Utility of biopsy and frozen section in detecting malignancy within clinically suspicious verrucous growths: a retrospective study.

Oral verrucous carcinoma (OVC) is a low-grade warty variant of oral squamous cell carcinoma. Although these verrucous lesions appear clinically malignant, biopsies often report them as benign, leading to repeat biopsies and diagnostic delay. A retrospective review of patients who underwent surgery for clinically suspicious oral verrucous growths between 2012 and 2022 was conducted. Overall, 228 patients were identified. Final histopathology confirmed OVC in 80 patients (35.1%). Biopsy showed very low sensitivity of 32.4% but high specificity of 93.5% for detecting malignancy in clinically susp

The role of rapid IHC in elevating STAS diagnostic accuracy during intraoperative frozen section for lung adenocarcinoma.

AIM: The aim of this study was to evaluate whether the utility of cytokeratin (AE1/AE3) rapid immunohistochemistry (IHC) in combination with haematoxylin-eosin (H&E) staining improves the intraoperative diagnosis of spread through air spaces (STAS) on frozen sections (FS). METHODS AND RESULTS: This study included 153 patients. Three pathologists independently evaluated STAS on FS using either HE staining alone or in combination with rapid IHC, with postoperative paraffin sections serving as the gold standard. Sensitivity and specificity were compared, along with interobserver and intraobserve…

Misdiagnosis of Endometrioid Borderline Ovarian Tumors by Intraoperative Frozen Section Pathology: A Case Report and Review of the Literature.

BACKGROUND Endometrioid borderline ovarian tumors (EBOTs) are a rare subtype posing a significant diagnostic challenge on intraoperative frozen section analysis due to their morphological overlap with benign lesions and invasive carcinoma. Misdiagnosis, particularly as adenocarcinoma, can lead to unwarranted extensive surgical staging, resulting in potential overtreatment. This study aims to highlight the diagnostic challenge of a rare bilateral EBOT on intraoperative FS analysis and the consequent risk of surgical overtreatment, through a case report and a review of the literature. CASE REPOR

From Cytology to Frozen Section: Diagnostic Challenges in Riedel's Thyroiditis-A Case Report and Brief Literature Review.

Background: Riedel's thyroiditis is a rare fibrosing thyroid disorder that remains one of the most difficult to diagnose, often being initially interpreted as malignant due to its clinical, radiological, and histopathological similarities with anaplastic carcinoma or other infiltrative thyroid diseases. Preoperative investigations, including fine-needle aspiration cytology (FNAC), may be misleading and contribute to an erroneous diagnosis of cancer. Methods: We report the case of a 44-year-old woman presenting with a rapidly enlarging, hard goiter associated with compressive symptoms and cytol

Intraoperative Frozen Section Biopsy for the Auxiliary Diagnosis of Transmural Intestinal Intermediate T-Cell Canine Lymphoma.

Intestinal lymphoma is characterized by a malignant lymphoid neoplasm with multifactorial causes, variable clinical symptoms, and, in some cases, rapid progression. This article describes a case of intestinal lymphoma in a dog that had a history of diarrhea lasting two months, accompanied by episodes of hematochezia, which was treated using the CHOP 19 protocol. A seven-year-old male Golden Retriever presented to a veterinary clinic with clinical signs of diarrhea, prostration, and the presence of bloody feces, as reported by the owner. Initial examinations, including hematology, serum biochem

Intraoperative Decision-Making: A Comparative Analysis of Scrape Cytology vs. Frozen Section in Eighty Consecutive Samples.

INTRODUCTION: Intraoperative tumor evaluation is essential for optimizing surgical decision-making and can prevent the need for unnecessary radical surgeries. Although frozen section (FS) is the gold standard for such evaluations in advanced centers, scrape cytology presents a simpler, cost-effective alternative that could be beneficial in resource-limited settings. However, this technique is often overlooked in favor of FS due to greater pathologist confidence in FS. MATERIALS AND METHODS: This study included 80 consecutive intraoperative samples from the Department of Pathology. Both scrape

Novel Diagnosis of Isolated Cardiac Erdheim-Chester Disease by Interatrial Biopsy Using Frozen Section Examination.

Erdheim-Chester disease (ECD) is a rare non-Langerhans cell histiocytosis characterised by multiorgan pathological histiocytic infiltration. Cardiac involvement occurs in 40-75% of patients and increases mortality. A 55-year-old woman with chest pain and dyspnoea was found to have an interatrial septal mass and aortitis suspicious for ECD without other organ involvement. After two inadequate transcatheter interatrial biopsies were obtained using ultrasound guidance alone, diagnostic samples were successfully obtained using intraprocedural frozen section examination. The patient started genotyp

Frozen section analysis of pancreatic resection margins during pancreaticoduodenectomy for pancreatic adenocarcinoma is not affected by neoadjuvant therapy.

BACKGROUND/OBJECTIVES: The aim of our study was to evaluate if the histopathological changes occurring in the pancreas post neoadjuvant-therapy (PNAT) for pancreatic ductal adenocarcinoma (PDAC) may negatively affect the assessment of intra-operative frozen section (FS) analysis of pancreatic resection margins (PRMs). METHODS: The clinicopathological data of patients who underwent pancreatoduodenectomy for PDAC between 2015 and 2022 were analyzed. Comparison of the accuracy of the FS analysis in treatment naïve (TN) and PNAT patients for all pancreatic margins was performed. RESULTS: We iden

Intraoperative diagnosis of breast cancer metastasis in axillary lymph nodes: Cytological Smear vs. Frozen Section.

BACKGROUND: Intraoperative evaluation of axillary lymph nodes is sometimes required to determine the extent of surgery. In this study, we wished to assess the reliability of cytologic smear (CS) in determining lymph node involvement with tumor. Theoretically, CS provides more substance for examination than touch-imprint cytology and is faster to perform than frozen section (FS). We hypothesized that CS sensitivity for tumor cell detection in the lymph nodes would be similar to FS, at least 0.90. METHODS: This was a retrospective observational study at the Rambam Health Care Campus (January, 2

Reconsidering the Need for Intraoperative Frozen Section in Sentinel Lymph Node Biopsy for Early Breast Cancer in Bahrain.

OBJECTIVES: Frozen section is performed in sentinel lymph node biopsy (SLNB) for early breast cancer to prevent a second surgery, but it has certain disadvantages. Several trials have demonstrated the oncological safety of avoiding axillary dissection in patients with 1-2 positive nodes. This study aimed to assess the need for frozen section during SLNB in early breast cancer. METHODS: This retrospective study included patients with early-stage clinically node-negative breast cancer who underwent SLNB with frozen section at Salmaniya Medical Complex, Manama, Bahrain, between October 2021 and

Challenging pitfalls in frozen section pathology: a case of mandible ghost cell odontogenic carcinoma and the literature review.

BACKGROUND: Ghost cell odontogenic carcinoma (GCOC) is a rare malignancy characterized by the presence of ghost cells, preferably in the maxilla. Only slightly more than 50 case reports of GCOC have been documented to date. Due to the rarity of this tumor and its nonspecific clinical criteria, there is a heightened risk of misdiagnosis in clinical examination, imaging findings, and pathology interpretation. CASE PRESENTATION: A 50-year-old male patient presented to the hospital due to experiencing pain in his lower front teeth while eating for the past 2 months. Upon examination, a red, hard,

Frozen Section Timeout: Pilot Study to Reconcile Margins Using 3D Resected Specimen and Defect Scans.

OBJECTIVE: Opportunities exist to improve intraoperative communication and documentation of resection margin details. We instituted a "frozen section timeout" that centers around visualization of the paired resection specimen and surgical defect-facilitating effective, bidirectional exchange of information. METHODS: We designed an interactive form for use during the "frozen section timeout" including annotated 3D virtual models of the resected specimen and surgical defect, plus a "line-item" table for primary and supplemental margin results. The "timeout" was conducted over a Zoom call betwee

An unexpected guest: Pulmonary echinococcosis diagnosed by intraoperative frozen section examination. A case report and literature review.

Echinococcosis is caused by tapeworms belonging to the Echinococcus genus. The most common site of infection is the liver although it may involve almost any organ. Symptoms of pulmonary echinococcosis vary depending on the location and structure of the cyst. While uncomplicated cysts usually appear at imaging as well-defined homogeneous lesions with fluid content and smooth walls of variable thickness, complicated lesions may have a more heterogeneous content with higher density making more difficult the distinction from malignancies or other infections. Hereby we describe the case of a 61-yea

Intraoperative intact parathyroid hormone monitoring and frozen section diagnosis are essential for successful parathyroidectomy in secondary hyperparathyroidism.

BACKGROUND: Total parathyroidectomy (PTx) is often performed to treat secondary hyperparathyroidism (SHPT). Successful PTx is essential to prevent recurrent and persistent SHPT because remnant parathyroid glands (PTGs) in the neck can be stimulated and may secrete excessive parathyroid hormone (PTH) in end-stage renal disease. However, to date, few studies have investigated factors contributing to successful PTx before the completion of surgery. MATERIALS AND METHODS: Between August 2010 and February 2020, 344 patients underwent total PTx, transcervical thymectomy, and forearm autograft for S

Accuracy of fine-needle aspiration and frozen section for the detection of squamous metastasis in cystic masses of the lateral neck.

Cystic masses of the lateral neck are mostly benign. However, the incidence of metastatic squamous cell carcinoma (SCC) in cervical cystic masses initially diagnosed as benign is quite high in patients older than 40 years. The aim of this study was to compare the diagnostic accuracy of preoperative cytology and intraoperative frozen section (FS) in detecting malignancy in cystic masses of the neck. We reviewed 61 patients who underwent preoperative ultrasound-guided fine-needle aspiration cytology (FNAC) and neck biopsy of a cystic neck mass, and analysed the concordance between FNAC and intra
