Evolution of surgical treatment of primary hyperparathyroidism in patients with multiple endocrine neoplasia type 2A.

OBJECTIVE: To determine the best surgical strategy for patients with multiple endocrine neoplasia type 2A (MEN 2A) who have primary hyperparathyroidism (PHPT). METHODS: We performed a systematic literature review and conducted a retrospective cohort study that included patients with PHPT identified from the MEN 2A database at the University Medical Center of Utrecht, Utrecht, the Netherlands, between 1979 and 2009. RESULTS: The review describes the course of worldwide parathyroid surgical management in MEN 2A PHPT over the past 75 years, which has evolved from aggressive parathyroid resectio

Interpretation of intra-operative PTH changes in patients with multi-glandular primary hyperparathyroidism (pHPT).

BACKGROUND: The correct interpretation of intraoperative parathyroid hormone (IOPTH) levels in patients with primary hyperparathyroid patients (pHPT) with multiglandular disease (MGD) can impact the success rate of initial parathyroid exploration, but it remains an understudied topic. METHODS: In all, 592 of 823 patients were explored by a single surgeon with biochemical evidence of pHPT (1997-2007) and underwent parathyroidectomy using IOPTH. We investigated the incidence of MGD in patients with an appreciable (>50%) decrease in IOPTH levels and in patients whose levels failed to normalize a

Endocrine response to surgery in children after premedication with midazolam or papaveretum.

The effect of two premedications on the sympatho-adrenal and endocrine stress-response to minor surgery under halothane anaesthesia was investigated in 16 children. One group (n = 9) was premedicated with midazolam, 0.1 mg kg-1, and atropine 0.2-0.4 mg i.m. The other group (n = 7) received papaveretum 0.4 mg kg-1 and hyoscine 0.008 mg kg-1 i.m. Plasma concentrations of catecholamines, ACTH and cortisol were measured during undisturbed anaesthesia, during surgery and 15 min post-operatively. There were no differences in catecholamine concentrations between the groups. Prior to surgery, plasma A

Pituitary Apoplexy: Results of Surgical and Conservative Management Clinical Series and Review of the Literature.

OBJECTIVE: Pituitary apoplexy is associated with visual, cranial nerve, and endocrine dysfunction. In this article, the results of surgical and conservative management of pituitary apoplexy in a single center are evaluated and a review of the literature is presented. METHODS: A retrospective analysis was made of patients with pituitary apoplexy who underwent surgery or conservative management at our center between January 2007 and June 2017. Surgery was typically selected for patients who presented with acute deterioration of visual status and/or level of consciousness. Patients with no visua

ASO Author Reflections: Patient Selection and Technique for Robotic Spleen-Preserving Distal Pancreatectomy.

Spleen-preserving distal pancreatectomy is an increasingly preferred approach for patients with multiple endocrine neoplasia type 1 (MEN1) and pancreatic neuroendocrine tumors (pNETs), offering potential long-term benefits, such as preserved immunologic function and reduced thromboembolic or oncologic risks. While traditional distal pancreatectomy with splenectomy facilitates lymph node clearance, it carries significant short- and long-term complications. In our recently published video, we demonstrate a robotic spleen-preserving distal pancreatectomy using the Kimura technique in two patients

[30 years of prophylactic thyroidectomy for hereditary medullary thyroid cancer : A milestone in translational medicine].

Medullary thyroid cancer (MTC) is the most frequent manifestation of multiple endocrine neoplasia type 2 (MEN2) that determines the oncological outcome. Germline mutations in the rearranged during transfection (RET) protooncogene, a tumor suppressor gene on chromosome 10q11.2, were identified 30 years ago as the genetic basis of MEN2 and published in 1993 and 1994. These seminal findings gave rise to the concept of prophylactic thyroidectomy for asymptomatic gene mutation carriers based on a positive RET gene test, which has become the standard of care ever since. Clinical genetic investigatio

Hemodynamic instability during surgery for pheochromocytoma: comparing the transperitoneal and retroperitoneal approach in a multicenter analysis of 341 patients.

BACKGROUND: Intraoperative hemodynamic instability is a major challenge during adrenalectomy for pheochromocytoma. Typically, pheochromocytoma is performed laparoscopically either through the retroperitoneal or transperitoneal approach. We aimed to determine if the operative approach affects intraoperative hemodynamic instability during surgery for pheochromocytoma in a large multicenter multicenter cohort. METHODS: Retrospective, multicenter analysis of consecutive patients with pheochromocytoma who underwent total unilateral laparoscopic adrenalectomy without conversion were included. Stati

Perioperative kinetics of parathyroid hormone in unilateral, primary thyroid surgery.

Dysfunction of the parathyroid glands is an important cause of complications after thyroid surgery. Intraoperative monitoring of the function of the parathyroid glands can be performed using parathyroid hormone (PTH) kinetics. Unilateral thyroid surgery is associated with a decreased risk for postoperative hypocalcemia (POH) and permanent hypoparathyroidism (PEH). We focused on unilateral thyroid surgery by monitoring the functionality of the parathyroid glands and comparing the perioperative PTH kinetics of patients with and without POH. In a prospective study, 143 patients scheduled for unil

[Influence of peptides from pineal gland on thymus function at aging].

The interference between thymus and pineal gland during their involution is considered in this review. The research data about influence of thymus peptides on pineal gland and pineal peptides on thymus is summarized. Analysis of these data showed that pineal peptides (epithalamin, epitalon) had more effective geroprotective effect on thymus involution in comparison with geroprotective effect of thymic peptides (thymalin, thymogen) on involution of pineal gland. The key mechanisms of pineal peptides effect on thymus dystrophy is immunoendocrine cooperation, which is realized as transcription's

Timing and special concerns regarding endocrine surgery during pregnancy.

Pregnancy may occur in the setting of endocrine diseases or be the first time these diseases manifest clinically. Management of pregnancy in these circumstances is challenging and requires a high degree of vigilance on the part of the treating physicians. The best outcome is achieved by a multidisciplinary approach consisting of endocrinology, obstetrics, anesthesiology, and endocrine surgery.

Conservative treatment of chronic pancreatitis.

BACKGROUND: Chronic pancreatitis is a progressive inflammatory disease giving rise to several complications that need to be treated accordingly. Because pancreatic surgery has significant morbidity and mortality, less invasive therapy seems to be an attractive option. AIM: This paper reviews current state-of-the-art strategies to treat chronic pancreatitis without surgery and the current guidelines for the medical therapy of chronic pancreatitis. RESULTS: Endoscopic therapy of complications of chronic pancreatitis such as pain, main pancreatic duct strictures and stones as well as pseudocyst

Pituitary apoplexy following cataract surgery: a case report.

BACKGROUND: Pituitary apoplexy is a rare, life-threatening complication of pituitary macroadenomas which is often triggered by physiological stressors. It typically occurs spontaneously or may be precipitated by surgical procedures. However, occurrence after cataract surgery is exceedingly rare. CASE PRESENTATION: A 61-year-old Pakistani hypertensive male presented with sudden-onset severe headache and projectile vomiting, 3 day post-cataract surgery. Imaging revealed a hemorrhagic pituitary macroadenoma consistent with apoplexy. Neurologic examination was normal. The patient was managed cons

Multimodal Management of Grade 1 and 2 Pancreatic Neuroendocrine Tumors.

Pancreatic neuroendocrine tumors (p-NETs) are rare tumors with a recent growing incidence. In the 2017 WHO classification, p-NETs are classified into well-differentiated (i.e., p-NETs grade 1 to 3) and poorly differentiated neuroendocrine carcinomas (i.e., p-NECs). P-NETs G1 and G2 are often non-functioning tumors, of which the prognosis depends on the metastatic status. In the localized setting, p-NETs should be surgically managed, as no benefit for adjuvant chemotherapy has been demonstrated. Parenchymal sparing resection, including both duodenum and pancreas, are safe procedures in selected

Surgical treatment of pancreas divisum causing chronic pancreatitis: the outcome benefits of duodenum-preserving pancreatic head resection.

Pancreas divisum (PD) represents a duct anomaly in the pancreatic head ducts, leading frequently leading to recurrent acute pancreatitis (rAP) or chronic pancreatitis (CP). Based on endoscopic retrograde cholangiopancreatography, pancreas divisum can be found in 1% to 6% of patients with pancreatitis. The correlation of this abnormality with pancreatic disease is an issue of continuing controversy. Because of the underlying duct anomalies and major pathomorphological changes in the pancreatic head, duodenum-preserving pancreatic head resection (DPPHR) offers an option for causal treatment. Thi

Differentiated thyroid cancer in children and adolescents: the importance of adequate surgery and review of literature.

Within 25 years, 13 girls and 5 boys between the ages of 9 to 20 years were treated for differentiated thyroid cancer (DTC) at our institution. In 67% of the cases the diagnosis was made at an advanced stage indicated by cervical lymphnode metastases, two children showed metastases of the lung. Except for one girl, all patients underwent a total thyroidectomy with various forms of lymph node dissections. All were treated post-surgically with radio-iodine (131I) and all patients received levothyroxine in suppressive doses. The complication rate was low (laryngeal nerve injury in only 3 cases an

Neuro-endocrine adenoma of the middle ear: a case study.

Neuroendocrine adenomas are rare tumors, which can appear in the middle ear. Approximately a hundred cases have been reported in the literature. We report the case of a 58-year-old man who consulted for an abnormal sensation of fullness in the right ear. The otoscopic examination showed a retrotympanic tumefaction. The CT scan and MRI of the middle ear demonstrated a well-defined tissue mass without any osteolysis. We performed surgical exeresis by transcanal procedure with a cartilage graft tympanoplasty. Microscopic examination and immunohistochemistry revealed an endocrine adenoma of the mi

Lactation Management in Patients With Cancer: An Evidence-Based Framework for Surgical Practice.

BackgroundThe coexistence of lactation and cancer care, termed oncolactation, is becoming increasingly common as childbearing is delayed and more women are diagnosed with malignancy during the reproductive years. However, evidence-based guidance regarding breastfeeding during oncologic evaluation and treatment remains fragmented, particularly for surgeons involved in multidisciplinary cancer care.MethodsWe conducted a narrative review with a systematic literature search using PubMed/MEDLINE and professional society guidelines (American Society of Breast Surgeons (ASBrS), American College of Ra
