[Asynchronous Bell's phenomenon in progressive supranuclear palsy].

A 60-year-old man suffering from progressive supranuclear palsy showed asynchronous Bell's phenomenon and apraxia of eye-lid opening. He showed synchronous upward rotation of the eyeball at closing eyes. Although apraxia of eye-lid opening was observed in both sides, eye opening of the right side took longer duration than that of the opposite side. When he tried to open his eyes, unilateral Bell's phenomenon was sustained in the side of involuntary eye closing for a several seconds. These findings indicated disorders of the coordination of eye opening-and-closing, which regulate the orbicural

Electro-oculogram in vitamin A deficiency associated with cystic fibrosis. Short communication.

Electro-oculograms (EOGs) were recorded in a patient with cystic fibrosis and vitamin A deficiency before and during vitamin A supplementation (25,000 IU/day). Before vitamin A supplementation the EOG Arden light/dark ratio was 1.27. After seven months of vitamin A supplementation the Arden ratio increased to 3.0. These results reveal that vitamin A deficiency can cause an abnormal EOG.

Reflex modification in psychosis-prone young adults.

The present study examined modification of the startle reflex in psychosis-prone and normal control subjects, using a continuous pure tone prestimulus (S1) and an intense white noise startle stimulus (S2). Reflex modification conditions consisted of stimulus pairs with onset asynchronies of 60, 120, and 2000 ms with a startle-alone condition as control. Startle was indexed by the eyeblink, which was measured by vertical electro-oculography. Subjects were identified as psychosis-prone by their high scores on the Perceptual Aberration or the Physical Anhedonia scales. Group differences in blink

Nyctalopia with normal rod function: a suppression of cones by rods.

Twenty-nine patients with exaggerated rod-cone interaction are described. All were referred because they appeared to suffer from night blindness. ERG and EOGs were performed but were normal. However, investigation with a modified dark-adaptometry technique showed that in these patients cone flicker thresholds rise considerably more during dark adaptation than is normal, and this is sufficient to explain the symptoms. In one case, the condition appears familial. Many patients report their symptoms begin in early adult life and slowly get worse, but we have no objective evidence of progression.

Polysomnography in locked-in syndrome.

Sleep patterns were evaluated in a case of 'locked-in' syndrome. This patient had an ischemic infarction involving the ventral portion of the upper half of the pons bilaterally, with a posteromedial extension into the tegmentum. Reticular structures, notably the median raphe nuclei, supposed to play a major regulatory role in sleep, were most probably involved. Unexpectedly, repeated polysomnographic studies revealed sleep patterns with only minor abnormalities.

Spontaneous vertical eye movements associated with pontine lesions.

Ocular bobbing, dipping and "reversed" ocular dipping were observed and recorded in two patients presenting a severe neurological symptomatology: quadruplegia and coma in one and locked-in syndrome in the other. CT scan showed a pontine infarction in both these patients. This is the first time that the ocular dipping and "reversed" ocular dipping are related to such an anatomic structure.

[Polysomnographic results of uvulo-palato-pharyngoplasty].

The paper deals with the polysomnographic issues from 7 patients diagnosed of obstructive sleeping illness and treated with an UPPP. Tracings were recorded after 3 and 12 months. The comparison between pre- and postoperative findings showed the improvement, being the higher apneic index decrease 50 per cent, among 57 per cent of the cases.

Sleep-wake abnormalities in narcolepsy.

To evaluate the degree to which sleep (REM vs. NREM) intrudes into wake and wake intrudes into sleep in narcolepsy, 103 patients with narcolepsy were compared to 105 patients with other diagnoses of disorders of excessive sleep (DOES). Narcoleptic patients had more frequent REM onsets on the multiple sleep latency test (MSLT) and nocturnal polysomnograms. But the MSLT latencies to REM versus NREM in narcoleptic patients did not differ. Nocturnal measures of REM pressure, percentage of REM, and REM latency excluding the REM onsets, did not differ among patient groups. With respect to the intrus

The nystagmus blockage syndrome. Congenital nystagmus, manifest latent nystagmus, or both?

We have carefully studied, by quantitative oculography, a patient with the nystagmus blockage syndrome (NBS), and two patients with a similar disorder of eye movements that might be mistaken clinically for NBS. Our recordings revealed two distinctly different abnormalities present in a single patient with NBS. Our NBS patient exhibited congenital nystagmus (CN) waveforms when viewing at distance; the CN did not damp with convergence on a near target. When the patient allowed one eye to become esotropic, however, the nystagmus damped considerably and abruptly changed from CN to manifest latent

[Features of the polygraphic picture of natural sleep in patients with subacute sclerosing panencephalitis].

The results of electropolygraphic examinations of 6 patients suffering from subacute sclerosing panencephalitis are presented. The polygraphic examination of sleep revealed a pronounced disorganization of the latter, the degree of which depended on the disease duration (the EEG of awakeness was abnormal, too). An importance of electromyographic changes (within the polygram) for differentiating the states of the sleep and awakeness is demonstrated. The sleep duration, the frequency of Radermaker's complexes, and the heartbeat rate were analyzed. An inverse relationship between the latter two pa

Double saccadic pulses and macrosaccadic oscillations from a focal brainstem lesion.

Double saccadic pulses (DSP) are saccadic intrusions that consist of an initial saccade away from a fixation followed immediately by a return saccade back to the fixation. DSP have been reported in patients with presumed multiple sclerosis and metabolic encephalopathy. However, DSP have not been described in a circumscribed brain lesion. We report a man who developed almost continuous DSP with intervening macrosaccadic oscillations from a circumscribed lesion in the dorsal pontine tegmentum which extended up to the midbrain level and spared the nucleus raphe interpositus where the pause cells

Association between P3 event-related brain potential amplitude and adolescent problem behavior.

This study examined P3 event-related brain potential amplitude and the age of onset of adolescent problem behaviors associated with the development of externalizing psychopathology. Five hundred and one male and 627 female 17-year-old twins reported whether and when they had initiated tobacco, alcohol, or illicit drug use, had police contact, or had sexual intercourse. P3 amplitude was recorded using a visual oddball task. Each of these behaviors was associated with reduced P3 amplitude. When these five behaviors were used to create a composite early problem behavior scale reflecting onset pri

Evaluation of vestibular function in young children.

Together, these tests provide a full evaluation of vestibular function in infants and small children. If attention is paid to creating the appropriate testing environment, and interpretation of the tests takes into account the unique properties of the responses of infants and young children, this testing can provide a useful adjunct to audiometric evaluation in young children.

[Creutzfeldt-Jakob disease associated with ocular dipping--a case report].

We presented a case of Creutzfeldt-Jakob disease (CJD) associated with ocular dipping (OD). A 65-year-old woman was hospitalized with two months history of progressive ataxia and mental deterioration. On admission, she was bed-ridden and stuporous with rigidity of the four limbs. Her both eyes were noted to deviate downward slowly from midpossition, taking 1 to 3 seconds to reach the nadir and rapidly returned to midpossition. It was felt that these abnormal eye movements are compatible with OD. Findings of EOG were compatible with that of typical OD. Electroencephalogram obtained a few months
