Musculoskeletal pain in psoriasis-relation to inflammation and additional value of ultrasound in psoriatic arthritis classification.

OBJECTIVE: To investigate and compare clinical features and US signs of inflammation in joints and entheses in patients with psoriasis with and without musculoskeletal pain, and the additional value of US in classification of PsA. Furthermore, to explore the association between such findings and patient-reported outcomes (PROs) and the performance of screening-questionnaires for identifying patients with PsA. METHODS: Patients with psoriasis (n = 126) recruited from a nationwide survey were evaluated at one of four rheumatology departments. The evaluation included clinical examination, labora

Controversies in rheumatology: imaging of enthesitis in spondyloarthritis-does it mean anything for treatment decisions?

Enthesitis is considered a hallmark manifestation of spondyloarthritis including axial spondyloarthritis and psoriatic arthritis. Detection of enthesitis might be challenging in both diagnostic and classification processes. In this debate, we discuss the controversy on the role of imaging in the detection of enthesitis including the relevance for treatment decisions in spondyloarthritis.

Reliability assessment of the definition of ultrasound enthesitis in SpA: results of a large, multicentre, international, web-based study.

OBJECTIVES: To investigate the reliability of the OMERACT US Task Force definition of US enthesitis in SpA. METHODS: In this web exercise, based on the evaluation of 101 images and 39 clips of the main entheses of the lower limbs, the elementary components included in the OMERACT definition of US enthesitis in SpA (hypoechoic areas, entheseal thickening, power Doppler signal at the enthesis, enthesophytes/calcifications, bone erosions) were assessed by 47 rheumatologists from 37 rheumatology centres in 15 countries. Inter- and intra-observer reliability of the US components of enthesitis was

Tender to touch-Prevalence and impact of concomitant fibromyalgia and enthesitis in spondyloarthritis: An ancillary analysis of the ASAS PerSpA study.

OBJECTIVES: The primary objective was to evaluate the co-existence of fibromyalgia (FM) & enthesitis in individuals with spondyloarthritis (SpA). Secondary objectives were to identify clinical features associated with the presence of FM in enthesitis and analyse sex-specific differences. METHODS: This was an ancillary analysis of the Assessment of SpondyloArthritis International Society Peripheral Involvement in SpA (PerSpA) study. Enthesitis was defined as the presence of enthesitis ever. Clinical FM was defined as the rheumatologist's confirmation of the presence of FM. A score of≥5/6 on th

Differentiating psoriatic arthritis sine psoriasis from seronegative rheumatoid arthritis-Experiences from five patients.

Psoriatic arthritis (PsA) without skin lesions is frequently confused with seronegative rheumatoid arthritis (RA). In this study, we aimed to promote diagnostic accuracy. Five PsA patients with no skin lesions were reviewed. We analyzed the clinical characteristics of these patients. All patients had multiple peripheral arthritis as well as axial involvement, and had been misdiagnosed with RA for several years initially. They developed severe deformation as a result of delayed diagnosis and inadequate treatment. Four patients had nail changes and one had a family history of psoriasis. They had

The Value of Musculoskeletal Ultrasound for Evaluation of Postinfectious Lyme Arthritis.

BACKGROUND: Inflammatory arthritis is the most common late manifestation of untreated Lyme disease in the United States. While antimicrobial therapy is effective in resolving swelling and pain for 90% of patients, many patients have persistent inflammation, termed postinfectious Lyme arthritis (PILA). Current outcome measures for Lyme arthritis have several limitations, as improvement is considered a dichotomous outcome based solely on physical examination. There is growing interest in the use of ultrasonography to better define outcomes in inflammatory arthritis, and this is particularly rele

Oligoarticular vs Polyarticular Psoriatic Arthritis: A Longitudinal Study Showing Similar Characteristics.

OBJECTIVE: The objectives of this study were to determine whether patients with oligoarticular presentation differ from those with polyarticular presentation and to identify potential predictors for evolution of oligoarthritis to polyarthritis in patients with psoriatic arthritis (PsA). METHODS: Patients who entered the University of Toronto PsA clinic between 1978 and 2018 within 12 months of diagnosis were identified. Only patients with ≥ 2 clinic visits were included. Patients were followed at 6- to 12-month intervals according to standard protocol, which included demographics, clinical hi

Spondylarthropathy and selective IgA deficiency.

No serum IgA was detected in a young male patient suffering from spondylarthropathy (SpA) with bilateral sacroiliitis arthritis, enthesopathy and inflammatory low back pain, whose symptoms occurred in reaction to a sexually induced urethritis. After a period of several months in which the spondylarthropathy was active, disease activity came to a rest. Three years later no progression of the SpA was observed. This finding might be an indication that IgA is not involved in the pathogenesis of spondylarthropathy.

Pathogenesis of some traumatic and degenerative disorders of soft tissue.

1. Trauma to soft tissues is associated with tissue rupture and formation of a haematoma. 2. Such lesions cannot be expected to resolve without complications in the absence of appropriate treatment. 3. Myositis ossificans should be anticipated in lesions of muscles adjacent to their attachments. 4. Chronic tendonitis and enthesopathies not associated with inflammatory connective tissue disease are primarily degenerative in nature and are probably the result of comparative ischaemia. Mesenchymoid transformation is a feature of both.

Cartilage Degeneration of the Metacarpal Condyle and Enthesopaties of The Collateral Ligaments of Equine Metacarpophalangeal Joint.

Articular degeneration can be characterized by fibrillation and eburnation of the articular layers of the metacarpophalangeal (MP) joint. Structural changes within the articular joint predispose the development of osteophytes, enthesophytes and, in many cases is associated with changes the collateral ligaments of the MP joint. The aim of the present study was to investigate the relationship between degenerative processes of the articular cartilage and the enthesopathies of collateral ligaments (superficial or deep portion) of the equine MP joint, using radiographic, ultrasonographic and anatom

[HLA-B27 associated spondyloarthritis in childhood].

Ankylosing spondylitis, indefinite juvenile spondyloarthritis (JSA), reactive arthritides, psoriasis arthritis, arthritis of inflammatory bowel disease, and probably other rare diseases, such as the Behçet-syndrome, are sharing several distinctive features which discriminate them as a group from other arthritides, particularly from the rheumatoid arthritis in children (JRA) and adults. These distinctive features are: familial aggregation; association with HLA-B27; enthesopathy, sacroiliitis; predominance of Lower-Limb-arthritis; onset mostly after the age of 10 years, and typical manifestation…

[The hyperostotic shoulder].

A systematic study of shoulder joints in 30 DISH patients demonstrates the frequency of shoulder involvement in this disease and its characteristic radio-clinical features. Radiological abnormalities observed in DISH shoulders are contrasted with those encountered in rotator cuff tears. However, both disorders can occur together, producing a "complex shoulder" syndrome. The most typical aspect of this is the presence of an osteoarthritic pseudoarthrosis between the greater tuberosity and the acromion which has developed from the subacromial enthesopathy.

Ultrasound Evaluation of Achilles Tendinopathy.

INTRODUCTION: We sought to highlight the significance of ultrasound in assessing injuries to the Achilles tendon. METHODS: Using a case study approach, we present two case reviews involving insertional enthesopathy and tendonitis of the Achilles. RESULTS: Our study shows the lesions in the Achilles tendon and the peritendinous structures noted on clinical examination of the patients reviewed were better characterized with the use of ultrasound. CONCLUSION: This outcome is in line with information from available literature and current practice.

Cervical anterior hyperostosis: a rare cause of dysphagia. Report of 3 cases.

Disorders of the cervical spine rarely cause dysphagia and/or dysphonia. Exuberant osteophytosis, secondary to diffuse idiopathic skeletal hyperostosis, also referred to as Forestier's disease, seems to be the predominant cause. Forestier's disease is a non inflammatory enthesopathy, of unknown etiology, ossifying the anterior longitudinal ligament of the spine, sparing the discs, and usually affecting older men. We describe the successful surgical treatment of 3 cases, over the course of 26 years, observed in our Neurosurgical Unit.

Echotexture Analysis of L4 Supraspinous Enthesis in Ankylosing Spondylitis.

BACKGROUND: Enthesopathy is a main characteristic of ankylosing spondylitis (AS). However, ultrasonographic features of supraspinous enthesis in AS have not yet been reported. METHODS: Forty-seven AS patients and 22 healthy individuals were enrolled and completed the study. L4 supraspinous entheses were assessed through an ultrasound (US) unit with the participants in a lateral decubitus position. Entheseal echogenicity was interpreted upon inspection of the US image. An entheseal grayscale (GS) value determination, along with an echotexture analysis using a gray-level co-occurrence matrix al…
