Interferon-gamma contributes to the normalcy of murine pregnancy.

Uterine natural killer (uNK) cells are transient, large, heavily granulated, maternal lymphocytes present on the mesometrial side of the pregnant mouse uterus. These cells contribute to normal implantation site development. Cytokine production, particularly interferon (IFN)-gamma, is a major function of most NK cell subsets. In this study, uNK cells were assessed for IFN-gamma production. Local concentrations of IFN-gamma were measured in the mesometrial regions of murine implantation sites between Days 6 and 16 of gestation. IFN-gamma was detected by ELISA at all days studied in a random-bred

[Maternal brain death during pregnancy].

The case of a pregnant patient who had a massive intracraneal haemorrhage at 18 weeks of gestation is presented. Patient's neurological damage evolved to brain death, but the fetus continued in good condition. The decision of withdrawing life support or to continue supporting the mother's life to allow fetal development aroused difficult ethical questions, both to relatives and professionals. This is an exceptional situation of a heart beating cadaver and a non viable fetus whose life depends on the continuation of treatments that are considered as experimental. A good decision should be based

Prognostic indicators of the resolution of nonimmune hydrops fetalis and survival of the fetus.

To delineate any possible prognostic indicators, we reviewed the ultrasonographic and nonmorphometric findings in 19 antenatally diagnosed cases of nonimmune hydrops fetalis in which it was chosen to continue the pregnancy. Thirteen fetuses died and six survived. Of all parameters examined, including associated anomalies, abnormal karyotype, location of serous fluid, anemia, and possible cause of nonimmune hydrops fetalis, the most sensitive prognostic indicator was the real-time-directed M-mode echocardiographic measurement of the biventricular outer dimension in diastole. All fetuses with bi

Some aspects of late abortion for congenital abnormality.

Present methods of prenatal diagnosis are not applicable until at least 16 weeks of gestation. Most abortions for suspected congenital abnormality are therefore usually done in the second trimester of pregnancy. Abortions at over 20 weeks of gestation account for about 0.8% of all abortions for residents of England and Wales, but the operations are unpleasant and carry a significant morbidity. Screening the population for neural tube defects has led to a marked reduction in the recorded incidence of infants born with the condition. A survey in the Northern Region of England, with official stat

Transabdominal cervicoisthmic cerclage: a reappraisal 25 years after its introduction.

Most cerclage operations for cervical insufficiency are performed transvaginally. The transabdominal route is beneficial in treating patients with cervices that are either extremely short, congenitally deformed, deeply lacerated, or markedly scarred because of previously failed transvaginal cerclage procedures. The average gestational age at surgery was 11.5 weeks and the operation was performed after early ultrasonographic verification of fetal viability. Patients with advanced cervical effacement or dilatation in the second trimester were excluded. A 5 mm wide Mersilene band was applied in a

Spleen-associated macrophage alterations and embryo viability in a murine model of recurrent spontaneous abortion.

BACKGROUND: Recurrent spontaneous abortion (RSA) is commonly attributed to immune dysregulation at the maternal-fetal interface. The contribution of the spleen, as a peripheral immune organ, remains unclear. This study investigated spleen-associated immune alterations in a murine model of RSA. METHODS: An RSA model was established by mating CBA/J females with DBA/2 males. Splenic and decidual immune-cell distributions were assessed by histology, flow cytometry, and cytokine analysis. Splenectomy and macrophage depletion were performed as experimental interventions. Decidual transcriptomic cha

Yellow Fever Vaccination in a Mouse Model Is Associated With Uninterrupted Pregnancies and Viable Neonates Except When Administered at Implantation Period.

The potential risk of yellow fever (YF) infection in unvaccinated pregnant women has aroused serious concerns. In this study, we evaluated the effect of the YF vaccine during gestation using a mouse model, analyzing placental structure, immunolocalization of the virus antigen, and viral activity at the maternal-fetal barrier and in the maternal liver and fetus. The YF vaccine (17DD) was administered subcutaneously at a dose of 2.0 log10 PFU to CD-1 mice on gestational days (gd) 0.5, 5.5, and 11.5 (n = 5-10/group). The control group received sterile saline (n = 5-10/group). Maternal liver, impl

From "can we treat?" to "should we treat?": a narrative review on resuscitation limits at the threshold of viability.

BACKGROUND: Advances in perinatal and neonatal care have progressively lowered the threshold of viability and improved survival among extremely preterm infants (EPIs). However, this increase in survival has not consistently been accompanied by comparable improvements in neurological outcomes, which in a relevant proportion of cases remain poor. This discrepancy generates the ethical dilemma of deciding whether to proceed with active resuscitation at the margins of viability. Historically, decisions have relied heavily on gestational age (GA), yet this single parameter has proven insufficient.

ACOG practice bulletin. Perinatal care at the threshold of viability. Number 38, September 2002. American College of Obstetrics and Gynecology.

The survival rate for extremely preterm or extremely low-birth-weight (LBW) newborns born at the threshold of viability (25 or fewer completed weeks of gestation) improved in the early 1990s, largely as the result of a greater use of assisted ventilation in the delivery room and surfactant therapy. Increased use of antenatal and neonatal corticosteroids also may have influenced survival rates. However, this improvement in survival has not been associated with an equal improvement in morbidity. The incidence of chronic lung disease, sepsis, and poor growth remains high and may even have increas

Antenatal fetal surveillance in pregnancies complicated by fetal gastroschisis.

OBJECTIVE: The purpose of this study was to determine if antenatal fetal surveillance should be considered in pregnancies complicated by fetal gastroschisis, and if so, what gestational age should testing begin. STUDY DESIGN: During an 18-year period, all pregnancies delivered of a newborn that had gastroschisis were identified. Numerous data parameters were collected, including gestational age at delivery, birthweight, indication for delivery, antenatal testing results if performed, and neonatal outcome. Fetal compromise was defined as stillbirth or moderate to severe arterial cord blood gas

RNA Binding Protein PCBP1 Functions in the Endothelial-to-Hematopoietic Transition During Hematopoietic Stem Cell Formation.

Two paralogous RNA-binding proteins, PCBP1 and PCBP2, are individually essential for mouse development. Pcbp2-null embryos lose viability at midgestation, whereas Pcbp1-null embryos have an earlier, peri-implantation lethality. Inactivation of each gene alone in the differentiating erythroid lineage failed to impact embryonic viability, whereas combined Pcbp1/Pcbp2 inactivation resulted in repression of erythroid/hematopoietic gene expression midgestation, decreased blood cell formation, and fetal demise. Here, we assess the impacts of individual and combined conditional inactivation of Pcbp1

Effects of graft placement site on the survival of adrenal medulla transplants into the brain and its relation with the recovery of motor function.

BACKGROUND: Because of their lack of long-term viability, adrenal tissue transplants have shown limited success in alleviating the motor disturbances associated with experimental and pathologic striatal dopamine denervation. In this study, we examined how the graft placement site influences adrenal medulla transplant survival and its relation with the reduction of motor deficits in rats bearing unilateral 6-OHDA lesion. METHODS: One or 5 microL of fetal adrenal medullar tissue was grafted either inside the striatal parenchyma or into the lateral ventricle in contact with the dopamine-denervat

Long-term outcomes and potential mechanisms of offspring exposed to intrauterine hyperglycemia.

Diabetes mellitus during pregnancy, which can be classified into pregestational diabetes and gestational diabetes, has become much more prevalent worldwide. Maternal diabetes fosters an intrauterine abnormal environment for fetus, which not only influences pregnancy outcomes, but also leads to fetal anomaly and development of diseases in later life, such as metabolic and cardiovascular diseases, neuropsychiatric outcomes, reproduction malformation, and immune dysfunction. The underlying mechanisms are comprehensive and ambiguous, which mainly focus on microbiota, inflammation, reactive oxygen

[Intrauterine death in twin pregnancies].

The loss of a fetus in a multiple pregnancy is relatively frequent. The main risk factor is a monochorionic twin pregnancy. If fetal loss occurs after 16 weeks of pregnancy, the survivor is also at increased risk of intrauterine death. In monochorionic twins, the risk of neurological sequelae can be as high as 25%. These lesions are due to acute episodes of hypoperfusion immediately following the death of the co-twin. Maternal complications including coagulation disorders are rare. The management will vary according to the type of placenta, the gestational age and the condition of the survivin

Prenatal diagnosis of congenital cystic adenomatoid malformation of the lung: four cases report.

Congenital cystic adenomatoid malformation of the lung (CCAML) is a rare pulmonary lesion, characterized by excessive overgrowth of the terminal respiratory bronchioles. Prenatal detection and serial sonographic study of fetuses with CCAML can provide information about the natural history of these lesions and reveal most of the nature history of pathophysiologic features which are likely to affect the clinical outcome. This information is crucial to the formulation of a prognosis and a management strategy. We report on four cases of CCAML, three of which involved macrocystic lesions including

Selective embryo reduction in multiple pregnancies resulting from assisted conception.

BACKGROUND: Assisted conception not only raises the pregnancy rate by treating infertility, but also increases the chance of obstetrical complication of multiple pregnancy. So it is important to develop a means by which the number of developing embryos can be reduced. METHODS: Eight multiply pregnant women (from triplets to octuplets) asked to have the number of viable fetuses reduced. By means of sonar guided puncture, a mixture of demerol and xylocaine was injected to the fetal thorax until the fetal heart ceased to beat. The procedures were done between the 7th and 11th week of gestational

[Routine pregnancy ultrasound scans can detect malformations. Multidisciplinary care of both the foetus and parent is required following diagnosed malformations].

Routine pregnancy ultrasound scans can detect malformations. Multidisciplinary care of both the foetus and parent is required following diagnosed malformations In Sweden, all women are invited to at least one ultrasound examination during pregnancy. The »Routine ultrasound screening« is usually performed by a midwife around 18 weeks of gestation. There are several aims of the examination: to check viability, to detect multiple fetuses, to determine the location of the placenta, to check the amount of amniotic fluid, to estimate the date of delivery (if this had not been done earlier) and to de

Oxytocin is neuroprotective against oxygen-glucose deprivation and reoxygenation in immature hippocampal cultures.

Oxytocin triggers an excitatory-to-inhibitory switch in GABA (gamma-aminobutyric acid) actions in immature neurons and this was found to increase their resistance to anoxic episodes. In this study we examined the neuroprotective effect of oxytocin on immature hippocampal cultures subjected to oxygen-glucose deprivation (OGD) both immediately after the insult, as well as after 6h of reoxygenation. We measured metabolic activity fluorometrically using resazurin and found that cellular viability was increased in the oxytocin treated group both immediately after OGD, as well as after 6 h of reoxyg

Influence of the magnesium aspartate hydrochloride administration to the maternal circuit on the aspartate concentration of the fetal circuit under in vitro perfusion of human placenta.

OBJECTIVES: Magnesium aspartate hydrochloride (Magnesiocard, Mg-Asp-HCl) is proposed as a substitute of magnesium sulfate for the treatment of preeclampsia and premature labor. After an i.v. administration of a dose equivalent to that used in the treatment of preeclampsia to nonpregnant volunteers, a 10-fold increase of aspartic acid (Asp) over the physiological level was observed. Animal experiments have demonstrated that highly increased fetal levels of acidic amino acids such as Asp could be associated with neurotoxic damage in the fetal brain. The influence of such an elevation of Asp conc
