Myospherulosis. Fine needle aspiration cytologic findings in 19 cases.

An analysis was made of 19 cases of myospherulosis seen on fine needle aspirates of mammary and subcutaneous tissue masses. Myospherulosis, best seen with the Papanicolaou stain, appeared as 4 microns to 7 microns spherules that were homogeneously smooth or contained one or more internal dense bodies. The spherules were dispersed singly or aggregated into sac-like structures. Myospherulosis accompanied 16 benign and 3 malignant conditions. In two aspirates, myospherulosis was seen simultaneously with breast carcinoma; in another, fat necrosis with myospherulosis masked an underlying breast mal…

Inflammation and infarction of adipose tissue: implications in bariatric surgery.

Fat necrosis occurs more frequently in patients who have obesity and diabetes mellitus and is linked to worsening of diabetes. Little evidence is available about surgical complications that are related to inflammation and necrosis of adipose tissue. We report two cases of young women with diabetes who underwent bariatric surgery and had complications resulting from extensive inflammation and necrosis of adipose tissue. The first patient was diagnosed with omental infarction, which is a type of fat necrosis that is rarely associated with obesity and bariatric surgery. The second patient had an …

The role of skin biopsy in diagnosis of panniculitides.

Several factors hamper the clinical and histologic diagnosis of panniculitis. Clinically the patients tend to present with erythematous subc … ription of the normal histology of subcutaneous fat. I also includes the definition of the types of fat necrosis, role of biopsy of panniculitis and its rules and pitfalls, up to a microscopic approach of a slide.

[3 cases of neonatal cyto-steatonecrosis. Physiopathological discussion].

Three cases of subcutaneous fat necrosis of the newborn (SFNN) are presented. They occurred after a therapeutic coma induced by intravenous barbiturates to control convulsions unresponsive to conventional treatment. Two patients had widespread lesions. Histopathology showed prominent vascular involvement. Since 1970, 26 cases of SFNN have been published. Among the aetiological factors already described, hypothermia and ischaemia play a major role. It is suggested that brown adipose tissue might be of importance in the pathophysiology of subdural fat necrosis of the newborn.

Acute pancreatitis masquerading as testicular torsion.

A 40-year-old man presented with fat necrosis of scrotum as the complication of acute pancreatitis. Excessive fluid accumulation in the pancreas and the extrapancreatic spaces, including around the spermatic cord, was seen on computed tomography. Surgical specimen showed typical fat necrosis of tunica vaginalis and the spermatic cord. After the surgery, pain of the testicle subsided completely, without recurrence. From the clinical presentation alone, it had been difficult to differentiate this patient's condition from torsion of the spermatic cord.

[Pancreatic Panniculitis in Patients with Chronic Pancreatitis: Case Report and Review of Literature].

Pancreatic panniculitis is a rare complication characterized by subcutaneous fat necrosis associated with pancreatic disease. It has been postulated that pancreatic panniculitis is caused by the systemic activity of pancreatic enzymes that lead to microcirculatory disturbances. We report a 41-year-old heavy alcoholic woman with pancreatic panniculitis that coexisted with acute and chronic pancreatitis. She was diagnosed with chronic pancreatitis and alcoholic liver cirrhosis 5 years ago. She presented with multiple, tender, erythematous, subcutaneous nodules with heat sensation on both lower l…

Indocyanine green fluorescence angiography algorithm for determining additional anastomosis in deep inferior epigastric perforator flap for breast reconstruction.

BACKGROUND: Intraoperative indocyanine green (ICG) fluorescence angiography (ICG-FA) is commonly used to detect areas with poor perfusion. Fat necrosis caused by poor perfusion is often associated with the use of deep inferior epigastric perforator (DIEP) flaps. We report the development of an effective ICG-FA algorithm to prevent the occurrence of fat necrosis in the use of DIEP flaps. METHODS: Fifty patients who underwent unilateral breast reconstruction using a DIEP flap and intraoperative ICG-FA were included. ICG-FA was performed after free-flap elevation to determine the extent of blood…

Abdominal subcutaneous fat thickness as a substitute for BMI in predicting complications in abdominally-based autologous breast reconstruction.

INTRODUCTION: Patient selection for autologous tissue transfer for postmastectomy breast reconstruction often utilizes body mass index (BMI) … pient site complications (p = .001). Further analysis revealed ASFT is specifically correlated with fat necrosis (p = .005). In independent multivariable regression models, both BMI (p = .011) and ASFT (p = .001) were significant predictors of fat necrosis. The ASFT model had a BIC of 335.42 compared to the BMI model with a value of 340.89, with smaller number …

Self-induced panniculitis in adolescence: Three cases and review of the literature.

Self-induced trauma is a rare cause of panniculitis in adolescents and may be associated with undiagnosed underlying psychological conditions. The condition often poses a diagnostic challenge as patients usually present with non-specific signs and symptoms, and often a concealed history of self-induced trauma. Here, we present three adolescent patients with self-induced traumatic panniculitis. All three patients were repeatedly admitted with non-specific cutaneous lesions as well as multiple somatic complaints. After extensive evaluation to exclude organic causes, psychiatric assessment eventu

The Effect of Venous Augmentation on Complication Rates in Deep Inferior Epigastric Perforator Breast Reconstruction.

BACKGROUND: With the rising popularity of the deep inferior epigastric perforator (DIEP) flap in breast reconstruction, use of the superficial inferior epigastric vein (SIEV) to augment venous outflow has been proposed as a strategy to prevent venous congestion, a complication positively associated with flap volume. This study evaluated the impact of routine SIEV venous augmentation on the risk of vascular complications or operative fat necrosis in the context of flap size and operating time. METHODS: A retrospective cohort study compared complication rates of patients with SIEV-augmented DIE…

A pediatric case report of Epiploic appendagitis presented with abdominal pain.

INTRODUCTION: Epilopic appendagitis (EA) is an uncommon condition of abdominal pain caused by the local inflammation of the fat-filled peritoneal outpouchings due to torsion or thrombosis of its vessels leads to ischemia and gangrenous necrosis of the aappendages, as it can cause peritoneal irritation, acute ischemia, and fat necrosis. CASE REPORT: We present a case of epilopic appendagitis mimicking appendicitis of a 10 years old male, presented to the emergency department with severe right quadrant pain pointedly at the right lumbar area. Associated with constipation and nausea for once. Co…

Mediastinal necrosis in acute pancreatitis - case report.

Ascites, pseudocyst, necrosis of the retroperitoneal fat tissue and pancreatopleural fistula with left sided pleural effusion may complicate pancreatitis. However, steatonecrosis of the mediastinum and right side pleural effusion are rather rare complications of pancreatitis. We present a case of a patient with alcohol induced pancreatitis. Chest x-ray showed right sided pleural effusion. Although high levels of amylase in pleural fluid made the diagnosis of pancreatopleural fistula most likely, necrosis of the mediastinal fat tissue with right side pleural effusion was found postmortem.

Amniotic membrane to the rescue of partially necrosed dermis fat graft.

A 25-year-old woman underwent dermis fat graft on the right side for persistent ocular pain with her polymethylmethacrylate (PMMA) spherical orbital implant. One week later she developed dermal necrosis of the dermis fat graft with potentially devastating fat atrophy if left uncovered. Amniotic membrane graft epithelial side up was sutured to the conjunctiva surrounding the fat. 2 weeks later the graft epithelialized with preservation of fat. Role of amniotic membrane graft as a routine lining over dermis fat grafts to promote conjunctival healing and prevent fat exposure needs to be studied i
