119 ears was retrospectively reviewed to assess the results of our cartilage-sparing technique. This method of otoplasty relies on a conchal set-back and suture transfixation technique to improve the antihelical contour. Auricular medialization averaged 10.3 mm. Over-correction is required, particularly in the superior pole, as loss of correction with healing was approximately 40% of the initial medialization. Revision surgery

Bilateral pyogranulomatous otitis externa with putative cartilage destruction in a dog: A severe case of auricular chondritis?

Auricular chondritis of unknown cause was suspected in a 10-year-old male Bolognese dog with a five-month history of painful bilateral nodular and ulcerative pyogranulomatous dermatitis of the pinnae with putative auricular cartilage destruction. Pain and lesions resolved with immunosuppressive doses of prednisolone, yet the condition resulted in deformity of both pinnae and external canals.

Comparison of the compressive mechanical properties of auricular and costal cartilage from patients with microtia.

Children born with a small or absent ears undergo surgical reconstruction to restore their auricle. Currently, rib (costal) cartilage is used to carve the auricle. However as alternative, tissue engineered and synthetic materials are being developed to restore the auricle shape to overcome donor site morbidity and limited availability of rib cartilage. However, to date there is limited knowledge regarding the mechanical properties of the auricular and costal cartilage to optimise the required compressive properties of the graft. The remnant auricular and costal cartilage from 20 patients under

Use of cartilage-conduction hearing aids in a child with Crouzon syndrome and meatal atresia.

A cartilage-conduction hearing aid (CCHA) transmits sound vibrations to the inner ear via the ear cartilage rather than through air or bone. They can be used in patients with meatal atresia and persistent otorrhea. Unlike bone-conduction hearing aid (BCHA), there is no need for pressure between the transducer and the temporal bone. Effective hearing is achieved by simply bringing the cartilage-conduction transducer into contact with the auricular cartilage. Crouzon syndrome is characterized by craniosynostosis and is associated with a high incidence of meatal atresia, which causes conductive h…

Otitis media in association with various congenital diseases. Preliminary study.

We studied temporal bone histopathology to obtain clues as to the role of eustachian tube (ET) disease in the pathogenesis of otitis media (OM) in patients with congenital anomalies. Thirteen temporal bones obtained from 13 individuals with various congenital anomalies were used. We looked in particular at severity of inflammation in the middle ear (ME) and ET, and at deformity of the ET cartilage. Evidence of OM was found in all the temporal bones. The ETs consistently showed a moderate degree of inflammation but without any particular correlation with the severity of ME inflammation. Deformi

Delayed ear reconstruction: case report of reconstruction of an avulsed ear 2 days after injury.

Auricular reattachment and reconstruction following traumatic ear avulsion is a challenging surgical problem. Suggested reconstruction methods include direct reattachment, composite grafting, pocket methods, coverage with periauricular flaps, and microsurgical repair. A published alternative is reattachment and burial of the amputated part under a postauricular flap. If circumstances delay the surgical intervention, what is a safe window to still consider this form of treatment? In the current report, we present a case of a complete partial ear avulsion, which was reattached in the ER as a fre

Multiple, bilateral and painful ear nodules of the anthelices: a variant of chondrodermatitis nodularis?

A case of a distinctive clinicopathologic condition of the ear cartilage is presented, characterized by multiple, bilateral and painful nodules of the anthelices without epidermal involvement. Histologically, there was a peri-chondrial lymphohistiocytic infiltrate and a small focus of degenerate, basophilic cartilage as well as cystic chondromalacia containing an amorphous mass. This condition is both clinically and histopathologically distinct from other causes of ear nodules, although the lesions seen in our patient exhibit features of chondrodermatitis nodularis helices and therefore could …

Microsurgical replantation and salvage procedures in traumatic ear amputation.

PURPOSE: The purpose of this study is to present our experience with patients who underwent traumatic ear amputation. METHODS: Between January 1988 and April 2002, 10 patients sustained ear amputations. Of these, six patients underwent microvascular replantation (arterial anastomosis only and arterial and venous anastomosis in three patients each), and replantation was attempted in one patient. However, no suitable vessel could be found for the anastomosis, and the amputated ear was treated as a composite graft and buried in a retroauricular pocket. Staged costal cartilage reconstruction was

Surgical Reconstruction of Traumatic Partial Ear Defects Based on a Novel Classification of Defect Sizes and Surrounding Skin Conditions.

BACKGROUND: Although a standard classification system and corresponding reconstruction methods are well described for microtia, establishing a classification system and corresponding surgical strategies for traumatic ear defects is difficult because the size, shape, and local skin conditions of each defect differ considerably. In this article, the authors describe a useful new classification system and corresponding surgical approaches. METHODS: Ear defects were classified into four types according to defect size and surrounding skin conditions. Defects in which the postauricular skin is of g

A New Classification of Helix Fabrication Methods with Autogenous Costal Cartilage in Microtia Reconstruction.

BACKGROUND: Recreating the complex architecture of the auricle poses one of the most demanding challenges for the plastic surgeon. The fabrication of the prominent helix is crucial to the final contour of the constructed ear. The success of helix carving depends greatly on the complicated characteristics of the cartilage used, which is commonly the eighth costal cartilage. The authors elaborate on the relevant details for helix sculpting based on different qualities of cartilage. METHODS: A series of 415 microtia patients underwent autogenous costal cartilage auricular reconstruction between

Lop Ear to Conchal Microtia: An Algorithmic Surgical Approach.

BACKGROUND: The lop ear deformity is defined by a deficient helix and scapha, underdeveloped anthelix, and downfolding of the helix. The terminology used is still confusing, and the treatment is not entirely structured. The aim of this study was to provide a new systematic surgical approach of this deformity based on our center's experience. MATERIALS AND METHODS: All patients undergoing surgical correction of lop ears between 2007 and 2019 at Great Ormond Street Hospital were included. Patients' data, surgical techniques, and postoperative complications were recorded. RESULTS: Based on our

Repurposing of a gill gene regulatory program for outer-ear evolution.

How new structures emerge during evolution has long fascinated biologists. An example is how the diminutive bones of the mammalian middle ear arose from ancestral fish jawbones1. By contrast, the evolutionary origin of the outer ear, another mammalian innovation, remains a mystery, partly because it is supported by non-mineralized elastic cartilage, which is rarely recovered in fossils. Whether the outer ear arose de novo or through the reuse of ancestral developmental programs has remained unknown. Here we show that the outer ear shares gene regulatory programs with the gills of fishes and am

Repositioning of posterior auricular muscle combined with conventional otoplasty: a personal technique.

Prominent ears are the most common deformity of the external ear in children. Since 1881 various treatments have been described but the choice of procedure still remains at the surgeon's preference. A posterior auricular muscle malposition is frequently present in prominent ear deformity even though this muscle shows only a rudimentary function in man. This article presents a technique to reposition the posterior auricular muscle as an adjunct to conventional otoplasty. A quadrangular cartilage paddle, where the muscle inserts, is raised with the muscle as a chondro-muscular flap that can be a
