Biofeedback baropodometry training evaluation: a study with children with equinus foot deformity.

The lack of perception in the hindfoot increases the plantar flexion, causing irregular posture due to the foot position, a disability known as equinus foot deformity. A portable device, named baropodometer, that measures the pressure at the forefoot and hindfoot regions was built to help this population in terms of balance and posture correction. Ten hemiparetic teenager volunteers with equinus foot participated in the experiments. The results demonstrated that the proposed device increased the weight-bearing in upright stance in the paretic side, decreasing the weight in the non-paretic side

Foot deformity in charcot marie tooth disease according to disease severity.

OBJECTIVE: To investigate the characteristics of foot deformities in patients with Charcot-Marie-Tooth (CMT) disease compared with normal persons according to severity of disease. METHOD: Sixty-two patients with CMT disease were recruited for this study. The normal control group was composed of 28 healthy people without any foot deformity. Patients were classified into a mild group and a moderate group according to the CMT neuropathy score. Ten typical radiological angles representing foot deformities such as pes equinus and pes varus were measured. The CMT group angles were compared with tho

The pathogenesis and surgical management of foot deformity in Charcot-Marie-Tooth disease.

The foot deformities in CMT follow certain general patterns; however, like the underlying motor failures that cause them, the deformities present in each patient are unique, and care must be individualized. There is no simple algorithm that can be applied to all patients. The hindfoot, forefoot, and toe deformities in CMT all ultimately are interconnected. As a general rule, it makes sense to address the plantar fascia, then proceed from the hindfoot to the forefoot in analyzing the deformities and in surgery. Releasing the plantar fascia may alter the amount of bony correction required in any

Assessment of hindfoot deformity by three-dimensional MRI in infant club foot.

In 12 infants aged under 16 months with unilateral club foot we used MRI in association with multiplanar reconstruction to calculate the volume and principal axes of inertia of the bone and cartilaginous structures of the hindfoot. The volume of these structures in the club foot is about 20% smaller than that in the normal foot. The reduction in volume of the ossification centre of the talus (40%) is greater than that of the calcaneus (20%). The long axes of both the ossification centre and the cartilaginous anlage of the calcaneus are identical in normal and club feet. The long axis of the os

Partial wound closure after surgical correction of equinovarus foot deformity.

Full correction of severe equinovarus foot deformities is frequently lost at the end of surgical release when the surgeon closes the skin incision. We retrospectively review 31 feet in 22 patients whose medial skin incisions were left open (typically 10 mm) to heal by secondary intention. The criterion to leave a wound open was if primary closure with the foot in full correction might compromise circulation to the skin or if closing the incision would require loss of corrected position. One or two cast changes were performed under outpatient anesthesia at 7- to 14-day intervals for wound care.

The high incidence of foot deformity in patients with high-level spina bifida.

In a consecutive series of 124 children with spina bifida we found that 220 (89%) of the 248 feet were deformed: 70 had a calcaneus deformity; 126 were in equinus; 16 were in valgus; 3 were in varus; and 5 had convex pes valgus. Operations were performed on 171 (78%) of the deformed feet. Spasticity of the muscles controlling the foot was detected in 36 (51%) of the 70 calcaneus feet and in 22 (17%) of the 126 equinus feet. The deformities were symmetrical in 94 children. There is a high incidence of foot deformity in patients with spina bifida who have no voluntary activity in the motors of t

[Tendon surgery in equinovarus deformity of the foot in children and adolescents with cerebral palsy].

Fifty-eight children or adolescents with cerebral palsy who presented an equino varus deformity of the foot were operated on between the age of 4 to 21 by tibialis posterior tenotomy (23 cases), tibialis posterior transposition (18 cases), tibialis posterior (10 cases) or anterior (6 cases) transfer on the cuboid, with triceps lengthening (37 cases). 56 adolescents were reviewed, 45 had achieved their growth. The foot was painless and weight bearing improved. 15 failures were caused by overcorrection in valgus (7 cases, in 5 of which the tendon was transferred on the cuboid) or under correctio

Transfer of half the calcaneal tendon to the dorsum of the foot for paralytic equinus deformity.

Transfer of half of the calcaneal tendon to the dorsum of the foot will maintain correction of a paralytic equinus deformity. We have used this procedure on 97 feet since 1967. Seventy-six were reviewed and the overall results were excellent or good in 69%. The outcome was better in children with cerebral palsy (85% excellent or good) than in those with poliomyelitis (only 13% excellent or good).

Tibialis posterior tendon transfer corrects the foot drop component of cavovarus foot deformity in Charcot-Marie-Tooth disease.

BACKGROUND: The foot drop component of cavovarus foot deformity in patients with Charcot-Marie-Tooth disease is commonly treated by tendon transfer to provide substitute foot dorsiflexion or by tenodesis to prevent the foot from dropping. Our goals were to use three-dimensional foot analysis to evaluate the outcome of tibialis posterior tendon transfer to the dorsum of the foot and to investigate whether the transfer works as an active substitution or as a tenodesis. METHODS: We prospectively studied fourteen patients with Charcot-Marie-Tooth disease and cavovarus foot deformity in whom twent

Impact of coronal and sagittal hindfoot alignment on metatarsus primus elevatus in patients with rheumatoid foot deformities.

BACKGROUND: Typical foot deformity patterns of patients with rheumatoid arthritis (RA) include hallux valgus, claw toes, splay foot, flat foot, and hindfoot valgus deformities. However, some patients present deformities that are different from a typical pattern, such as metatarsus primus elevatus, described as dorsal elevation of the first metatarsal in relation to the lesser metatarsals. We speculated that metatarsus primus elevatus might be associated with calcaneal inclination and hindfoot varus alignment; however, studies on the association of hindfoot alignment with metatarsus primus elev

Assessment of percutaneous V osteotomy of the calcaneus with Ilizarov application for correction of complex foot deformities.

Complex foot deformity can be described as a foot with multiplanar abnormalities with or without shortening of the foot. Conventional surgical treatment may not be able to correct these deformities. In this study we evaluate the results of percutaneous V osteotomy of the calcaneus with an Ilizarov external fixator for treatment of complex foot deformity. Twenty feet with a complex deformity were treated by the Ilizarov method in 15 patients. The aetiologic factors were neglected or relapsed clubfoot (13 patients) and poliomyelitis (2 patients). All patients underwent percutaneous V osteotomy o

Assessment of the position of the navicular by three-dimensional magnetic resonance imaging in infant foot deformities.

As the navicular is unossified in infants with foot deformities, until 5 years old (boys) and 3 years old (girls), it is difficult to give the exact position of hindfoot bones. Magnetic resonance imaging associated with three-dimensional reconstruction allows the visualization of the cartilaginous volume of the bones and the measurement of the relationships of the navicular with the talus. In normal feet, we noted that in the coronal plane the navicular was laterally situated (75%) relative to the talus. In club feet (17 patients), the navicular and the cuboid had a large medial displacement (

Transposed skin graft for wound closure after Cincinnati incision. Experience in 7 patients with severe foot deformities.

I report a technique of wound closure by a transposed skin graft after operative treatment of foot deformities, using a Cincinnati incision. Three-dimensional correction of severe foot deformities (congenital talipes equinovarus, congenital vertical talus, etc.) results in skin excess on one side of the foot and lack of this on the opposite side. The skin excess can be cut out and used as a full-thickness skin graft for wound closure on the opposite side of the foot.

[Elimination of pathologic torsion of the crural bones during treatment of various severe foot deformities in children].

The author has studied the mechanism of the torsion development of shin bones in normality. The disturbance of the mechanism as an extensive external torsion of shin bones is established in 127 patients with neglected congenital talipes and as an internal twist in critical forms of plano-valgoid deformity of feet in 16 patients. The significance of the two kinds of the torsion pathology of shin bones in the treatment of such feet deformities is substantiated. An original author's method of treatment with an original apparatus is described.

Surgical correction of foot deformities in Charcot-Marie-Tooth-disease: effects on personal goals and gait capacity.

Charcot Marie Tooth disease (CMT) is the most common group of inherited muscular disorders, resulting in foot deformities. Foot and ankle surgical procedures are frequently performed to improve function in persons with CMT. Understanding the patients' personal goals is crucial for effective counselling. However, there is limited evidence regarding the extent to which personal goals are achieved through surgery, as well as the impact of surgery on balance and gait capacity. A prospective cohort study was conducted in 28 persons with CMT who underwent surgery. A wide range of personal goals was

Recurrence rates of burn contracture ankle equinus and other foot deformities in children treated with Ilizarov fixation.

The purpose of this study was to determine recurrence rates of pediatric foot and ankle burn deformities treated with the Ilizarov method. A total of 19 patients with 29 foot and ankle deformities were studied. The most common deformity treated was equinus (23). Rocker-bottom and cavus foot deformities were each treated three times. The average age of the patient at the time of the burn injury was 3.2 years, and the average age at the time of fixation was 9.4 years. Equinus contractures averaged -34 degrees (34 degrees of plantarflexion) before surgery and +7 degrees (7 degrees of dorsiflexion

[Foot deformities and relation to the length of leg muscles in Danish children aged 3-17 years].

Seven hundred and sixty-nine children and adolescents from three to seventeen years of age, being 98% of the pupils in different classes in seven schools and ten kindergartens on Funen, Denmark, were examined in 1991. The Triceps Surae muscle was found to be too short, if the corrected foot can only be pressed slowly to the right angle. The 1520 footprints showed an increase in the number of highly-arched feet through the age groups from 2.3% to 7.3% and a decrease in flat-footedness from 41.8% to 5.7%. In the two oldest age groups, 39% of those with a high arch, 56% of those with a normal arc
