Suppression of neurokinin-1 receptor in trigeminal ganglia attenuates central sensitization following inflammation.

This study examined whether local application of a neurokinin-1 (NK1) receptor antagonist into the trigeminal ganglia (TRGs) modulates hyperexcitability of trigeminal spinal nucleus caudalis (SpVc) wide-dynamic range (WDR) neuron activity innervating both the temporomandibular joint (TMJ) region and facial skin following TMJ inflammation. Extracellular single unit recording combined with multibarrel electrodes was used. TMJ inflammation was induced by the injection of complete Freund's adjuvant (CFA). WDR neurons responding to electrical stimuli of the TMJ region and facial skin were recorded

Successful treatment of hemifacial spasm with selective facial nerve block using doxorubicin (adriamycin) under local anesthesia.

OBJECTIVE: To describe our experience with selective chemical rhizotomy of facial nerves using adriamycin (ADM) in a patient with hemifacial spasm (HFS). This unique technique is less invasive than intracranial neurosurgery and enables one to perform a permanent nerve block under local anesthesia. PATIENT: The patient, a 73-year-old female, had difficulty opening her left eye. Following unsuccessful treatment with anti-epileptic medicine, she received selective intraneural injections of ADM under local anesthesia. One week after the surgery the spasms had disappeared completely. No major comp

[Acoustic Neuroma:Surgical Strategy Using Intraoperative Monitoring and Trend Graphs from the Neurosurgery Perspective].

This study assessed the efficacy of combining preoperative three-dimensional (3D) fusion imaging with continuous intraoperative neurophysiological monitoring to enhance the anatomical and functional visualization of cochlear and facial nerves during acoustic neuroma surgery. Data from 282 consecutive patients (2006-2023) were analyzed. In all cases, dorsal cochlear nucleus action potential (DNAP) and facial nerve root evoked electromyography (FREMAP) were continuously monitored. A novel system was implemented for real-time waveform visualization, tracking amplitude trends, and displaying nerve

Chorda tympani nerve analysis with electron microscopy in chronic suppurative otitis media.

CONCLUSION: Chronic suppurative otitis media causes some disturbance to the chorda tympani nerve (CTN), which may affect the facial nerve. It is not possible to perform a biopsy of the main truncus of the facial nerve, therefore studies of the CTN might show possible pathologic or physiologic changes of the facial nerve in the future. OBJECTIVES: The specific aim of this study was to investigate the effects of chronic suppurative otitis media on the CTN. METHODS: The tympanic segments of CTNs were collected for ultrastructural investigations in patients with chronic suppurative otitis media

Gene expression dynamics of human and mouse craniofacial development at the single-cell level.

Craniofacial development is a complex process that involves the specification of diverse and transient cell types. However, our understanding of these processes and the cell-types present during human craniofacial developmental remains limited. We address this gap in knowledge through single-nucleus RNA sequencing of human craniofacial development spanning 4 to 8 post-conception weeks. This resource identifies multiple subtypes of mesenchyme, epithelium, and cranial neural crest, among other functionally distinct cell types. Extensive comparisons to single-nucleus gene expression and spatial t

Hypercondylia: problems in diagnosis and therapeutic indications.

Condylar hyperplasia is a rare disease that alters the anatomy of one of the two condylus of the mandible as a consequence of the abnormal growth of the interested condylus and results in facial asymmetry and functional problems at puberty. Both sexes are affected. Two cases were analyzed, including an active and an inactive nucleus. Histopathologically, two types are distinguished: active and inactive hypercondylia. Diagnosis is usually achieved through clinical and radiographic approach and is utterly important to distinguish the consequent therapeutic approach. Thanks to the development of

Trigeminal autonomic pathways involved in nociception-induced reflex cardiovascular responses.

Reflex cardiovascular responses elicited by noxious oro-facial stimulation are well known but the neural pathways that underlie trigeminal cardiovascular reflex reactions remain to be elucidated. In previous studies, we have shown that noxious electrical stimulation of the mandibular incisor in the anesthetized rat elicits increases in mean arterial blood pressure and heart rate (Allen, G.V., Barbrick, B. and Esser, M.J., Trigeminal parabrachial connections: possible pathway for nociception-induced cardiovascular reflex responses, Brain Res., 715 (1996) 125-135). In this study, microinjections

A histological morphometric study of nuclear size in benign and malignant neoplasms of the human cheek.

This report describes the application of simple morphometric methods to generate quantitative data on nuclear size from tissue sections of normal, benign and malignant oral epithelium of the cheek. Measurements of nuclear areas of basal and spinous cells from cheek mucosa were made on haematoxylin and eosin-stained sections using a Zeiss VIDAS image analyser. The lesions examined comprised benign squamous cell papillomas and islands of cells from invasive squamous cell carcinomas. Normal control epithelium was obtained from the biopsy margins of non-neoplastic lesions. The nuclear areas (AN) w

[Partial vertical nucleotomy as a modification of tractotomy of the trigeminal nerve].

The authors report on partial vertical nucleotomy, a modification of tractotomy of the quintothalamic tract. In 1979 eight patients underwent this procedure. Three of them showed a recurrence of pain from genuine trigeminal neuralgia. Three patients had symptomatic trigeminal neuralgia, one patient atypical neuralgia of the face without organic cause, whereas in one case the neuralgia occurred subsequent to a herpes zoster. At the time of examination all patients were free from pain.

Rapidly progressive fatal respiratory failure (Ondine's curse) in the lateral medullary syndrome.

A 70-year-old man presented with unilateral lateral medullary infarction, and then died of rapidly progressive respiratory failure within a day. The clinical manifestations were hiccups, hoarseness, dysarthria, nystagmus, left central facial paralysis, paralysis of the left soft palate, dysphagia, decreased superficial sensation over the right face and upper limb, and cerebellar ataxia in the left upper and lower limbs. The arterial blood gas analysis revealed mild hypoventilation. Soon thereafter, an apneic episode occurred during a sleep and advanced to ataxic respiration, and the patient di

[Strategy for the prevention and treatment of post-paralytic facial syncinesis].

The existence of facial heimspasm and post-paralytic syncinesia is in general interpreted as the result of aberrant reinnervation following a Bell's palsy. In a certain number of cases, electrophysiological tests reveal synaptic abnormalities in the facial nucleus. These findings must be taken into consideration, together with the possibility of ephapsic stimulation of the proximal part of the facial nerve, when explaining the regression of syncinesia which is found in certain patients during the reinnervation phase. Prevension is based principally upon the quality of treatment and the rapidit…

Microvascular Decompression for Trigeminal Neuralgia Caused by Vascular Compression on the Trigeminal Sensory Nucleus and Descending Trigeminal Tract.

BACKGROUND: Trigeminal neuralgia (TN) is characterized by paroxysmal episodes of severe shocklike orofacial pain typically resulting from arterial compression on the trigeminal root entry zone. However, neurovascular conflict in more proximal parts of the trigeminal pathway within the pons is extremely rare. METHODS: The authors present a case of microvascular decompression for TN caused by dual arterial compression on the dorsolateral pons, along with a brief literature review. RESULTS: Our patient was a 74-year-old man with episodic left-sided facial stabbing pain. Brain magnetic resonance

Clinical spectrum of Kufor-Rakeb syndrome in the Chilean kindred with ATP13A2 mutations.

We report the clinical features of the original Chilean family with Kufor-Rakeb syndrome (KRS) that led to the discovery of the ATP13A2 gene at the PARK9 locus. KRS is a rare juvenile-onset autosomal recessive disease characterized by progressive Parkinsonism, pyramidal signs, and cognitive decline in addition to vertical gaze palsy and facial-faucial-finger minimyoclonus. Neurological and neuropsychological examination during a 10-year period, videotaping, neuroimaging, and measurement of DNA methylation of the ATP13A2 promoter region were performed. The youngest 5 of 17 children of nonconsan

Trigeminal Neuralgia Resulting from Delayed Cervical Cord Compression after Acute Traumatic Fracture of Odontoid Process.

Trigeminal neuralgia is caused by compression of trigeminal nerve root and it leads to demyelination gradually. It was almost idiopathic and occurred unexpected. The upper cervical spinal cord contains the spinal trigeminal tract and nucleus. Fibers with cell bodies in the trigeminal ganglion enter in the upper pons and descend caudally to C2 level. We experienced a rare patient with facial pain, which was paroxysmal attack with severe pain after a clear event, cervical spinal injury (C2). So, this case reminds us of a possible cause of trigeminal neuralgia after a trauma of the head and neck.

Eight-and-a-half syndrome: a rare presentation of pontine infarction.

"Eight-and-a-half" syndrome is "one-and-a-half" syndrome (conjugated horizontal gaze palsy and internuclear ophthalmoplegia) plus ipsilateral fascicular cranial nerve seventh palsy. This rare condition, particularly when isolated, is caused by circumscribed lesions of the pontine tegmentum involving the abducens nucleus, the ipsilateral medial longitudinal fasciculus, and the adjacent facial colliculus. Its recognition is therefore of considerable diagnostic value. We report a case of a 65-year-old man who presented with eight-and-a-half syndrome in which brain magnetic resonance imaging scan

Inhibition of posttraumatic microglial proliferation in a genetic model of macrophage colony-stimulating factor deficiency in the mouse.

Activation and proliferation of microglia are common cellular hallmarks in many different pathological processes of the central nervous system. Although a number of colony-stimulating factors enhance microglial proliferation in vitro, little is known about the endogenous mitogens. In the present study we show a strong and selective inhibition of microglial proliferation in the facial nucleus of osteopetrotic (op/op) mice, with a genetic deficiency in biologically active macrophage colony-stimulating factor (MCSF). Posttraumatic activation of adjacent axotomized neurons and reactive astrocytes …

Hitting a Wall: An Ambiguous Case of Wallenberg Syndrome.

Wallenberg syndrome is the most common stroke of the posterior circulation. Diagnosis of Wallenberg syndrome is often overlooked as initial MRI may show no visible lesion. We present an atypical case of Wallenberg syndrome in which the initial MRI of the brain was normal. Our patient is a 65-year-old male who was brought in by emergency medical services complaining of right-sided facial droop, slurred speech, and left-sided weakness for one day. Physical examination showed decreased left arm and leg strength compared to the right side, decreased left facial temperature sensations, decreased le

Phox2b-expressing retrotrapezoid neurons and the integration of central and peripheral chemosensory control of breathing in conscious rats.

Chemoreception is the classic mechanism by which the brain regulates breathing in response to changes in tissue CO2/H(+). A brainstem region called the retrotrapezoid nucleus (RTN) contains a population of Phox2b-expressing glutamatergic neurons that appear to function as important chemoreceptors. In the present study, we ask whether the destruction of a type of pH-sensitive interneuron that expresses the transcription factor Phox2b and is non-catecholaminergic (Phox2b(+)TH(-)) could affect breathing in conscious adult rats. The injection of substance P (1 nmol in a volume of 50 nl) into the R

Physiological identification of jaw-movement-related neurons in the trigeminal nucleus of cats.

Although neurons responsive to jaw movements have been identified in most parts of the trigeminal brainstem nuclei, little is known about how this information is relayed to the thalamus and ultimately to the cortex for kinesthetic functions and sensorimotor integration. The present extracellular recording experiments showed that a substantial amount of movement-related information is relayed to the thalamus through the caudal part of subnucleus interpolaris (Vi) in adult cats. Vertical jaw displacements, natural mechanical stimuli, and electrical stimulation of the masseter nerve were used to
