Spontaneous uterine rupture at 22 weeks' gestation in a multipara with previous hysteroscopic resection of fibroid.

A 34-year-old multiparous woman presented at 22(+6) weeks' gestation with severe abdominal pain which preceded a mechanical fall down the stairs. On admission, fetal movements were felt and fetal heart rate was detected by Doppler ultrasound. Over the course of 12 h, although she maintained her vital signs, repeat laboratory and radiological investigations suggested sinister intra-abdominal pathology. Subsequently, fetal movements and heart rate ceased and the patient began to decompensate. A CT scan confirmed uterine rupture. Subsequent emergency laparotomy revealed a 2 l haemoperitoneum and …

Long bone development in extrinsic fetal akinesia: an experimental study in rat fetuses subjected to oligohydramnios.

The transverse growth of long bones during intrauterine development was studied in rat fetuses subjected to experimental oligohydramnios in order to determine whether the skeletal changes, if any, in extrinsic fetal akinesia were similar to those observed in curarized rat fetuses with the fetal akinesia deformation sequence. Oligohydramnios was induced by daily extraction of amniotic fluid from day 17 of gestation until term. Experimental fetuses were compared with a sham-operated control group. The total area and perimeter, the absolute and relative amount of periosteum and bone trabeculae, t

Prenatal diagnosis of a new syndrome: holoprosencephaly with hypokinesia.

Markedly decreased fetal activity (akinesia/hypokinesia) is usually readily apparent to experienced mothers, and frequently this concern leads to attempts at prenatal diagnosis. We report prenatal diagnosis of two fetuses with congenital contractures, markedly decreased fetal movement, and microcephaly due to severe holoprosencephaly. Such familial recurrence to phenotypically normal parents suggests a newly recognized autosomal recessive or X-linked syndrome that is readily detectable by prenatal ultrasonography.

Umbilical cord length as an index of fetal activity: experimental study and clinical implications.

Umbilical cord length varies considerably and the factors controlling cord length are unknown. Experiments in rat fetuses indicate that (1) restriction of fetal movements by oligohydramnios leads to short cord. The umbilical cords were significantly shorter in proportion to the duration or time of onset of the oligohydramnios. The mean cord length represented 65% of littermate control values when persistent oligohydramnios was induced on day 15, 71% for day 16 and 78% for day 17 (term day 21). (2) Suppression of fetal movements by curarization from day 18 on leads to short cords, irrespective …

The potential of four-dimensional (4D) ultrasonography in the assessment of fetal awareness.

AIM: The aim of the study was to observe different expressions and movements of a fetal face during investigation of fetal behavior in the second and the third trimester of normal pregnancies, as a probable manifestation of fetal awareness. SUBJECTS AND METHODS: Over a 6-month period a study was conducted in three centers in Zagreb, Croatia and in Barcelona and Malaga, Spain. Women with singleton pregnancies (16-33 weeks) who were referred for ultrasound check-up for determination of gestational age, suspicious fetal malformations, polyhydramnios, and/or the assessment of biophysical profile

Fetal brain death syndrome--a case report and literature review.

This case describes a new feature of fetal brain death syndrome, abnormal movements mimicking fetal convulsions being subsequently found to be decerebrate hypertonicity in a brain-dead fetus. It also confirms the diagnostic criteria of fetal brain death, both clinical and ultrasonic. The development of polyhydramnios both prior to and after the presumed neurological event is suggested as an association with the diagnosis of fetal brain death. Increased awareness of this event and the heterogeneity of the presentation may prevent further unnecessary Caesarean sections, as to date only 4 of the

The development of spontaneous body movement in prenatal and perinatal mice.

The qualitative and quantitative changes in behavioral development in the prenatal and perinatal periods have been insufficiently studied. We observed the occurrence of spontaneous body movements in three strains of mice (ICR, C3H, and BALB) and the pulse counts measured by EMG activity gestation period normal for mice. During the prenatal period, fetal movements changed from simple movements to complex and coordinated movements. Differences in the timing of the development of these movements were not found in the three strains of mice. During the perinatal period, the three kinds of simple mo…

Can fetal ultrasound result in prenatal diagnosis of Prader-Willi syndrome?

OBJECTIVE: To define fetal ultrasound characteristics triggering an antenatal diagnosis of Prader Willi syndrome (PWS). METHODS: Retrospective analysis of sonographic characteristics retrieved from obstetric ultrasound records. All children (n=11) had a postnatal genetically confirmed diagnosis of PWS. RESULTS: All patients (n=11) showed at least one aspecific abnormality on prenatal ultrasound. Ten out of eleven (90.9 %) had decreased fetal movements, 7 (63.6%) presented in breech position, 7 (63.6%) had severe intra-uterine growth restriction (<5th centile) and 4 (36.4%) showed a polyhydra…

Absence of Wharton's jelly.

Wharton's jelly is a specialised tissue which surrounds the vasculature within the fetal umbilical cord. We present the case of a 42-year-old woman who gave birth to a female infant via emergency caesarean section. At the time of delivery, absence of Wharton's jelly was noted. This finding was confirmed by histological examination. Emergency caesarean section was necessitated due to a fetal bradycardia, and of note, the patient had presented twice prior to this with reduced fetal movements.

Cardiotocographic and sonographic findings in two cases of antenatally diagnosed intrauterine fetal brain death.

Intrauterine fetal brain death is a rare cause of a fixed fetal heart rate pattern. Seven cases have been previously reported in the literature, but only two of them were diagnosed prenatally and all the newborns died soon after delivery. Two additional cases of antepartum diagnosis of intrauterine fetal brain death, managed expectantly, are reported. We had the unique opportunity to document progressive sonographic cerebral changes during the follow-up period, following the neurological event, while the fetus continued life and growth in utero. The cardiographic and sonographic findings sugge

Surgical resolution of an ectopic pregnancy in a captive gerenuk (Litocranius walleri walleri).

A full-term ectopic pregnancy was diagnosed in a multiparous, captive gerenuk (Litocranius walleri walleri) following prolonged gestation without parturition. Until 213 days of gestation, fetal viability was determined by transabdominal ultrasound of fetal heartbeat, physical palpation, and visual observations of fetal movements. Survey abdominal radiographs obtained at day 229 of gestation revealed a fully mineralized fetal skeleton in an inappropriate position for parturition. Surgical exploration revealed a full-term, nonviable, male calf contained within fetal membranes attached to the ome…

Ultrasound assessment of the fetal biophysical profile: what does an radiologist need to know?

Proposed by Frank Manning about 26 years ago, fetal biophysical profile has been incorporated to the propaedeutics of non-invasive fetal well being assessment in high-risk gestations. Despite the existence of other methods for assessing fetal vitality, as Doppler flowmetry, the biophysical profile continues to be important in estimating the risk of hypoxia and perinatal morbimortality for those fetuses. In the present article, the authors review the regulatory mechanisms of fetal biophysical activities, as well as physiological and pathological factors that interfere with them. The main object

Success rate and challenges of fetal anesthesia for ultrasound guided fetal intervention by maternal opioid and benzodiazepine administration.

OBJECTIVE: The safe and effective methods of fetal anesthesia/analgesia during ultrasound guided direct fetal procedure are yet to be determined. The authors investigated whether maternal diazepam/fentanyl administration meets this purpose. METHODS: The medical/anesthesia records were retrospectively reviewed in cases between 2001 and 2010 at a tertiary perinatal center. Success rate as well as maternal and fetal complications were analysed. RESULTS: Among the 150 procedures in 118 fetuses, diazepam 10 mg and fentanyl 200 µg sufficiently prevented fetal movement upon the procedure in 56% of …

Screening and triage of intrauterine growth restriction (IUGR) in general population and high risk pregnancies: a systematic review with a focus on reduction of IUGR related stillbirths.

BACKGROUND: There is a strong association between stillbirth and fetal growth restriction. Early detection and management of IUGR can lead to reduce related morbidity and mortality. In this paper we have reviewed effectiveness of fetal movement monitoring and Doppler velocimetry for the detection and surveillance of high risk pregnancies and the effect of this on prevention of stillbirths. We have also reviewed effect of maternal body mass index (BMI) screening, symphysial-fundal height measurement and targeted ultrasound in detection and triage of IUGR in the community. METHODS: We systemati…

Fetal meconium peritonitis after maternal hepatitis B: a case report and review of the literature.

Maternal hepatitis virus has rarely been implicated in fetal meconium peritonitis (FMP), and its underlying mechanism is largely unknown. We describe a case of FMP presumably caused by maternal chronic hepatitis B virus (HBV). A 29-year-old primigravid woman was referred to our hospital at 35 weeks of gestation for the disappearance of fetal movements. The maternal prenatal history included HBV for more than 10 years. Her HBV DNA level was suppressed (<20 IU/mL) and she was taking oral tenofovir disoproxil fumarate (300 mg/day). At 21+5 weeks, fetal ascites, echogenic bowel, and intra-abdomina…
