Ultrasound-Assisted Posteromedial Portal Placement of the Elbow Joint to Prevent Ulnar Nerve Injury.

Direct posterior and posterolateral portals are the standard portals used in posterior elbow arthroscopy. A posteromedial portal in the elbow is not recommended because of its proximity to the ulnar nerve. However, iatrogenic injuries to the ulnar nerve have been reported after elbow arthroscopy using the standard posterior portals, especially in posteromedial elbow joint pathologies. We present a surgical technique applicable to posteromedial elbow pathology by using ultrasound-assisted posteromedial portal placement of the elbow joint. Through this technique, the position of the ulnar nerve

In Vitro Kinematic Assessment of a Hinged Elbow Orthosis Following Lateral Collateral Ligament Injury.

PURPOSE: Elbow lateral collateral ligament injuries (LCLI) are often managed with protected mobilization using a hinged elbow orthosis (HEO). The objective of this investigation was to determine the effectiveness of an HEO in stabilizing the elbow following LCLI. METHODS: Seven fresh-frozen cadaveric upper extremity specimens were studied using a custom simulator that enabled elbow motion via computer-controlled actuators and servomotors attached to relevant tendons. Specimens were examined in 4 arm positions (dependent, overhead, horizontal, and varus) and 2 forearm positions (pronation and

Triceps to biceps transfer for restoration of elbow flexion following upper brachial plexus injury.

INTRODUCTION: Upper brachial plexus injury in adults causes loss of elbow fl exion; when the primary nerve surgery has failed or the patient seeks treatment after 12 months of injury and pedicled muscle transfers are required. Most commonly, the latissimus dorsi or the Steindler flexorplasty is used. MATERIAL AND METHODS: We have transferred one of the heads of triceps muscle to restore the elbow flexion in such cases. In addition to return of elbow flexion, extension of elbow following surgery is retained. Ten patients suffering from upper brachial plexus injuries underwent transfer of one h

Injury complexity factors predict heterotopic ossification restricting motion after elbow trauma.

BACKGROUND: Heterotopic ossification (HO) is a common extrinsic cause of elbow stiffness after trauma. However, factors associated with the development of HO are incompletely understood. QUESTIONS/PURPOSES: We retrospectively identified (1) patient-related demographic factors, (2) injury-related factors, and (3) treatment-related factors associated with the development of HO severe enough to restrict motion after surgery for elbow trauma. We also determined what percentage of the variation in HO restricting motion was explained by the variables studied. METHODS: Between 2001 and 2007, we per

Prevention of nerve injury during arthroscopic capsulectomy of the elbow utilizing a safety-driven strategy.

BACKGROUND: A major factor limiting the use of elbow arthroscopy for contracture release is concern regarding nerve injury. The purpose of this report is to document the risk of nerve injury in a large series of arthroscopic contracture releases utilizing a safety-driven strategy. METHODS: A series of 502 arthroscopic elbow contracture releases (including 388 osteocapsular arthroplasties) performed in 464 patients by one surgeon was reviewed retrospectively. The safety-driven step-wise strategy had been carried out in a standardized sequence: (1) Get In and Establish a View, (2) Create a Spac

Ipsilateral compound distal radius fracture with missed elbow dislocation. A rare injury pattern.

A rare injury pattern of compound distal radius fracture associated with a missed posterolateral elbow dislocation in an adult is described. To the best of our knowledge, this has not been previously described in the literature. The probable mechanisms of injury are speculated to include longitudinal compressive force along with valgus and external rotatory stress to the semiflexed elbow leading to elbow dislocation. A continuation of the impact when her hand hit the ground led to her hand deviating radially resulting in compounding on the ulnar side. Early reduction of the dislocation and rig

Muscle changes in brachial plexus birth injury with elbow flexion contracture: an MRI study.

BACKGROUND: Muscle pathology of the arm and forearm in brachial plexus birth injury (BPBI) with elbow flexion contracture has not been evaluated with MRI. OBJECTIVE: To determine whether limited range of motion of the elbow in BPBI is correlated with specific patterns of muscular pathology. MATERIALS AND METHODS: For 15 BPBI patients, total active motion (TAM) of the elbow (extension-flexion) and the forearm (pronation-supination) were measured. MRI of the elbow joints and musculature allowed assessment of elbow congruency. Fatty infiltration and size reduction of the muscles were graded sem

[Excision of heterotopic new bone around the elbow in patients with head injury: 51 cases].

PURPOSE OF THE STUDY: Neurogenic paraosteoarthropathy of the elbow is a frequent complication in head injury patients. The functional impairment may be severe. The purpose of this work was to evaluate the efficacy of our medico-surgical approach and to determine indications for surgery. MATERIAL AND METHODS: We analyzed retrospectively a continuous series of 44 patients (51 elbows) with neurogenic heterotopic new bone of the elbow joint managed from 1993 to 2000. The heterotopic bone had developed on a central deficit limb in 70% of the patients. Flexion-extension was limited in all patients,

Restoration of elbow flexion after brachial plexus injury: the role of nerve and muscle transfers.

Brachial plexus trauma results in a variable loss of upper extremity function. The restoration of this function requires elbow flexion of adequate strength and range of motion. A proper evaluation of brachial plexus lesions is a prerequisite to any reconstructive procedure, and appropriate guidelines are presented. One option for restoring elbow flexion is a nerve transfer. The best results with this procedure are obtained in young patients treated within 6 months of injury. Another option is a free or pedicled muscle transfer, which should be considered in older patients or patients treated m

Long-term outcomes after nonoperative management of selected stable terrible triad injuries of the elbow: a single-center case series.

BACKGROUND: Terrible triad injury of the elbow (TTI) is usually managed surgically because of instability and the risk of poor outcomes. Recent reports suggest that a carefully selected subgroup with concentric reduction and satisfactory early stability may be managed nonoperatively. This study aimed to describe the long-term functional and radiographic outcomes of patients with TTI in whom nonoperative management was maintained and to identify the common characteristics of this selected subgroup. HYPOTHESIS: We hypothesized that, in carefully selected TTIs with stable concentric reduction an

Early supraclavicular nerve grafting or distal nerve transfer for restoration of elbow flexion in incomplete adult traumatic brachial plexus injuries.

INTRODUCTION: Restoration of elbow flexion is the primary treatment goal in adults with supra-clavicular traumatic brachial plexus injuries (ATBPI). To date, many nerve surgeons will initially await spontaneous recovery when dealing with an incomplete brachial plexus injury to allow sufficient recovery in the median and ulnar nerves to enable distal fascicular nerve transfers (DFNT). An alternative strategy is reconstruction by supraclavicular nerve grafting, which should be performed as early as possible after trauma to minimize denervation time. RESEARCH QUESTION: Does early supraclavicular

A Multicenter Validation of a Novel Prediction Model for Elbow Flexion Recovery after Nerve Transfer Surgery in Brachial Plexus Injuries.

BACKGROUND: Nerve transfer surgery for brachial plexus injuries exhibits variable success rates, potentially resulting in prolonged limb dysfunction for more than 2 years. A proposed prediction model has been developed to predict the unsuccessful recovery of elbow flexion after the surgery. The model consisted of six variables, namely body mass index 23 kg/m2 or more, smoking, total arm type, donor nerve, ipsilateral upper extremity fracture, and ipsilateral vascular injury. This study aimed to assess the external validity of the model for wider applicability. METHODS: This retrospective anal

Outcomes of the Posterior Approach for the Treatment of Radial Head Fractures and Associated Elbow Injuries: A Retrospective Observational Study.

BACKGROUND: The lateral approach to the radial head remains the routinely used approach for surgical fixation or replacement despite the risk of injury to lateral soft tissue structures. Multiple approaches are required when dealing with complex elbow injuries involving other bony and soft tissue structures which lead to greater soft tissue dissection, prolonged immobilization, and a higher rate of elbow stiffness. This article utilizes a single posterior approach involving the Boyd interval in the surgical management of radial head fractures with an associated elbow injury. METHODS: Thirteen…
