Acute compartment syndrome in tibial plateau fractures--beware!

Acute compartment syndrome can complicate fractures of the tibial plateau and must be recognized early so that timely fasciotomy can be performed. The presence of the open fasciotomy wound complicates management of the underlying proximal tibial fracture, which ideally needs early anatomic reconstruction of the articular surface and stable metaphyseal fixation. The purpose of this review is to highlight the importance of this problem, understand treatment strategies, and review outcomes.

Early Detection of Compartment Syndrome With Minimal Symptoms: A Case Report on Continuous Pressure Monitoring.

Compartment syndrome (CS) arises from various etiologies but is most commonly associated with severe traumatic injuries. It is a difficult d … s while ensuring diagnosis in a timely fashion of true positives. The treatment for CS is immediate fasciotomy, but fasciotomy is not a complication-free procedure. Physicians need to be sure of the diagnosis both in order not to have the devastating consequence of a missed case but also not to perform with prophylactic fasciotomies that add to patient compl …

[Traumatic rhabdomyolysis. Physiopathology and treatment].

Crush syndrome or traumatic rhabdomyolysis constitutes the systemic changes seen after crush injury, i.e. the damages seen after a prolonged … l failure develops, haemodialysis is started. The crush injuries are treated conservatively without fasciotomy, despite high or increasing intracompartmental pressure. The only indications for fasciotomy are lack of a distal pulse or open lesions. If fasciotomy is performed, radical removal of all necrotic muscle is essential.

Acute compartment syndrome of the thigh in combat casualties.

Acute compartment syndrome of the thigh is a rare clinical entity often caused by high-energy trauma and presenting with a spectrum of associated injuries. Service members in combat are at risk for these causative mechanisms. This study presents a large cohort of thigh compartment syndrome combat casualties and investigates the injury mechanisms, associated mortality, and complications related to fasciotomies. Blasts were the most frequent injury mechanism, overall mortality was 23%, burns were associated with a higher mortality, and fasciotomy morbidity was reported by all respondents. The mo…

Compartment syndrome of upper limb following snake bite, a case report.

INTRODUCTION: Snakebites, recognized as a neglected tropical disease by the WHO, cause significant morbidity and mortality globally. Although antivenom is the primary treatment, managing complications like compartment syndrome (CS) and soft tissue necrosis remains challenging. This case report describes a 39-year-old woman who developed CS following a green pit viper bite and subsequent antivenom administration, necessitating a fasciotomy. CASE: The patient sustained a bite to her right middle finger, receiving 10 vials of polyvalent anti-snake venom. The next day, she developed symptoms sugg…

Treatment of bilateral Posterior Tibial Compartment Syndrome using a Novel Surgical Approach: A Case Report of the Posterior Approach.

Osteofascial compartment syndrome is a serious surgical emergency that requires prompt diagnosis and treatment. It presents a challenge for surgeons due to its high disability rate and difficult management. Early fasciotomy decompression is crucial in preventing severe complications. Classic fasciotomy approaches for tibial osteofascial compartment syndrome include double-incision and single-incision techniques. This paper presents a case of a 24-year-old female with bilateral tibial posterior compartment syndrome resulting from prolonged squatting after alcohol intoxication, which is a relativ…

[Magnetic resonance tomography evaluation of the lower limb after compartment syndrome].

For the purpose of follow-up of the disease as well as the availability of magnetic resonance imaging (MRI) as a method for diagnosing soft tissue changes 20 patients with imminent and manifest compartment syndrome were examined for their fluid content of the lower limb. Considering the normal side as well as 10 healthy volunteers a significant reduction of fluid content was diagnosed in the manifest compartment syndrome after fasciotomy in the late phase of disease (r = 0.49, p > 0.005, n = 29). This was interpreted as a sign of fibrosis. The patients with imminent compartment syndrome and fa…

Accessory soleus muscle. A report of 4 cases and review of literature.

Cadaveric studies have demonstrated the incidence of an accessory soleus muscle ranges from 0.7% to 5.5%. The differential diagnosis of a painful soft tissue mass in the posteromedial region of the ankle includes ganglion, lipoma, hemangioma, synovioma, and sarcomas. In light of these possibilities, most of the early reports of accessory soleus included evaluation and treatment with biopsy, fasciotomy, or excision. Four patients, ranging in age from 14 to 66 years of age at the time of presentation, are discussed. Reports in the literature indicate that accessory soleus is a benign condition, …

Acute Compartment Syndrome: Do guidelines for diagnosis and management make a difference?

BACKGROUND: The best outcomes following Acute Compartment Syndrome (ACS) are attributed to early diagnosis and treatment. National guidelines were issued in the United Kingdom in 2014 (BOAST 10) to standardise and improve management. We analysed standards of diagnosis and management before and after the introduction of the guidelines. METHODS: We retrospectively reviewed the data of all patients with ACS requiring fasciotomy between March 2010 and May 2015 across four Major Trauma Centres (MTCs) in the Northwest of England. We analysed the pooled data for variations between the centres and th…

[Rupture of a Baker cyst with compartment syndrome. A rare complication of knee arthroscopy].

We report a case of leakage of irrigation fluid into the extra-articular soft tissue, due to rupture of a Baker's cyst during arthroscopy of the knee joint. A four-quadrant fasciotomy of the calf musculature was performed to prevent the devasting ischemic complications of an imminent compartment syndrome. A survey of the literature reveals no consensus regarding the treatment of such complications. On the contrary, some authors recommend avoiding fasciotomy and waiting for spontaneous reabsorption of the irrigation fluid.

[Surgical treatment and late results of foot compartment syndrome].

From 1982 to 1988 a total of 29 patients with compartment syndrome of the foot were treated by fasciotomy. The most common causes were fracture dislocations of the Lisfranc (n = 14) and Chopart joints (n = 4). Since these injuries lead to a severe damage to soft tissue structures--joint capsules, ligaments, fasciae--the muscular compartments often communicate and decompression can be achieved by a longitudinal dorsal incision of the skin and fasciotomy of the fascia dorsalis pedis and the retinacula extensorum superior and inferior. Subsequent measurement of intracompartmental pressure dictate…

Percutaneous Needle Decompression: A Novel Approach Toward Treating Compartment Syndrome of the Leg.

Percutaneous needle decompression (PND) can be a successful alternative to open fasciotomies for acute compartment syndrome (ACS). We present the case of a 45-year-old male patient who survived a road traffic accident and developed ACS following his open fracture of the tibia and fibula. He was treated by performing PND on all compartments of the affected leg using a 24 gauge needle thus avoiding the complications of a double incision fasciotomy.

Testicular compartment syndrome: a new approach to conceptualizing and managing testicular torsion.

OBJECTIVES: Decompression of compartment syndrome is known to salvage tissues in numerous organ systems. To demonstrate evidence that testes exposed to prolonged ischemia exhibit compartment syndrome physiology and propose a novel technique in treating this phenomenon. METHODS: Three boys, aged 11, 14, and 16 years, with prolonged testicular torsion lasting 6-7 hours were taken to the operating room. All testes appeared dusky and congested on manual detorsion. Testicular fasciotomy was performed by making a longitudinal incision in the tunica albuginea. The coloration of all testes improved d…

Upper limb compartment syndrome after a viper bite in a child: A case report.

The occurrence of compartment syndrome after a viper bite is a rare phenomenon and its treatment remains controversial. We report the case of an 11-year-old boy who developed compartment syndrome of the right upper limb following a viper bite. Decompression by fasciotomy was performed urgently with a good outcome. Surgical decompression for compartment syndrome following a viper bite is not a common treatment according to the literature. Fast access to care and antivenom administration seem to be effective at treating the compartment syndrome without recourse to fasciotomy.

Reversed Palmaris Longus Muscle Causing Volar Forearm Pain and Ulnar Nerve Paresthesia.

A case of volar forearm pain associated with ulnar nerve paresthesia caused by a reversed palmaris longus muscle is described. The patient, an otherwise healthy 46-year-old male laborer, presented after a previous unsuccessful forearm fasciotomy for complaints of exercise exacerbated pain affecting the volar forearm associated with paresthesia in the ulnar nerve distribution. A second decompressive fasciotomy was performed revealing an anomalous "reversed" palmaris longus, with the muscle belly located distally. Resection of the anomalous muscle was performed with full relief of pain and senso…

[Acute compartment syndrome following a total knee arthroplasty].

Acute compartment syndrome is a rare, but severe complication to knee arthroplasty. The main symptoms are extreme pain unrelated to the surgery, pulselessness, and tense and pallid skin. The syndrome can be disguised by an effective pain management, but due to the consequences, an early diagnosis and fasciotomy is important. We describe a case with a patient, who had undergone a total knee arthroplasty and after a postoperative period of 5,5 hours she developed extreme pain in the limb, below the operated knee. The diagnosis was confirmed by sonography, and an acute fasciotomy was performed.

Upper limb compartment syndrome after an adder bite: a case report.

Compartment syndrome after an adder bite is extremely rare, whose effects are only secondary to the cytotoxic and hemorrhagic effects of venom. Here we reported a case of compartment syndrome in the upper limb following an adder bite in the thenar eminence. Elevated compartment pressure was documented and immediate surgical fasciotomy was practiced. The patient achieved complete recovery with a good functional result. We discussed the controversies on fasciotomy and non-invasive measures in such a situation, and recommended intracompartmental pressure monitoring during the management of compar…
