Can ventricular tachycardia non-inducibility after ablation predict reduced ventricular tachycardia recurrence and mortality in patients with non-ischemic cardiomyopathy? A meta-analysis of twenty-four observational studies.

BACKGROUND: At present, the role of ventricular tachycardia (VT) non-inducibility after ablation in patients with non-ischemic cardiomyopathy (NICM) remains controversial. We conducted a meta-analysis of the published literature to assess whether VT non-inducibility after ablation could predict reduced VT recurrence and mortality in patients with NICM. METHODS: PubMed, ScienceDirect, and the Cochrane library were searched for studies evaluating the effects of VT non-inducibility after catheter ablation on the long-term outcome in NICM patients with sustained VT. Results were analyzed using a

Surgical and concomitant epicardial-endocardial (hybrid) ablation of persistent and long-standing persistent atrial fibrillation.

Catheter ablation of atrial fibrillation (AF) has been shown to be effective for paroxysmal AF. However, for patients with persistent or longstanding persistent AF, the success rates for catheter ablation is low. The Cox-Maze procedure is the most effective non-pharmacological treatment of AF. However, due to the need for open-heart surgery and the morbidity associated with the surgical Cox-Maze procedure, minimally invasive and epicardial-endocardial (hybrid) ablation procedures have been developed. This article will review the main surgical and hybrid approaches used for the treatment of per

Comparison of the arrhythmogenic substrate between men and women with nonischemic cardiomyopathy.

BACKGROUND: Outcomes of ventricular tachycardia (VT) ablation in structural heart disease have been reported to differ by sex. Whether this is due to differences in the underlying arrhythmogenic substrates among patients with nonischemic cardiomyopathy (NICM) remains unclear. OBJECTIVE: The purpose of this study was to compare the characteristics of arrhythmogenic substrates between women and men with NICM. METHODS: We analyzed 160 consecutive patients (26 women) with NICM who were undergoing VT ablation at the Hospital of the University of Pennsylvania. Of these 160 patients, 59 (13 women)

Accessory atrioventricular pathway in an infant: prediction of location with body surface maps and ablation with cryosurgery.

A 10-month-old infant with multiple muscular ventricular septal defects, congestive heart failure, Wolff-Parkinson-White syndrome, and supraventricular tachycardia is presented. The site of ventricular pre-excitation was predicted by analysis of ST-T wave isopotential body surface maps to be in the posterior free wall of the right ventricle. The site was confirmed by epicardial mapping of the ventricles during surgery. The pathyway was cryoblated and the ventricular defects were closed. The patient has been free of pre-excitation and supraventricular tachycardia for over two years since surger…

Successful ventricular tachycardia radioablation in a patient with previous chemical pleurodesis: A case report.

INTRODUCTION: Stereotactic arrhythmia radioablation (STAR) is a novel technique for the ablation of ventricular tachycardia in patients with contraindications to standard procedures, i.e., radiofrequency ablation. CASE PRESENTATION: We report the case of a 73-year-old man with non-ischemic dilated cardiomyopathy and recurrent VT episodes. Electroanatomic mapping showed VT prevalently of epicardial origin, but direct epicardial access through subxyphoid puncture could not be performed due to pleuropericardial adhesions from a past history of chemical pleurodesis. STAR was performed, with no VT

Epicardial ablation of ventricular tachycardia in a patient with dilated cardiomyopathy due to Becker muscular dystrophy: a case report.

BACKGROUND: Becker muscular dystrophy (BMD) is frequently associated with cardiac involvement. The underlying pathoanatomical substrate includes replacement of cardiomyocytes by fibrous tissue, leading to extensive myocardial fibrosis of the posterolateral wall of the left ventricular (LV) epicardium. Cardiac arrhythmias, including ventricular tachycardia (VT), are common in this condition, particularly when LV ejection fraction (LVEF) declines. CASE SUMMARY: A 45-year-old male with dilated cardiomyopathy due to BMD presented for routine follow-up of his implantable cardioverter defibrillator

Identification and catheter ablation of extracardiac and intracardiac components of ligament of Marshall tissue for treatment of paroxysmal atrial fibrillation.

INTRODUCTION: The ligament of Marshall is a left atrial neuromuscular bundle with sympathetic innervation that may be a source of atrial fibrillation (AF)-inducing automatic activity. METHODS AND RESULTS: Twenty-four patients with paroxysmal AF (including 18 with adrenergic AF) and 25 with other arrhythmias underwent catheter mapping. In cases of adrenergic AF, radiofrequency ablation was attempted when Marshall potentials were recorded. Patients were followed for 2 months before and 11.2 +/- 4.2 months after the procedure. Catheterization of the distal superoposterior coronary sinus was feas

Endomyocardial substrate of ventricular arrhythmias in patients with autoimmune rheumatic diseases.

INTRODUCTION: Delayed enhancement-magnetic resonance imaging (DE-MRI) has demonstrated that nonischemic cardiomyopathy is mainly characterized by intramural or epicardial fibrosis whereas global endomyocardial fibrosis suggests cardiac involvement in autoimmune rheumatic diseases or amyloidosis. Conduction disorders and sudden cardiac death are important manifestations of autoimmune rheumatic diseases with cardiac involvement but the substrates of ventricular arrhythmias in autoimmune rheumatic diseases have not been fully elucidated. METHODS AND RESULTS: 20 patients with autoimmune rheumatic

Cibenzoline transforms random re-entry into ordered re-entry in the atria.

The goal of this study was to test the hypothesis that class I drugs modify the electrophysiological substrate of atrial fibrillation. The e … ibenzoline were studied intra-operatively before undergoing a 'corridor' operation. High resolution epicardial mapping (248 channels) was used to reconstruct the atrial activation patterns before and after administration of the drug. In 18 out of 20 patients administration of cibenzoline resulted in conversion of atrial fibrillation into regular monomorphic atrial t …

Global, segmental and layer specific analysis of myocardial involvement in Duchenne muscular dystrophy by cardiovascular magnetic resonance native T1 mapping.

BACKGROUND: Progressive cardiomyopathy accounts for almost all mortality among Duchenne muscular dystrophy (DMD) patients.‍ Thus, our aim was to comprehensively characterize myocardial involvement by investigating the heterogeneity of native T1 mapping in DMD patients using global and regional (including segmental and layer-specific) analysis across a large cohort. METHODS: We prospectively enrolled 99 DMD patients (8.8 ± 2.5 years) and 25 matched male healthy controls (9.5 ± 2.5 years). All subjects underwent cardiovascular magnetic resonance (CMR) with cine, T1 mapping and late gadolinium e

First clinical experience using a novel automated mapping algorithm for mapping of ventricular arrhythmias.

A new automated vector-based mapping algorithm (AMA) for 3-dimensional (3D) mapping has been introduced. The aim of this study was to present our experience using AMA to recognize additional catheter ablation targets in patients with ventricular arrhythmias (VA). A total of 16 patients (ICM; ischemic cardiomyopathy, n = 6; NICM; non-ischemic cardiomyopathy n = 10) suffering from VA underwent catheter ablation. Following bipolar voltage mapping, AMA was utilized to reveal zones of decelerated conduction velocity vectors (CVV) and this information was superimposed onto the 3D reconstructions and

Ultra-high-frequency ECG volumetric and negative derivative epicardial ventricular electrical activation pattern.

From precordial ECG leads, the conventional determination of the negative derivative of the QRS complex (ND-ECG) assesses epicardial activation. Recently we showed that ultra-high-frequency electrocardiography (UHF-ECG) determines the activation of a larger volume of the ventricular wall. We aimed to combine these two methods to investigate the potential of volumetric and epicardial ventricular activation assessment and thereby determine the transmural activation sequence. We retrospectively analyzed 390 ECG records divided into three groups-healthy subjects with normal ECG, left bundle branch

Premature atrial contractions promote local directional heterogeneities in conduction velocity vectors.

AIMS: Loss of cell-to-cell communication results in local conduction disorders and directional heterogeneity (LDH) in conduction velocity (CV) vectors, which may be unmasked by premature atrial contractions (PACs). We quantified LDH and examined differences between sinus rhythm (SR) and spontaneous PACs in patients with and without atrial fibrillation (AF). METHODS AND RESULTS: Intra-operative epicardial mapping of the right and left atrium (RA, LA), Bachmann's bundle (BB) and pulmonary vein area (PVA) was performed in 228 patients (54 with AF). Conduction velocity vectors were computed at ea…

Characterization of pre-existing arrhythmogenic substrate associated with de novo early and late postoperative atrial fibrillation.

BACKGROUND: PoAF is the most common complication after cardiac surgery and may occur in patients with pre-existing arrhythmogenic substrate. Characterization of this substrate could aid in identifying patients at risk for PoAF. We therefore compared intra-atrial conduction parameters and electrogram morphology between patients without and with early- (≤5 days after surgery) and late- (up to 5 years) postoperative atrial fibrillation (PoAF). METHODS AND RESULTS: Epicardial mapping of the right and left atrium and Bachmann's Bundle (BB) was performed during sinus rhythm (SR) in 263 patients (20…

Trans-myocardial bipolar electrogram: A strategy for mapping and determining efficacy of bipolar ablation of deep foci.

Mapping and ablation of intramural ventricular tachycardia (VT) remain a challenge. We developed a trans-myocardial electrogram recording across distal tips of two separate ablation catheters placed on contralateral sides of the myocardium to record a trans-myocardial bipole and a novel pacing electrode configuration. This trans-myocardial bipole was applied during bipolar ablation in a patient with septal VT. Local activation in this trans-myocardial bipole was similar to the earliest activation recorded from detailed activation maps from both sides of the septum. Pacing from this trans-myoca

Epicardial Ablation of Ventricular Tachycardia in Ischemic Cardiomyopathy.

Catheter ablation can effectively reduce the frequency of ventricular tachycardia in ischemic cardiomyopathy by ablating sites of reentry within complex regions of myocardial scar. In cases of near transmural infarction, this arrhythmia substrate may be nearer the epicardium than the endocardium, and epicardial ablation may be necessary. An epicardial substrate location can potentially be predicted by imaging that suggests transmural infarction. Percutaneous epicardial ablation improves outcomes in selected patients, but is higher risk and avoided in patients with prior coronary artery bypass

Dynamics of Focal Fibrillation Waves during Persistent Atrial Fibrillation.

The incidence and appearance of focal fibrillation waves on the right and left atrial epicardial surface were visualized during 10 seconds of persistent atrial fibrillation in a 71-year-old woman with valvular heart disease. The frequent, nonrepetitive, widespread, and capricious distribution of focal waves suggests that transmural conduction of fibrillation waves is most likely the mechanism underlying focal fibrillation waves.
