When Indirect Decompression Fails: A Review of 220 Consecutive Direct Lateral Interbody Fusions and Unplanned Secondary Decompression.

STUDY DESIGN: A consecutive series of patients who underwent minimally invasive spinal surgery by a single surgeon at a high-volume academic medical center were studied. OBJECTIVE: The objective of this study was to identify the prevalence, radiographic features, and clinical characteristics of patients who require unplanned secondary decompressive laminectomy or foraminotomy after lateral lumbar interbody fusion (LLIF). SUMMARY OF BACKGROUND DATA: LLIF indirectly decompresses the spinal canal, lateral recess, and neural foramen when properly performed. However, indirect decompression relies…

Tapia syndrome: an unusual complication following posterior cervical spine surgery.

Tapia syndrome, a rare complication of posterior cervical surgery, characterised by concurrent paralyses of recurrent laryngeal branch of vagus and hypoglossal cranial nerves, occurred in a patient after posterior cervical foraminotomies for radiculopathy. We discuss hypothesised pathophysiology, and diagnostic, therapeutic and avoidance strategies in relevance to prone neurosurgical procedures.

Degenerative spondylolisthesis. Its surgical management.

Little attention has been given to the amount of further slip and the success rate after decompression for degenerative spondylolisthesis. In this retrospective study, the answers to the following were sought: (1) Did decompression relieve symptoms; (2) Which decompressive technique was best; (3) Did further slip occur; (4) Did further slip make patients worse? Seventeen patients were available for an average 20-month follow-up. Six patients had a decompression laminectomy with facetectomies, and 11 had a decompression laminectomy with foraminotomies. None was fused. Patients rated their resul

Posterior Cervical Keyhole Laminoforaminotomy: A Cadaveric Comparative Study to Evaluate Limits of Bony Resection.

BACKGROUND: The posterior cervical keyhole (KH) laminoforaminotomy has been described to involve the lateral portion of cervical laminae of the upper vertebra alone (small KH) or of both upper and lower vertebrae (large KH). OBJECTIVE: To microscopically compare the two keyhole techniques in terms of their ability to expose the corresponding cervical roots. METHODS: Ten cadaveric specimens were operated bilaterally from C3-4 to C6-7 level to expose a total of 80 nerve roots. The large KH was applied to the left side, the small KH to the right side. The maximal length of exposed nerve roots w…

Posterior surgical approach to the cervical neural foramen for intervertebral disc disease.

A POSTERIOR FORAMINOTOMY (hemilaminotomy and medial facetectomy) is indicated for the treatment of nerve root compression secondary to posterolateral disc herniation or spondylotic foraminal stenosis. We describe the normal and pathological anatomy of the cervical neural foramen as well as our surgical technique, which has been highly effective in cases of cervical discogenic radiculopathy.

Surgical treatment of vertebro-epidural neoplasms of the cervical tract via the anterior approach.

Fifteen cases of vertebral-epidural neoplasms of the cervical tract, treated surgically by the technique of longitudinal median somatotomy with inlay arthrodesis via the anterior approach are presented. Compression of the spinal anterior artery is eliminated by the wide anterior longitudinal exposition, which also creates a support for a possible second operation, either laminectomy or foraminotomy.

Keyhole approach for posterior cervical discectomy: experience on 84 patients.

Anterior cervical discectomy with or without fusion has been commonly used for cervical disc disease since the description by Smith and Robinson in 1958. In this report, surgical technique, advantages, and disadvantages of the posterior approach, known as the posterior keyhole laminotomy-foraminotomy, are reviewed and motion versus fusion surgery discussed. Between 1996 and 2004, the keyhole laminotomy-foraminotomy was performed on 84 patients suffering from lateral cervical soft disc herniation or osteophytes. All the procedures were performed under the surgical microscope. A high-speed drill…

Neurosurgical procedures, spinal nerve roots - one stage removal of thoracic dumb-bell tumor: role of spinal evoked potential.

We report a rare case of benign thoracic dumb-bell tumor in the upper posterior mediastinum, which was successfully removed by posterolateral thoracotomy and foraminotomy, using intraoperative monitoring of spinal motor-evoked potentials. This technique has many advantages including minimal morbidity and mortality, a single incision, one-step complete resection with adequate exposure, spinal stabilization, avoidance of laminectomy, nerve root identification, and good predicted postoperative function.

Posterior minimally invasive approaches for the cervical spine.

Cervical microendoscopic foraminotomy/discectomy and cervical microendoscopic decompression of stenosis are highly effective, minimally invasive approaches to cervical radiculopathy due to foraminal osteophytes or lateral disc herniation and cervical spondylotic myelopathy, respectively. The operative technique utilized in each of these procedures is described in detail, together with the advantages of the minimally invasive technique over traditional open approaches. Complication avoidance and management is also discussed.
