We describe a case of a 4-year-old girl with Burkitt's lymphoma, who suffered from a massive gastrointestinal hemorrhage 3 days after chemotherapy. In spite of applying the common practice in correction of coagulopathy, thrombocytopenia persisted and bleeding became life-threatening. In the present case report, we report a successful control of bleeding with a single-dose administration of a biosimilar recombinant activated human factor VII (AryoSeven).

Factor VII is a new coagulation factor replacement therapy. It has permitted the practice of invasive procedures which were up until recently associated with a huge risk of bleeding in patients with hemophilia with inhibitors. Spontaneous haemothorax is a rare and often lethal complication of neurofibromatosis. Two kinds of vascular involvement are described: 1) stenotic or aneurysmatic modifications of large intrathoracic vessels, mainly treated with endovascular techniques; 2) dysplastic alterations in highly vascularized mesenchymal tumors. We describe a rare case of massive intrathoracic haemorrhage due to an upper mediastinic neurofibroma, successfully controlled with recombinant activated factor VII associated with surgical treatment.

Factor VII (FVII) deficiency is the most common among the rare bleeding disorders, which is caused by mutations in coagulation factor VII. Clinical features caused by FVII deficiency vary from mild or asymptomatic to fatal cerebral hemorrhage.
