The human endolymphatic sac (ES) is situated in a duplicature of the dura in the posterior cranial fossa and constitutes a part of the inner ear. The sac possesses immunological capacities and is responsible for a major part of the trans-epithelial ion transport occurring within the inner ear, via molecular mechanisms similar to that of the kidney collecting duct epithelia. Dysfunction of the trans-epithelial ion transport has been hypothesized as the reason for the endolymphatic hydrops occurring in Menieres diseases. Thus, candidate drug selection for medical treatment of Menieres disease ha…

The nature of surgical treatment for Ménière's disease has evolved from destructive to conservative in the interest of preserving hearing. We have performed a combined procedure that involves a vestibular neurectomy to control vertigo, which is followed by an endolymphatic sac shunt to control hydrops in 26 patients. Both procedures are performed via the retrosigmoid approach during the same surgical step. We believe this combination procedure is a worthwhile option to consider in order to achieve good control of vertigo and endolymphatic hydrops and to preserve hearing in patients with Ménièr…

The transepithelial potential in the endolymphatic sac (ESP) was recorded up to 60 min after apical injection of ouabain, bumetanide, quinine, barium, tetraethylammonium, and 4-aminopyridine. After control injection, ESP decreased by 74% and completely recovered at 30 min. After ouabain, barium, or quinine injection, the ESP time course was similar to that in the control group. After bumetanide, tetraethylammonium, or 4-aminopyridine injection, complete recovery was only observed at 60 min. These results suggest that apical K+ conductance and Na-K-2Cl cotransporter could be involved in the gen…

Between April 1990 and June 1996, the extraosseous portion of endolymphatic sac has been removed from the affected ear in 77 patients suffering from Menière's disease. Removal of the extraosseous part of the endolymphatic sac without any drainage procedure did not increase the frequency or severity of the attacks of vertigo in any patient. The results of 43 patients with unilateral disease who had a follow-up period of two years are presented. Only eight of the patients had more than two recurrent attacks of vertigo lasting over two minutes within the two years after the surgery, and in three …

The effect of blood pressure (BP) changes on per-rotational nystagmus (PRN) was compared between normal guinea pigs and guinea pigs with unilaterally obliterated endolymphatic sac. Eye movements were recorded 3 months postoperatively. When the animal was rotated sinusoidally, asymmetry of PRN was minimal in the control animals. In the operated guinea pigs, however, nystagmus oscillating toward the operated side was largely suppressed when BP was decreased (30-40 mmHg). Thus, directional preponderance of PRN toward the nonoperated side was established. Oxygen tension (pO2) was measured in the p…

We used light microscopy and computerized graphic reconstruction techniques to examine the endolymphatic duct and sac in 20 pairs of bones f … ular aqueduct but also of the sac is reduced in Menière's disease. The results may suggest that the endolymphatic sac is pathologically changed in Menière's disease and that a reduced resorptive capacity of a small endolymphatic sac could result in endolymphatic hydrops.

OBJECTIVE: Endolymphatic sac tumours (ELSTs) of the inner ear occur in 16% of patients with the hereditary tumor syndrome von Hippel-Lindau disease (vHL). ELSTs of all sizes can cause irreversible hearing loss which can, however, be prevented through early diagnosis and treatment. We aim to emphasize the challenges of prophylactic ELST screening and to explore the role of audiometry in pre-symptomatic ELST screening.
DESIGN: For a period of 17 years our patient was screened for ELSTs with inner-ear MRI (magnetic resonance imaging), audiometry, and clinical interviews.

By using the whole-cell patch-clamp technique, an amiloride-sensitive Na+-selective conductance was found in epithelial cells from the endolymphatic sac (ES) epithelia of guinea-pigs. In the current-clamp configuration, the average resting membrane potential was -41.7+/-8.4 mV (n = 22). Application of amiloride at a concentration of 20 microM elicited a decrease in cation conductance that was responsible for a membrane hyperpolarization by 17.9+/-6.0 mV (n = 22). Substitution of N-methyl d-glucamine chloride (NMDG-Cl) for external NaCl led to a more significant membrane hyperpolarization by 28…

Endolymphatic system surgery for Meniere's disease has been shown to be effective in selected cases; however, some patients do not achieve substantial long-term relief from their symptoms in spite of early success and may require revision surgery. Revision endolymphatic system surgery has been reported in the literature in 9.7% to 16.3% of patients. As part of an ongoing prospective analysis of endolymphatic valve implant surgery, the first 400 procedures performed over 6 years were analyzed to determine the rate and cause for revision surgery. Fifty-four of 386 (13.9%) of endolymphatic valve

Meniere's disease is a common disease, that presents with recurrent vertigo and cochlear symptoms. The pathology of Meniere's disease was first reported to involve endolymphatic hydrops in 1938. The endolymphatic sac is thought to have a role to keep the hydrostatic pressure and endolymph homeostasis for the inner ear. As a surgery for intractable Meniere's disease, endolymphatic sac drainage with intraendolymphatic sac application of large doses of steroids is performed to control the endolymphatic hydrops and preserve or improve inner ear function. In the present study, to observe the effect…

Background/Objectives: Endolymphatic sac surgery (ELSS) is a non-destructive surgical option for medically refractory Ménière's disease (MD), yet factors influencing surgical outcomes remain poorly understood. This exploratory study aimed to describe clinical outcomes following ELSS and identify potential associations between preoperative characteristics and surgical success. Methods: This retrospective, bicentric cohort study included 45 patients with definite MD who underwent ELSS (predominantly endolymphatic duct blockage) between 2019 and 2024. Vertigo control was assessed using AAO-HNS cr…

The endolymphatic sac (ES) is a cystic structure of the inner ear connected to the cochlea and vestibule, which plays a role in regulating ion homeostasis in inner ear fluid. Disruption of ion homeostasis can cause inner ear disorders with hearing loss and dizziness, such as Meniere's disease. Herein, we found, for the first time, functional evidence for the involvement of β1- and β2-adrenergic receptors in apical electrogenic ion transport by human ES epithelium by using electrophysiological/pharmacological and molecular biological methods, which were dependent on K+ and Cl- ion transport. Th…

This report describes a patient with von Hippel-Lindau disease who presented with an 8-year history of a slow-growing, locally invasive vascularized lesion of the temporal bone involving the cerebellopontine angle. The mass, studied by computerized tomography scanning and magnetic resonance imaging techniques, was partly cystic in appearance. After removal of the mass, pathological studies confirmed a papillary cystic tumor with characteristics that have been described in tumors with an endolymphatic sac origin. These rare neoplasms constitute a distinct pathological entity and deserve wider r…

Endolymphatic sac tumors are rare neoplasms arising from the posterior aspect of the petrous temporal bone and are characterized by locally aggressive behavior with significant bone destruction. Although they may occur sporadically, a well-established association with von Hippel-Lindau (VHL) disease has been reported. This study aimed to characterize the clinical features, management, and long-term outcomes of a consecutive series of seven patients treated at a tertiary referral center, including cases associated with VHL. A retrospective analysis of institutional cases was performed and compl…
