Chronic exertional compartment syndrome of the forearm in adolescents.

PURPOSE: Chronic exertional compartment syndrome (CECS) is a well-known process, although rare in the forearm. The diagnosis is based on history, clinical examination, and compartment pressure readings. My objective is to present the largest series of CECS of the forearm in adolescents and describe my experience in its management and evolution. METHODS: I reviewed 5 patients, 4 male (competing in motorcycling or motocross) and 1 female (CECS in both the legs and forearms), aged between 15 and 18 years. Volar and dorsal compartments were affected in 3 patients and isolated volar in 2 cases. Th…

Paediatric fracture dislocation of the forearm: an unusual presentation.

Traumatic injuries of the elbow in children pose a diagnostic challenge. They demand precise knowledge of radiographic anatomy, growth characteristics of healthy and injured bones as well as specific fracture dynamics in this age group. Dislocation of the elbow is rare in children, accounting for 3-6% of all childhood elbow injuries. Fractures at the distal humerus and proximal forearm have been extensively discussed in the literature. We describe a unique case of a 4-year-old child presenting with a traumatic radial head fracture and ulnar dislocation not previously reported. Diagnosis and re…

Nonunion of forearm fracture: a rare instance in a toddler.

When compared to adults, pediatric fractures unite readily and nonunion is quite rare. Nonunion is often associated with open fractures, operative interventions, infection, pediatric osteogenesis imperfecta and neurofibromatosis. There are only a few studies and reports mentioning nonunion following conservative ma- nagement of closed pediatric fractures. We report here a case of an eighteen-month-old child who developed nonunion following treatment of fracture of both forearm bones with cast and was successfully treated with plating. To the best of our knowledge, this is the youngest reported…

Penile reconstruction with the radial forearm flap: an update.

BACKGROUND: Many methods and many free or pedicled flaps have been used in phalloplasty. None of these techniques is able to completely fulfill the well described goals in penile reconstruction. Still, the radial forearm glap is currently the most frequently used flap and thus universally considered the gold standard. PATIENTS AND METHODS: Since 1992, we have performed the largest series of 316 radial forearm phalloplasties to date performed by a single surgical team. From these extensive data we critically evaluate how this current supposed gold standard can meet the requirements of an ideal…

Current strategies for sarcoma reconstruction at the forearm and hand.

Multistructural reconstructive procedures are often required to achieve a sufficient reconstruction of the forearm and/or hand after radical tumor resection. Clear margin (R0) resection is the main therapeutic goal for the treatment of sarcomas. Plastic reconstructive procedures with the possibility of microvascular transplantations play a key role in coverage of complex defects. In our department, 20 patients with soft tissue sarcomas of the hand/forearm were treated between 1995 and 2005. Eleven patients were male, nine female. The average follow-up time was 42 months. The most common tumor …

Isolated tuberculous tenosynovitis of the forearm in an immunocompetent patient.

Primary tuberculous tenosynovitis is a rare manifestation of extraspinal musculoskeletal tuberculosis. The diagnosis may be easily delayed because of its nonspecific clinical signs. We report a case of culture-proven tuberculous tenosynovitis of the extensor carpi ulnaris tendon and common extensor tendon in a 68-year-old female without concomitant pulmonary tuberculosis, nor documented immunodeficiency. The diagnosis was initially overlooked due to the lack of appropriate histological and bacteriological analyses and the lesion recurred after surgery. MR imaging represents the most accurate m

[Therapy principles of distal fractures of the forearm in childhood].

Fractures of the forearm in children represent one of the most frequent injuries. Most are monotraumatic occurring during sport or leisure activities. Diagnosis can be made by conventional X-ray examination in 2 planes. Distal forearm fractures are classified into epiphyseolysis and epiphyseal fractures, metaphyseal greenstick and buckle fractures, complete distal radius and forearm fractures as well as Galeazzi lesions. The vast majority of cases without relevant dislocation can be treated with immobilization employing a cast. The remaining fractures have to be repositioned and restored depen…

Clear-cell hidradenoma of the forearm in a young boy.

We treated a 5-year-old boy with clear-cell hidradenoma of the forearm. Our literature search found only one previous report of this disease occurring during the first decade of life, and the present patient is the first with involvement of the limbs. This presentation demonstrates that clear-cell hidradenoma needs to be considered in the differential diagnosis of cutaneous tumors even in young children, which is of interest because malignant transformation can occur.

[Combined injuries of arteries, nerves and tendons of the forearm].

Combined injuries of the forearm occupy an important place in modern structure of functionally severe traumas. Discussions concerning the best time for nerve reconstructions and unsolved questions of a necessity to restore one of the arteries if the other is safe confirms the importance of the problem. An analysis of the domestic and foreign literature data on diagnosing and treatment of the traumas in question is given. Main specific features of treatment depending on the time, character of injuries and functional role of the injured structures are described.

[Bipolar dislocation of the forearm: elbow and perilunate dislocation].

We report a case of bifocal dislocation of the forearm due to posterior dislocation of the elbow and transcaphocapital retrolunate dislocation of the carpus, Fenton's syndrome, associated with fracture of both bones of the forearm. The emergency procedure achieved reduction of the elbow, plate fixation of the radius and ulna, used a posterior and an anterior access to achieve reduction and osteosynthesis of the radial styloid, the scaphoid, and the capitatum and temporary schapholunate, triquetrolunate, scaphocapitate, and radiolunate pinning. At ten months, the patient only complained of mode…

[Overuse injury syndromes of the hand, forearm and elbow].

The basic role of the shoulder, upper arm, elbow, forearm, and wrist is to place the hand in the appropriate position, so that it can fulfill its function. The upper extremity, especially the wrist and the hand, has a very complex anatomy with a large number of bones, muscles, tendons and nerves. They are essential to work and sport activities and are subject to acute and/or chronic mechanical injuries. This is why overuse injuries are the most common in these regions. They are often characterised by tendinitis, tenosynovitis, tunnel syndromes, or stress fractures. This paper gives an overview…

The management of forearm fractures in children: a plea for conservatism.

A retrospective review was undertaken to evaluate the efficacy of primary nonoperative treatment (closed reduction and long-arm casting) alo … oup treated in this manner included two superficial pin infections treated with antibiotics and two forearms with moderate loss of pronation/supination not requiring treatment. We believe that closed reduction of pediatric forearm fractures remains the accepted standard and the technique of pins and plaster should be considered a reliable alternative for …

Quantitative bone mineral assessment at the forearm: a review.

Bone mineral density and geometric properties of the human forearm can be measured to determine the amount of bone or bone loss at the scanning site and to predict the risk of forearm fractures. These forearm measurements are also used to estimate bone mass at remote anatomical locations and thereby estimate the risk for spine, hip and other fractures. The peripheral location of the human forearm, with its relatively small amount of surrounding soft tissue, improves the accuracy and the precision of bone mass measurement and has made this site an early choice for the assessment of a subject's …

[Functional fracture treatment of the forearm. The indications and results].

In a retrospective study from 1986 to 1995, 64 forearm fractures were treated with brace. Clinical and roentgenographic follow-up data were available for 49 patients (76.6%). There were 49.0% ulna shaft, 38.9% radius and 12.2% forearm fractures. The average time to healing was 10.2 weeks. Functional results were excellent in 57.1%, good in 34.7% and poor in 8.2% of cases. Two (4.1%) fractures (radius, forearm shaft) were not considered as healed and were re-operated with plate osteosynthesis. The ideal indication for fracture bracing is ulna shaft fracture. Radius and forearm shaft fractures c…

Hydrochlorothiazide exerts no direct vasoactivity in the human forearm.

BACKGROUND: Recently, hydrochlorothiazide has been shown to relax vascular smooth muscle in vitro in clinically relevant concentrations by the opening of calcium-activated potassium channels, leading to hyperpolarization and consequent closing of voltage-operated calcium channels. Long-term administration of hydrochlorothiazide reduces peripheral vascular resistance in vivo in man. These results indicate that hydrochlorothiazide has hemodynamic activity, and we therefore examined the direct vascular action of this drug in vivo. SUBJECTS AND METHODS: Forearm vasodilator responses to the infusi…

Direct revascularization for the treatment of forearm and hand ischemia.

During an 8-year period, 65 patients underwent operation for ischemia of the arm and hand. Fifty-four patients presented with an acutely ischemic extremity. Twenty-two had cardiac origin emboli, 9 had arterial-arterial emboli, 8 had traumatic occlusion of the brachial artery, and 15 patients developed arterial insufficiency after iatrogenic intervention. Twenty-one of 22 patients with cardiac emboli were successfully treated with embolectomy alone. The majority of patients with arterial-arterial emboli and traumatic or iatrogenic injuries required arterial reconstruction in addition to clot re

Toe transfer to the vessels of the reversed forearm flap.

A 23-year-old man had a severe degloving injury of his hand, amputation of multiple fingers, and devascularization of the thumb. Primary care consisted of successful revascularization of the thumb and skin grafting of the hand. Secondary soft tissue reconstruction of the hand was done with a reverse forearm flap. This was followed by second-toe transfer to the hand, with the recipient vessels, the radial artery, and the venae comitantes of the reverse forearm flap.
