This article presents two cases of actinomycosis. Case 1 was a 39-year-old man who was first seen with the chief complaint of swelling around the left submandibular region. Case 2 was a 40-year-old woman who was first seen with the chief complaint of mass formation around the left buccal area. Both cases were initially diagnosed as malignant tumors and were later histopathologically interpreted as actinomycosis because of the presence of sulfur granules.

A man with hilar adenopathy had smooth firm purplish papules (lupus pernio) on his face and pseudofolliculitis barbae (PFB) in his beard area. Biopsies of papules of both the lupus pernio and the PFB revealed non-caseating granulomas consistent with sarcoidosis. Actinomycotic infection involving the head and neck is not commonly diagnosed and is often a diagnostic enigma to the otolaryngologist. An 11 year old boy with medullary aplasia developed extensive mycosis of the face. This mucormycosis is a rare affection that appears in patients with predisposing disease (diabetes, blood disorders) and is easily diagnosed, if considered, from clinical features. Diagnosis is confirmed by histopathology, culture sometimes identifying the causative fungus. Despite the intrinsic serious course of this affection (facial necrosis, death) prognosis appears to be related essentially to the rapid control of the subjacent disease.

The infestation of a 3-month-old child with Sarcoptes scabiei by way of infested nipples during the breast-feeding period led initially to facial papules and later to generalized dermatitis. Scabies mites were not detected on the face; studies on ten infested babies in India supported this findings. Thus, direct antiscabies treatment to the face is not necessary in babies.

Sporotrichosis is usually considered a fungal infection affecting adults, but it can also affect children. A case of sporotrichosis in an 18-month-old boy is presented which is unusual in that the patient was extremely young and had a relatively atypical clinical presentation. His condition responded rapidly to potassium iodide therapy at a very low dosage. A brief review of childhood sporotrichosis in the American literature is included.

A 36-year-old healthy man with atypical necrobiosis lipoidica of the face is described. The lesions were annular with a raised erythematous palpable border. The centres were slightly depigmented and atrophic, without telangiectasia or hair loss. Histopathological changes revealed prominent giant cells in small groups without clear granuloma formation. They were located at all levels of the drmis between the collagen bundles.

A 73-year-old woman from Guyana had nodular keloidal lesions on her face which had persisted for 56 years. A biopsy of the lesion revealed lobomycosis, a rare fungal infection. Microscopically, numerous fungi were seen in giant cells and macrophages. The macrophages had abundant pink reticulated cytoplasm. These macrophages in lobomycosis, considered by some in the past to be granular myoblastoma cells, were rich in glycoproteins and appeared to be "Gaucher-like cells" in electronmicrographs. We observed a newborn infant of a previously reported kindred with absent dermal ridge pattern, syndactyly, and facial milia. The infant's features were consistent with three other kindreds, suggesting that this entity is a single disorder with variable expression. Furthermore, this entity should be considered in the differential diagnosis of excessive congenital facial milia and erosions.
