Multimodality Imaging in Wrist Fractures and Dislocations.

Wrist fractures and dislocations are frequently encountered in the emergency department and can cause significant long-term disability. Imaging plays a crucial role in the evaluation of wrist injuries, with conventional radiography being the first imaging investigation. Cross-sectional imaging is playing an increasingly important role in management of wrist injuries. Computed tomography with 3D and multiplanar reformatting capabilities is in particular useful for detailed evaluation of bony injuries and can provide vital information to orthopaedic surgeons for adequate surgical planning. Traumatic Cervical Facet Fractures and Dislocations.

»: Flexion-distraction, axial loading, and rotational forces can cause various degrees of osseoligamentous disruption of the cervical spine, leading to traumatic cervical facet fractures and dislocations. »: Low-energy forces lead to minimally displaced facet fractures that often can be treated with immobilization only. High-energy forces are more likely to cause unstable injuries with or without neurologic compromise, which may require surgical intervention. »: The initial treatment of cervical facet injuries requires patient evaluation and management through the Advanced Trauma Life Suppor

Galeazzi Fracture Dislocations: An Illustrated Review.

Galeazzi fracture dislocations are a fracture of the distal one third of the radius shaft with a concomitant dislocation of the distal radioulnar joint (DRUJ). These injuries usually occur by axial loading on an outstretched arm with pronation or supination of the wrist which determines the angulation of the fracture. Surgical treatment has been historically by the anterior (volar) approach to the forearm with plate fixation with or without pinning of the distal radioulnar joint. Failed or inadequate treatment may lead to complications including chronic pain, malunion or instability of the DRU…

Management of Adult Elbow Fracture Dislocations.

Elbow fracture dislocations are complicated injuries that are difficult to manage and fraught with complications. A complete series of radiographs is typically complemented with CT scan to evaluate the elbow and assist preoperative planning. Typically, operative intervention is necessary and a systematic approach to the elbow injuries should be chosen. This article addresses the coronoid and proceeds to the radial head, lateral soft tissues, and finally the medial ligaments if elbow instability persists. With a focused, systematic surgical approach, improved outcomes have been demonstrated and…

Fourth and fifth carpometacarpal fracture dislocations.

OBJECTIVE: The aim of this study was to evaluate the mid-term results of patients with surgically treated 4th and 5th carpometacarpal (CMC) fracture dislocation. METHODS: The study included 9 CMC dislocation patients (9 males; mean age: 31.2 years, range: 20 to 40 years) treated with open reduction and internal fixation between 2008 and 2012. Mean follow-up period was 19.4 months. Mean interval between trauma and operation was 10.7 (range: 3 to 35) days. Radiological evaluation was performed at the final follow-up. Hand grip power was measured using a hand dynamometer and the injured and unin…

[Diagnosis and treatment of radiocarpal fracture dislocations].

In contrast to the common intra- or extra-articular fractures of the distal radius, radiocarpal fracture dislocations are rare injuries. Concerning this issue, only a small number of publications can be found. Nevertheless, it is important to be informed about this injury since prompt operative treatment is often required and immobilization alone will not be sufficient. Sometimes, radiocarpal fracture dislocations are combined with carpal injuries. In such cases, both the radiocarpal dislocation and carpal injury have to be treated. Diagnostic difficulties can lead to misinterpretation or unde…

[Therapeutic management of dislocated laryngeal fractures].

BACKGROUND: Acute trauma with laryngeal fracture is a rare injury affecting all functions of the larynx. Resuscitation follows the ABC principles for acute trauma life support. The priority has always been the establishment of an adequate and secure airway. To achieve optimal therapeutic long-term results, permanent stabilisation of the airway and good functional restoration of phonation and swallowing are necessary. We describe the therapy and long-term follow-up of two patients who suffered laryngeal fracture. CASE REPORTS: The first case concerns a 31-year-old woman who suffered polytrauma

[Arterial vascular injuries in fractures and dislocations].

We analyzed reasons, numbers and results of arterial lesions accompanying fractures (n = 21) and luxations (n = 6) in a 6-year-period (1993-1998) retrospectively. Traffic accidents were in nearly 50% responsible for the injuries. 8 patients had suffered multiple injuries. In 17 patients the lower, and in 10 patients the upper extremities were affected. The vascular wall was completely disrupted or severed in 74%. In 7 cases (26%), patients had suffered blunt or indirect arterial trauma with intima- and media-lacerations due to subcapital fracture of the humerus (n = 2), fractured femoral bone

Vascular trauma associated with fractures and dislocations.

Vascular trauma occurs relatively infrequently in association with general orthopedic trauma but may be seen more often in injuries involving joint dislocations and areas in which vascular structures are tethered at the fracture site. The mechanisms of vascular trauma are identified, and the general principles in managing these injuries, including the operative approach to the injury itself and the options in repairing both arterial and venous injuries, are discussed. The role of fasciotomy and primary amputation are also reviewed. Several specific injuries, including pelvic fracture, knee dis

Thoracolumbar spine stabilization in fracture--dislocations.

This study analyzed a series of 35 patients with unstable fractures, with and without neurologic deficit including complete paraplegia. The patients underwent spinal stabilisation using plates and screws after sustaining fracture dislocations. Very good results were obtained in 19 patients with non-complicated fractures operated on by dorsal stabilization using Meurig-Williams plates. Complete recovery of the neurologic deficit was achieved in five out of 13 patients with neurologic deficit two of whom were paraplegic. Use of stabilization operation accompanied by dorsal vertebrodesis and post…

[Osteosynthesis in dislocated fractures of the humerus head].

The operative treatment of displaced fractures of the proximal humerus has changed during the last few years. Minimal osteosynthesis takes into consideration to a great extent the biological aspects of these fractures and provides sufficient stability for early functional therapy. Neer's classification has proved very useful for both the indication for and the choice of an operative procedure. The high necrosis rate of the operatively stabilized humeral head has considerably influenced the operative strategy. This is shown in two separately controlled patient groups. From 1970 to 1980, 65 pati

Sport-related fractures and dislocations in the hand.

The long-term prognosis regarding any sport-related injury should remain guarded. Although optimism may be expressed by the physician regarding a specific injury, the athlete must be adequately informed of the natural history of his or her injury. The hand surgeon's management efforts must always be exact and appropriate, but the athlete must understand from the outset that the final functional and cosmetic result may depend more on the nature of the specific injury than on the treatment provided.

Fractures and dislocations of the distal radioulnar joint.

Fractures and dislocations of the distal radioulnar joint are frequently visualized as a secondary problem in comparison to the more apparent radius fractures. Frequently, in the long-term follow-up of patients with radius fractures, ulnar wrist pain secondary to distal radioulnar joint incongruity is the final outcome. Therefore, in the evaluation of the injured forearm the distal radioulnar joint must be assessed clinically and radiographically. In this assessment if distal radioulnar joint instability or incongruity is present then joint stabilization or reduction, respectively, must be att

Management of posterior fracture-dislocations of the shoulder.

Posterior fracture-dislocation of the shoulder is rare. Comprehensive treatment guidelines for posterior fracture-dislocation of the shoulder with fracture of the humeral head have not been previously published. Although open reduction and internal fixation of the proximal humerus for posterior fracture-dislocation has been reported in several series, the successful reconstruction of the articular surface by rigid internal fixation of a large osteochondral fragment has not been reported. This paper describes two cases of posterior fracture-dislocation of the shoulder with a substantial defect

Treatment of pelvic fractures and associated dislocations.

Fractures of the pelvis result in high morbidity and mortality as a result of uncontrolled hemorrhage and associated soft tissue damage. Early reduction and fixation of these fractures has been shown to result in control of hemorrhage, decreased morbidity and mortality, and improved function. Seventeen pelvic fractures, so treated, are reported. Despite a high complication rate of almost 75%, surgical fixation resulted in rapid control of hemorrhage, restoration of hip joint congruity, early mobilization of the patient, and decreased incidence of decubitus ulcers, bronchopneumonia, thrombophle
