Triaging in Mass Casualty Incidents: A Simulation-Based Scenario Training for Emergency Care Senior Residents.

BACKGROUND: Mass casualty incidents (MCIs) are events where the number of patients may exceed the resources available for emergency management. In emergency departments (ED), MCIs go along with high patient allocation in a short time period. For ED staff, simulation-based immersive scenario training can be an effective tool to develop communicative and leadership competencies to manage these situations. APPROACH: A novel simulation-based immersion experience was developed for senior residents in emergency care. To our knowledge, this is the first scenario training focusing on in-hospital tria

COVID-19 Mass Vaccination and Flu Season: Concern for Decreased Public Health Measures and Worsening the Influenza Situation.

Reports show that other ordinary childhood infections like measles or influenza are likely to reemerge. The re-emergence of infectious diseases may happen due to the direct impact of the pandemic on the community because of decreased access to health and medical services, interrupted transport systems, weaknesses in the supply chain, flight restrictions, closings of the border, and international trade problems. The most prevalent cause (60.9%) for low vaccine uptake and coverage during the current pandemic was fear of exposure to the COVID-19 virus outside the home. The expectation and hope th

Seasonal and pandemic influenza preparedness: a global threat.

The increase in the incidence of avian influenza worldwide in both poultry and humans introduces the potential for another influenza A pandemic that could pose a significant threat to both human health and the global economy. The impact of the next influenza pandemic will be influenced, in part, by how well the medical, government, business, and lay communities are prepared. Despite the additional tools and resources that have become available since prior epidemics, there are limits to the quantity of antiviral drugs that can be manufactured and concerns over the current vaccine production sys

Relying on the National Mobile Disaster Hospital as a business continuity strategy in the aftermath of a tornado: The Louisville experience.

On 28th April, 2014, a tornado left much of Louisville, MS and the local hospital, Winston Medical Center, in ruin. In the USA, temporary hospital solutions have been used to augment the mainstream healthcare system since the American Civil War. As memories fade, however, the necessary readiness for a temporary hospital also fades, at times leaving a patchwork of resources either underfunded or abandoned. With the creation of the Hospital Preparedness Program, several temporary hospital solutions were created in various states across the USA. In the present case, Mississippi and North Carolina

Focus on anorexia nervosa: modern psychological treatment and guidelines for the adolescent patient.

Anorexia nervosa is a serious condition associated with high mortality. Incidence is highest for female adolescents, and prevalence data highlight a pressing unmet need for treatment. While there is evidence that adolescent-onset anorexia has relatively high rates of eventual recovery, the illness is often protracted, and even after recovery from the eating disorder there is an ongoing vulnerability to psychosocial problems in later life. Family therapy for anorexia in adolescence has evolved from a generic systemic treatment into an eating disorder-specific format (family therapy for anorexia

Pragmatic trial of a virtual dementia collaborative care management program: protocol for the Aging Brain Care Virtual (ABCV) program.

INTRODUCTION: Providing care management, treatment and support to patients with Alzheimer's Disease and Related Dementias (ADRD) is a difficult task for health systems. Over the past 20 years, interventions designed to improve outcomes for patients living in the community with dementia and their care partners have moved progressively, but separately, from large scale trials and pragmatic models of collaborative care. Given the projected increase in the number of people living with dementia coupled with the realignment of payment for services to be value-based and provided in the community, sys

Extreme weather events and HIV: development of a conceptual framework through qualitative interviews with people with HIV impacted by the California wildfires and their clinicians.

BACKGROUND: People with HIV (PWH) are disproportionately vulnerable to the impacts of wildfires, given the need for frequent access to healthcare systems, higher burden of comorbidities, higher food insecurity, mental and behavioral health challenges, and challenges of living with HIV in a rural area. In this study, we aim to better understand the pathways through which wildfires impact health outcomes among PWH. METHODS: From October 2021 through February 2022, we conducted individual semi-structured qualitative interviews with PWH impacted by the Northern California wildfires and clinicians

Integrating Cancer Education Into High School Curricula.

Cancer is a leading cause of death and its incidence is rising among younger populations, yet formal cancer education is rarely included in U.S. high school curricula. Adolescence is a critical period for establishing lifelong health behaviors, and emerging evidence suggests that school-based cancer education can improve knowledge, shift attitudes, and promote risk-reducing behaviors. In this narrative review, we summarize studies of high school and middle school cancer education programs, including curricula focused on cancer biology, modifiable risk factors (e.g., tobacco, alcohol, diet, phy

Diagnostics barriers and innovations in rural areas: insights from junior medical doctors on the frontlines of rural care in Peru.

BACKGROUND: Worldwide, rural communities face barriers when accessing health services. In response, numerous initiatives have focused on fostering technological innovations, new management approaches and health policies. Research suggests that the most successful innovations are those involving stakeholders at all levels. However, there is little evidence exploring the opinions of local health providers that could contribute with further innovation development and research. The aims of this study were to explore the perspectives of medical doctors (MDs) working in rural areas of Peru, regardin

Trends in antifungal use among hospitalized patients in the USA, 2018-23.

BACKGROUND AND OBJECTIVES: Fungal infections cause substantial morbidity and mortality. Monitoring antifungal use is a foundational aspect of antifungal stewardship, particularly as new disease-causing fungi emerge and antifungal resistance spreads. We assessed recent patterns in systemic antifungal medication use among hospitalized patients within a diverse convenience sample of academic medical centres and community hospitals in the USA. METHODS: We conducted a multicentre retrospective cohort study using the Vizient® Clinical Data Base. We selected hospitalized patients who received ≥1 dos

'Just a delirium': a qualitative study of care home managers' perspectives on barriers to delirium recognition and management in UK care homes.

Delirium is a common but frequently under-recognised neuropsychiatric syndrome in care homes, especially among residents living with dementia. Delirium is associated with substantial morbidity, mortality and preventable healthcare complications, yet evidence on how it is recognised and managed in residential care remains limited. This qualitative study explored care home managers' perspectives on delirium care within privately owned care homes in a region of England, UK, and data analysed thematically to identify key challenges and opportunities for improvement. Three overarching themes were i

Exploring the evolution of US medical students' social support networks across the continuum: a cross-sectional group interview study.

Perceived social support - feeling cared for and having access to a reliable network - has been linked to reduced stress and burnout, and greater quality of life among medical students. Yet, how perceived support systems evolve throughout medical school remains little explored. We examined how medical students' perceived social support systems shift during medical school adaptively to students' changing academic and emotional needs. Between January and May 2023, we conducted six hour-long group interviews with a cross-sectional convenience sample of 50 medical students (graduating Classes of 2

Impacts of Residential Segregation and Comorbidity on Racial Disparities in Acute Care Utilization.

PURPOSE: To examine whether living in highly segregated communities or having comorbidity is associated with an increased risk of acute use of emergency and inpatient services (i.e., acute care utilization [ACU]) among racial minorities. METHODOLOGY: This longitudinal study included adult patients who underwent health-related social needs screening during their medical appointments within the Mass General Brigham healthcare system from March 2018 to January 2023. Our primary outcome was 180-day ACU defined as a composite outcome of all-cause emergency departments visits or hospital admissions

Stakeholder Roles and Views in the Implementation of the Differentiated HIV Treatment Service Delivery Model Among Female Sex Workers in Gauteng Province, South Africa.

Background: Key populations (KPs), particularly female sex workers (FSWs), continue to face significant barriers in accessing HIV-related healthcare services in South Africa. Structural challenges have historically hindered equitable HIV treatment access, worsened by the COVID-19 pandemic. Overburdened clinics, staff shortages, and travel constraints disrupted HIV services and ART adherence. In response, the Differentiated Service Delivery (DSD) model was rapidly scaled up to decentralise care and improve treatment continuity. Objective: To solicit the views of stakeholders regarding their int

Types of Challenges and Barriers in Accessing Paediatric Palliative Care in Romania: A Qualitative Study Based on Focus Groups Guided by a Semi-Structured Discussion Guide.

Background and Objectives: Paediatric palliative care in Romania is underdeveloped and unevenly distributed, which creates major difficulties in accessing services for children with life-limiting illnesses and their families. The lack of a dedicated national strategy, the shortage of specialised staff, and socio-economic barriers exacerbate the vulnerability of these groups. This study aimed to explore parents' and caregivers' experiences, to analyse the perspectives of public institutions and NGOs involved in supporting these children, and to identify the main barriers and facilitators in acc
