Intramedullary Fixation Versus Plate Fixation of Distal Fibular Fractures: A Systematic Review and Meta-Analysis of Randomized Controlled Trials and Observational Studies.

Intramedullary fixation (IMF) has been described as a minimally invasive alternative to open reduction and internal fixation for operative treatment of distal fibular fractures in case of compromised soft tissue or severe comorbidities. The objective was to compare postoperative complications and functional outcomes of intramedullary versus plate fixation (PF) in distal fibular fractures. A systematic review and meta-analysis was performed. The PubMed/MEDLINE, Embase, Cochrane, and CINAHL databases were searched for both randomized controlled trials and observational studies. A total of 26 stu

Intramedullary nail fixation versus open reduction and internal fixation for treatment of adult diaphyseal forearm fractures: a systematic review and meta-analysis.

BACKGROUND: Diaphyseal radius and ulna fractures require surgical fixation in adults. Open reduction and internal fixation (ORIF) have been considered the gold standard of treatment. The recent development of an interlocking intramedullary nail (IMN) has provided an alternative treatment method for these fractures. The objective of this meta-analysis is to compare the outcomes and complications of IMN versus ORIF for diaphyseal forearm fractures in adults. METHODS: MEDLINE and Embase were searched from January 1, 2000, through January 7, 2024. All English-language studies were included compar

Elastic stable intramedullary fixation using epibloc versus crossed kirschner wires fixation for distal forearm fractures in children: A retrospective analysis.

BACKGROUND: Childrens' distal forearm fractures (DFFs) could be treated conservatively with closed reduction and immobilisation, but post-reduction displacements often occur. Displaced DFF should be surgically fixed, to avoid further displacement. Nevertheless, immobilisation after surgery is recommended. Epibloc system (ES), a system of stable elastic nail fixation, is widely used to stabilise adults extra-articular distal radius fractures, with advantages to not requiring post-surgical immobilisation. The present investigation represents a retrospective analysis of paediatric patients with D

Fracture table vs. lateral positioning for intramedullary fixation of femur fractures (The FLiP Trial): the feasibility of a cluster randomized crossover trial.

BACKGROUND: Femoral shaft fractures are common severe injuries that carry an elevated risk of operative complications, including femoral malrotation, neurologic, and vascular injuries. There is no definitive research comparing two commonly used surgical tables and patient positions in aiding with both reduction and fixation of femoral shaft fractures. The objective of this pilot trial was to test the feasibility of a cluster randomized crossover trial that assesses the comparative effectiveness of supine positioning on a fracture table versus lateral positioning on a radiolucent table for ante

Internal fixation of dorsally comminuted fractures of the distal part of the radius: a biomechanical analysis of volar plate and intramedullary nail fracture stability.

INTRODUCTION: The purpose of the present study was to carry out biomechanical testing of "new generation" volar plates and an intramedullary nail. METHODS: Four volar locking plates (Column Plate, VariAx distal radius, 2.4 mm-LCP and 3.5 mm-LCP) and the intramedullary nail, Targon-DR, were implanted in biomechanically validated artificial bones after simulation of a wedge osteotomy with total transection of the volar cortex to mimic a type 23 A3-fracture according to the AO-classification. Axial load (250 Newton [N]) and volar and dorsal bending loads (both 50 N) were applied. Axial load was

UK Fixation of Distal Tibia Fractures (UK FixDT): protocol for a randomised controlled trial of 'locking' plate fixation versus intramedullary nail fixation in the treatment of adult patients with a displaced fracture of the distal tibia.

INTRODUCTION: The treatment of displaced, extra-articular fractures of the distal tibia remains controversial. These injuries are difficult to manage due to limited soft tissue cover, poor vascularity of the area and proximity to the ankle joint. Surgical treatment options are expanding and include locked intramedullary nails, plate and screw fixation and external fixator systems. The nail and plate options are most commonly used in the UK, but controversy exists over which treatment is most clinically and cost-effective. In this multicentre randomised controlled trial we aim to assess ratings

Comparative Analysis of Intramedullary Nail versus Plate Fixation for Fibula Fracture in Supination External Rotation Type IV Ankle Injury.

BACKGROUND Lateral malleolus fractures, typically from trauma, sports, or accidents, are common, with supination external rotation (SER) injuries being most prevalent. SER injuries involve complex joint mechanics and often necessitate surgical intervention for instability. This study compares intramedullary nail and plate fixation for fibula fractures in SER type IV ankle injuries, considering their biomechanical properties and influence on fracture healing. MATERIAL AND METHODS A prospective, randomized study was conducted between January 2021 and December 2021. A total of 81 patients with SE

Intramedullary Plate Fixation and Viable Bone Allografting in a Complex Intra-articular Distal Radius Fracture Nonunion: A Case Report.

In this report, we detail a 69-year-old female who sustained a comminuted intra-articular left distal radius fracture that failed to heal with bridge plate fixation. Given the patient's poor subchondral bone stock and refusal of bone autograft, we designed a construct using a dorsal spanning plate and an intramedullary fragment-specific plate as a volarly placed strut in combination with viable bone allograft and cancellous bone chips to treat this nonunion. This case demonstrates an option for distal radius non-union treatment and highlights the importance of ingenuity that orthopedic surgeon

Treatment of Jones fracture nonunions and refractures in the elite athlete: outcomes of intramedullary screw fixation with bone grafting.

BACKGROUND: Fractures of the proximal fifth metatarsal metaphysis (ie, the Jones fracture) can be problematic in the elite athlete because of a high incidence of nonunion and refracture with nonoperative treatment. Although these fractures are not common, athletes can suffer refracture or nonunion of a Jones fracture despite operative stabilization. This is often attributable to hardware of insufficient strength, aggressive postoperative rehabilitation, or biologic insufficiency at the fracture site. PURPOSE: The authors review the results of revision intramedullary screw fixation with cancel

Clamshell Corticotomy: A Technique to Address Challenges of Narrow Medullary Canal during Intramedullary Nailing of Tibial Shaft Fracture Fixation.

Intramedullary nailing for diaphyseal fractures is a gold standard procedure, that is, often associated with some intraoperative challenges. Fixing an intramedullary nail in a narrow canal in an unplanned surgery is a nightmare. Forceful implantation leads to iatrogenic fracture or comminution at the fracture site. Forceful reaming in such scenarios is also associated with thermal osteonecrosis and at times incarceration of flexible reamer head. It is not uncommon to face a challenge intraoperatively with regards to a narrow medullary canal, however, well we plan preoperatively. Clamshell oste

Cost-effectiveness analysis of routine follow-up for hip fracture patients after dynamic hip screw and intramedullary nail fixation.

INTRODUCTION: The rising number of hip fractures has incentivised several quality improvement initiatives aimed at improving outcomes. These include the national hip fracture audit and the best practice tariff. Whilst there is an established standard of care for inpatients, the optimal outpatient management of patients after hip fracture fixation remains undefined. We aim to evaluate the cost-effectiveness of routine surgical appointments and provide evidence to support an improved outpatient hip fracture care pathway that focuses on more comprehensive multidisciplinary treatment. METHODS: Th

Fracture Fixation Strength in Metacarpal Plating Versus Intramedullary Nailing Using a 3-Point Bending Model: A Cadaveric, Biomechanical Study.

PURPOSE: The purpose of our study was to evaluate the ultimate load to failure for 2 metacarpal fracture fixation strategies, plating versus intramedullary nailing, and then compare them to the native metacarpal. Our hypothesis was that the intramedullary nail after fracture fixation would more closely restore the native strength of the metacarpal when compared to plate fixation. METHODS: Matched pairs of cadaveric hands (age range, 19-49; 5 men and 6 women) were dissected to produce 88 intact and equally distributed metacarpals (little, ring, middle, and index fingers). The metacarpals were

Intramedullary Fixation of Metacarpal Fractures Utilizing Retrograde Screws versus Antegrade Steinmann Pins: A Retrospective Review.

While the antegrade intramedullary pin remains a simple, inexpensive, and accessible technique with consistently favorable outcomes, retrograde headless compression screws offer a faster operative solution, with excellent radiographic alignment and low complication rates. The decision between these two implants should be individualized based on fracture pattern, implant availability, patient needs, and surgeon experience.

Flexor Tendon Injury Risk With Percutaneous Intramedullary Screw Fixation for Lesser Metatarsal Fractures: A Cadaveric Study.

BACKGROUND: Percutaneous intramedullary screws have recently become a fixation option for the surgical management of lesser metatarsal fractures. Their advantages over Kirschner (K)-wire fixation include avoidance of pin site complications, fixation stability, and need for removal. To date, there has been limited literature published on their use and safety profile. The purpose of this study is to determine the frequency and extent of flexor tendon disruption associated with percutaneous intramedullary screw fixation for lesser metatarsal fractures. METHODS: Seven left and right lower extremi

Intramedullary nailing versus plate fixation for the treatment of proximal tibia extra-articular fractures: a meta-analysis.

PURPOSE: The incidence of proximal tibia extra-articular fractures is fairly common accounting for approximately 5-11% of all tibia fractures (Lang in Clin Orthop Relat Res 315:64-74, 1995; Freedman in Clin Orthop Relat Res 315:25-33, 1995). The optimal treatment method for these fractures between intramedullary nailing and proximal locking plate remains equivocal. The purpose of this meta-analysis is to assess the clinical, radiological, and functional outcomes as well as the complications in adult patients with extra-articular proximal tibia fractures treated with IMN or PLP.
