[Predictors of early adverse events after the frozen elephant trunk procedure].

AIM: The aim of this study was to identify predictors of adverse events after the frozen elephant trunk procedure in the early postoperative period. PATIENTS AND METHODS: Between March 2012 and March 2020, a total of 273 patients were operated on for aortic pathology. A retrospective analysis aimed at identifying probable predictors was performed on 83 patients who underwent the FET procedure. Uni- and multivariate logistic regression was used to identify predictors of such adverse events as postoperative delirium, respiratory failure, acute kidney injury, and in-hospital mortality. RESULTS:

Ureteropelvic junction obstruction - mimicking an "elephant head" on magnetic resonance imaging.

Ureteropelvic junction obstruction (UPJO) with giant hydronephrosis is relatively rare in adults as compared to children. Most of the UPJO reported or seen in daily practice have a distinct hydronephrosis with a narrow ureteropelvic junction and a collapsed ureter distally. We present images a case of an adult female with Left UPJO, which on MRI mimicked an 'elephant head'.

Lateral Proboscis (Elephant Tusk) with Orofacial Clefts: A Report of a Rare Case.

Lateral proboscis is a rare congenital anomaly. Lateral proboscis is a rare craniofacial malformation characterized by a rudimentary tubular, nose-like structure occurring in association with a wide spectrum of other anomalies. We presented a seven-month-old girl's lateral proboscis, cleft lip, and palate. Proboscis was excised by an elliptical incision, and the cleft was repaired at the same surgery.

The Elephant in the Room: Outbreak of Carfentanil Deaths in Minnesota and the Importance of Multiagency Collaboration.

Over a three-month period in early 2017, the Hennepin County Medical Examiner's Office investigated nine apparent opioid toxicity deaths that occurred in three separate urban, suburban, and rural counties in our jurisdiction. All decedents were known substance abusers and had reportedly recently used heroin; most were found with drug paraphernalia. Complete autopsies variably showed classic stigmata of opioid overdose with no significant injury or natural disease to explain death. Initial toxicology screens failed to identify heroin or other narcotic substances. Several cases were presumptivel

[The frozen elephant trunk technique for the treatment of complex lesions of the thoracic aorta].

The treatment of patients with extensive disease of the thoracic aorta represents a formidable challenge for the cardiovascular surgeon. The beginning of thoracic aortic endovascular aortic arch repair has promoted the development of different hybrid approaches as the frozen elephant trunk (FET) strategy, which includes classic arch replacement and antegrade stenting of the descending thoracic aorta. It represents an interesting approach for patients with extensive disease of the thoracic aorta, and its application has significantly increased over recent years. Our indications for this techniq

Discovery of conventional prolactin from the holocephalan elephant fish, Callorhinchus milii.

The conventional prolactin (PRL), also known as PRL1, is an adenohypophysial hormone that critically regulates various physiological events in reproduction, metabolism, growth, osmoregulation, among others. PRL1 shares its evolutionary origin with PRL2, growth hormone (GH), somatolactin and placental lactogen, which together form the GH/PRL hormone family. Previously, several bioassays implied the existence of PRL1 in elasmobranch pituitaries. However, to date, all attempts to isolate PRL1 from chondrichthyans have been unsuccessful. Here, we cloned PRL1 from the pituitary of the holocephalan

Response to Burgman and Regan: the elephant in the rhetoric on info-gap decision theory.

The formal, rigorous assessment of IGDT in Sniedovich (2012) reveals that this theory's central pillar, namely its robustness model, is a reinvention of a well-established model of local robustness, known universally as radius of stability (circa 1960). As a matter of fact, this robustness model is a simple model derived from Wald's famous maximin paradigm (circa 1940). This means that had there been any gap in the state of the art that IGDT could have possibly presumed to fill, this gap had already been filled decades ago, well before IGDT was even contemplated. The conclusion therefore is th

Experience with the conventional and frozen elephant trunk techniques: a single-centre study.

OBJECTIVES: The treatment of patients with extensive thoracic aortic disease involving the arch and descending/thoracoabdominal aorta is often performed using an elephant trunk procedure. We retrospectively analysed our results comparing two different techniques: the conventional elephant trunk (cET) and the frozen elephant trunk (FET) operation. METHODS: Between January 2003 and December 2011, 171 consecutive patients underwent total aortic arch replacement with either a cET (n = 125) or FET (n = 46) technique. The mean age was 64 ± 13 years and was significantly higher in the FET group (P <

The blind men 'see' the elephant-the many faces of fatty liver disease.

Nonalcoholic fatty liver disease (NAFLD) is a group of diseases with excess fat in liver in the absence of a poorly defined limit of alcohol consumption. Most common variety, a universal public health problem, is associated with insulin resistance caused by a host of genetic and epigenetic defects modulated by life style and environmental factors. In fact the term NAFLD is loose to incorporate so many etiologies except alcoholism and few other etiologies, presenting as fat in liver. However as a sign fatty liver is very important in predicting the risk of diabetes, cardiovascular disease, stro

Arterial blood gas control in the upright versus recumbent Asian elephant.

In the elephant, there is concern that lateral recumbency (LR) impairs respiratory muscle and lung function resulting in clinically significant arterial hypoxemia. Using healthy adult female Asian elephants (Elephas maximus, n=6), the hypothesis was tested that, given the O(2) binding characteristics of elephant blood, substantial reductions in arterial O(2) pressure (Pa(O(2))) in LR could be tolerated without lowering arterial O(2) content appreciably. Fifteen minutes of LR decreased Pa(O(2)) from 103+/-2 (upright, U) to 77+/-4 mmHg (P<0.05) and hemoglobin O(2) saturation (U, 97.8+/-0.1, LR, …

The osteology of the African elephant (Loxodonta africana): vertebral column, ribs and sternum.

The vertebral column, sternum and ribs of the African elephant were studied and illustrated. In the cervical series, the vertebrae are characterized by very short (compressed) vertebral bodies and short spinous processes. There are 20-21 thoracic vertebrae that carry ribs, and three lumbar vertebrae. The neural arches of the five sacral vertebrae fuse with each other as well as with the wings of the ilium, while the intervertebral discs do not ossify and the vertebral bodies remain separate. There are 19-21 caudal vertebrae. In the latter, the neural arches of only the first five to six verteb

The intestine and endocrine pancreas of the African elephant: a histological immunocytochemical and immunofluorescence study.

Histological, immunocytochemical and immunofluorescence methods were employed to study the intestine and endocrine pancreas of the elephant. The histological findings were in line with those in monogastric mammals. In the mucosa of intestine, endocrine cells were immunoreactive to somatostatin, gastrin, CCK, GIP, secretin, motilin, glucagon and NPY. Nerve cells immunoreactive to somatostatin, substance P, VIP, PHI, NPY, bombesin and CGRP were detected. No immunoreactivity to neurotensin was observed, islets of the pancreas had insulin cells in their cores and glucagon and somatostatin cells in

A case of the elephant man phenotype with giant rhinophyma and benign symmetric lipomatosis.

We described an unusual male patient with two disfiguring diseases, benign symmetric lipomatosis and giant rhinophyma, which gave him a grotesque appearance. His physical appearance had isolated him from the rest of society and was probably the main reason for his alcohol abuse.

The Emerging Role of Prolactin as a Biomarker for Reproduction, Health, and Welfare in Wildlife Species: Elephants as a Model.

Prolactin is a key hormone that plays significant roles in reproduction, extending far beyond its well-known function in mammalian lactation … mia is the most common pituitary disorder affecting fertility in women and other species, including elephants, hypoprolactinemia can lead to inadequate parental care due to poor milk production or inadequate brooding. Environmental stressors such as pollution and climate change can further disrupt prolactin levels, compounding reproductive failures. Its sen …

Sexual Dimorphism in the Skeletal Morphology of Asian Elephants (Elephas maximus): A Preliminary Morphometric Study of Skull, Scapula, and Pelvis.

BACKGROUND: Sexual dimorphism in Asian elephants (Elephas maximus) is evident in external features, but skeletal differences remain underexplored. This study aimed to examine the skull, scapula, and pelvis using traditional morphometric methods to assess sex-related variation. METHODS: Eleven skeletal specimens were analyzed, including nine skulls, eleven pelves, and eighteen scapulae. Linear measurements were obtained using measuring tape and calipers. Statistical analyses included Mann-Whitney U tests, Pearson's correlation, and logistic regression for sex prediction. RESULTS: No significa…

Optimisation of bronchoalveolar lavage fluid preparation for mononuclear cell isolation and cytologic evaluation in free-ranging African elephants (Loxodonta africana).

Understanding immune responses to infectious diseases, such as tuberculosis (TB), is essential for developing diagnostic tests and studying disease progression. Although TB affects African savanna elephants (Loxodonta africana), few studies have investigated immune cells and function in this species, especially in the respiratory tract. Techniques for isolating immune cells from elephant bronchoalveolar lavage (BAL) samples have not been previously reported. Therefore, this study aimed to develop and optimise a protocol to isolate and characterise alveolar cell types in BAL fluid collected fro…

Walking Behavior of Zoo Elephants: Associations between GPS-Measured Daily Walking Distances and Environmental Factors, Social Factors, and Welfare Indicators.

Research with humans and other animals suggests that walking benefits physical health. Perhaps because these links have been demonstrated in other species, it has been suggested that walking is important to elephant welfare, and that zoo elephant exhibits should be designed to allow for more walking. Our study is the first to address this suggestion empirically by measuring the mean daily walking distance of elephants in North American zoos, determining the factors that are associated with variations in walking distance, and testing for associations between walking and welfare indicators. We u…
