Better perineal outcomes in sitting birthing position cannot be explained by changing from upright to supine position for performing an episiotomy.

BACKGROUND: women who give birth in supine position are more likely to have an episiotomy than women who give birth in sitting position. A confounding effect may be that women in upright positions in second stage of labour are asked to lie down if a professional needs to perform an episiotomy. This prospective cohort study aimed to determine whether this factor can explain the lower rate of episiotomy in sitting compared to supine position. METHODS: data from 1196 women who had a spontaneous, vaginal birth were analysed. Positions during second stage and at birth were carefully recorded. Thre…

Management of the perineum during forceps delivery. Association of episiotomy with the frequency and severity of perineal trauma in women undergoing forceps delivery.

OBJECTIVE: To examine the association of the frequency and severity of perineal trauma with episiotomy performed at forceps delivery. STUDY DESIGN: This retrospective study analyzed all forceps deliveries at the Semmelweis Women's Hospital Vienna between February 1999 and July 1999. Evaluation of a possible association of episiotomy with the frequency and severity of perineal trauma was the main objective of the study. Episiotomy was not performed routinely and was either midline or mediolateral. RESULTS: In conjunction with forceps delivery episiotomy, 76/87 women (87%) underwent forceps de…

Transfer of Restrictive Episiotomy Practice with Low OASIS Rate from Tertiary to Primary Care Level-Lessons Learned and Future Challenges.

Introduction: For more than 40 years, obstetricians and midwives worldwide have continued to debate the significance of episiotomy and methods of reducing the rate of obstetric anal sphincter injuries (OASIS). Methods: A concise review of relevant recently published articles on obstetric perineal trauma was conducted, together with an analysis and comparison of the annual rates of episiotomy and OASIS (primary outcomes) at Clinical Hospital "Sveti Duh", Zagreb (tertiary care level), and Požega General County Hospital, Požega (primary care level), over a ten-year period (2015-2024). Results: Ro…

The MOVE-trial: Monocryl® vs. Vicryl Rapide™ for skin repair in mediolateral episiotomies: a randomized controlled trial.

BACKGROUND: Previous studies have shown that complaints after episiotomy repair depend on the method and material used for repair. The objective of our study was to determine which of two frequently used suture materials, Monocryl® (poliglecaprone 25) and Vicryl Rapide™ (polyglactin 910), is superior for intracutaneous closure of the skin in mediolateral episiotomies. METHODS: In a randomized controlled trial performed in a teaching hospital in the Netherlands between 2010 and 2013 250 primiparous women with uncomplicated mediolateral episiotomies were randomly allocated to intracutaneous ski…

Hysteroscopy and episiotomy in a rescued, cold-stressed Florida manatee (Trichechus manatus latirostris) for diagnosis and treatment of a retained fetal skeleton.

A rescued female manatee was observed expelling a fetal bone from the vulva. The manatee was anesthetized and diagnosed with uterine retention of a fetal skeleton by ultrasound and hysteroscopy. Episiotomy was performed to gain manual access to the vagina and uterus for removal of the skeleton. Second intention healing of the episiotomy site produced excellent results. Rescued female manatees should receive a thorough reproductive tract evaluation since presence of retained fetal tissues might not be evident in blood or hormone analyses. Retention of a whole or partial dead fetus can be life-t…

The Association Between Episiotomy and Obstetrical Anal Sphincter Injuries Among Vacuum-Assisted Vaginal Deliveries: A Population-Based Retrospective Canadian Cohort Study.

INTRODUCTION AND HYPOTHESIS: Vacuum-assisted vaginal deliveries (VAVD) represent a growing proportion of operative vaginal deliveries. Mediolateral episiotomy in patients undergoing VAVD has been shown to reduce obstetrical anal sphincter injuries (OASIS) in some settings; however, results of Canadian studies have been conflicted, and the practice has not been widely adopted by Canadian providers. The objective of this study was to estimate the association between episiotomy and OASIS among patients undergoing VAVD in a Canadian cohort. METHODS: A population-based, retrospective cohort of pat…

The Effects of Transcutaneous Electrical Nerve Stimulation on Post-Episiotomy Pain Severity in Primiparous Women: A Randomized, Controlled, Placebo Clinical Trial.

BACKGROUND: Episiotomy or cutting the perineum is the most common operation in obstetrics. Perineal pain is the main complication of episiotomy that affects the quality of life and mental health of the mother. Reducing the pain and side effects of the chemical drugs prescribed for pain relief has attracted the attention of both physicians and scientists. This study was conducted to determine the effects of transcutaneous electrical nerve stimulation (TENS), as an alternative method, on post-episiotomy pain severity. MATERIALS AND METHODS: This randomized clinical trial was performed on 120 pr…

Episiotomy is associated with a reduced risk of obstetric anal sphincter injury in nulliparous vacuum-assisted deliveries, particularly in high-risk cases.

INTRODUCTION: Vacuum-assisted delivery is a major risk factor for obstetric anal sphincter injury (OASI), yet agreement is lacking on whether mediolateral episiotomy mitigates risk. The objective of this study was to evaluate whether episiotomy during vacuum-assisted delivery in nulliparous women affects the risk of OASI and to identify subgroups who may benefit from it. MATERIAL AND METHODS: A retrospective cohort study including all nulliparous singleton pregnancies delivered by vacuum-assisted delivery at a tertiary center (January 2011-December 2022). Twin deliveries and multiparous women…

Evaluation of a policy of restrictive episiotomy on the incidence of perineal tears among women with spontaneous vaginal delivery: A ten-year retrospective study.

INTRODUCTION: Routine episiotomy is no longer recommended to limit obstetrical anal sphincter injuries (OASIs). We aimed to evaluate the effect of a restrictive policy of episiotomy on the risk of OASIs during spontaneous vaginal deliveries. MATERIAL AND METHODS: We performed a retrospective single-center observational study among women with a term singleton cephalic fetus, with spontaneous vaginal delivery. The occurrence of episiotomy, intact perineum, first, second, third or fourth-degree (OASIs) perineal tears were compared before (period A, from 01/01/2006 to 12/31/2008) and after (perio…

A mixed methods study to explore women and clinicians' response to pain associated with suturing second degree perineal tears and episiotomies [PRAISE].

BACKGROUND: perineal suturing is a common event which affects women across a variety of international settings. Women have expressed pain related to perineal trauma in the postnatal period but little is known about pain during suturing. In addition, there is a lack of evidence to identify how professional decisions are made about pain management during the suturing process. OBJECTIVE: to explore women and clinicians' response to pain during the suturing of second degree tears and episiotomies. DESIGN: mixed method feasibility study which included observation, questionnaires and interviews.

Selective episiotomy guided by a perineal evaluation form for forceps-assisted vaginal delivery: a retrospective cohort study on maternal and neonatal outcomes.

BACKGROUND: Forceps-assisted vaginal delivery is prone to perineal laceration and other complications. Episiotomy decisions are often experience-dependent, leading to excessive intervention. The Perineal Evaluation Form (PEF) provides an objective basis for episiotomy, but its clinical efficacy in forceps-assisted vaginal delivery has not been fully validated. METHODS: A single-center retrospective cohort study was conducted on 313 parturients who underwent forceps-assisted vaginal delivery at the Third Affiliated Hospital of Zhengzhou University from January 2017 to December 2024, divided int…

Episiotomy and perineal trauma during childbirth in primiparous women: associations with anxiety, quality of life, vaginal and sexual symptoms in the first year postpartum.

INTRODUCTION: Childbirth-related pelvic floor trauma is prevalent among primiparous women and can lead to significant physical and psychological consequences. While the impact of pelvic floor trauma on physical outcomes has been studied, the relationship between anxiety caused by such trauma and long-term patient-reported outcomes (PROs) such as vaginal symptoms, sexual function, and quality of life (QoL) remains underexplored. This study aims to fill this gap by investigating the association between anxiety induced by pelvic floor trauma during childbirth and these key PROs. METHODS: This pr
