Health Disparities, Inequalities and Inequities in Stroke Care: A Global Critical Integrative Review (1991-2024).

Growing evidence demonstrates the impact of health disparities on stroke care and outcomes. Although reviews have examined disparities related to sex and gender, race and ethnicity, socioeconomic status and rural-urban residence, most have not accounted for the influence of multiple intersecting social identities or positions. This critical integrative review used a modified integrative review methodology with an intersectionality approach. We reported bibliographic trends and generated Upset Plots to analyze intersections among categories of social identity/position. Analysis was iterative to

Determinants of women's employee engagement in the Bangladeshi workforce.

This study examines the determinants of women's employee engagement in Bangladesh, focusing on socio-cultural influences, organizational culture, leadership support, work-life integration, and career development opportunities. Drawing on a cross-sectional survey of 242 women employees across four industries, the study applies partial least squares structural equation modeling (PLS-SEM) to estimate the associations between these five determinants and women's employee engagement. Organizational culture, leadership support, and work-life integration are each positively associated with women's emp

Applying Critical Feminist and Critical Race Theory to Address the Cultural and Financial Needs of Black/African American Women With Cancer: A Narrative Review.

Black/African American women with breast and gynecologic cancers face stark end-of-life (EOL) inequities rooted in structural racism, gendered oppression, and financial toxicity. Despite abundant evidence of outcome gaps, theory-driven guidance for equitable, culturally responsive EOL care is limited. This narrative review argues for integrating Critical Race Theory (CRT) and Critical Feminist Theory (CFT) into EOL care frameworks to address the cultural and financial needs of Black/African American women. Drawing on interdisciplinary literature from healthcare, philosophy, sociology, and econ

Gendered health systems: evidence from low- and middle-income countries.

BACKGROUND: Gender is often neglected in health systems, yet health systems are not gender neutral. Within health systems research, gender analysis seeks to understand how gender power relations create inequities in access to resources, the distribution of labour and roles, social norms and values, and decision-making. This paper synthesises findings from nine studies focusing on four health systems domains, namely human resources, service delivery, governance and financing. It provides examples of how a gendered and/or intersectional gender approach can be applied by researchers in a range of

Exploring Bias-Based Bullying and Intersecting Social Positions as Correlates of Sexual Risk Behaviors Among Adolescents.

Purpose: The current study extends the limited body of intersectional research on adolescents' sexual health by examining experiences of bias-based bullying and multiple intersecting social positions associated with engagement in sexual risk behaviors. Methods: Participants were 14,968 sexually active 9th and 11th grade students surveyed as part of the 2019 Minnesota Student Survey (15% lesbian/gay/bisexual/queer/pansexual/questioning [LGBQ] and/or transgender/gender diverse [TGD] or gender questioning). Person-Centered Strategies to Promote Health Equity and Tackle Stigmatization: The Role of the Clinical Nurse Specialist.

INTRODUCTION: Discrimination and stigmatization in emergency departments continue to threaten equitable and safe patient care, particularly for individuals with psychiatric disorders and other marginalized groups. This could include people who are homeless or who are discriminated against because of their sexuality or gender or for other reasons. Traditional education focused solely on knowledge transfer has proven insufficient to address these issues. Person-centered practice, grounded in values, beliefs, and authentic relationships, offers a promising framework to foster equity, inclusion, a

From the Individual to the Ecosystem: An Institutional Framework for the Career Sustainability of Early-Career Health Professions Faculty.

The career sustainability of early-career faculty in the health professions is often framed as a matter of individual resilience, a perspective that obscures institutional determinants of ill-being and attrition. We argue that retention and thriving depend on an institutional ecosystem that integrates structured mentoring, psychopedagogical support and formal recognition of faculty work. Drawing on recent evidence on burnout, resilience and career development in academic medicine, we propose the Health Professions Faculty Career Sustainability Institutional Ecosystem (EISCD-Health) and the Fac

Systems Connected to Inequities in Access to Kidney Transplantation and the Value of Intersectionality.

Patients from minoritized backgrounds based on race/ethnicity, gender, sexuality, and other social identities are more likely to experience inequities in access in kidney transplantation. Although these inequities have been reported over the decades, limited research focuses on the experiences of patients with intersecting minoritized social statuses and the mechanisms that contribute to their reduced access to transplantation. Intersectionality, a framework for understanding the ways in which multiple social identities represent interacting systems of oppression and privilege, offers a nuance

The HIV Reservoir and Immune Landscape Across the Life Course of Women: Implications for Cure Strategies.

Sex hormones and reproductive transitions profoundly shape immune, metabolic, neuropsychological, and mucosal biology, as well as HIV persistence, across women's lives. Although women constitute the majority of people with HIV worldwide, hormonal and reproductive biology are still not sufficiently incorporated into HIV cure research. Cisgender women, transgender women on gender-affirming hormone therapy (GAHT), and gender-diverse individuals assigned female at birth are underrepresented in cure studies, despite experiencing unique and changing hormonal states that influence estrogen signaling,

Gender disparities in Swedish specialized nephrology care.

BACKGROUND: Although women have more prevalent chronic kidney disease (CKD) they less often have recommended therapy or a documented CKD diagnosis in primary care. This study investigates whether gender disparities remain in specialized nephrology care as a possible expression of gender bias. METHODS: Patients with incident estimated glomerular filtration rate (eGFR) <30 mL/min/1.73 m2 registered in the Swedish Renal Registry between 2010 and 2020 were included. Logistic regression investigated gender differences in having a CKD diagnosis, kidney biopsy, blood pressure (BP) <140/90 mmHg and g

Moving the dial on out-of-hospital cardiac arrest survival in Aotearoa New Zealand.

Out-of-hospital cardiac arrest (OHCA) remains a major public health challenge in Aotearoa New Zealand, with 30-day survival plateauing at approximately 11% in recent years. Registry data demonstrate that early bystander cardiopulmonary resuscitation (CPR) and community defibrillation significantly increase survival, yet community defibrillation rates remain low, and inequities persist across ethnicity, gender, deprivation and rurality. International evidence shows that system-level improvements in early response and equitable access to post-arrest care can substantially improve outcomes. Impro

Gender differences in the prevalence of anxiety and post-traumatic stress disorder and associated factors among adults in sub-Saharan Africa: a systematic review and meta-analysis.

BACKGROUND: Sex and gender equity in research has a growing focus, but there are gaps in fully integrating it in mental health research, limiting adoption of sex and gender-sensitive interventions. This review adopted a gendered perspective to synthesize prevalence and associated factors contributing to differences in anxiety and PTSD in SSA. METHODOLOGY: Electronic search was conducted in PubMed, PsycINFO, CINAHL, Scopus, Web of Science databases. Empirical studies conducted in SSA, targeting adults >18 years and having gender-disaggregated anxiety and PTSD prevalence and factors were includ…

Disease Site Specialization in the Academic Radiation Oncology Workforce: Evidence of Gender Differences.

PURPOSE: Because some stakeholders within medicine seek to diversify and attain greater workforce equity, it is critical to understand gender-based divisions within specialization. Radiation oncology (RO) has one of the smallest proportions of women representation of all specialties, and to our knowledge, no prior studies have investigated gender differences in all the disease site specializations within RO. Thus, we analyzed the relationship between gender and disease site(s) treated in academic RO (ARO). METHODS AND MATERIALS: Faculty gender and disease site(s) treated by faculty from ARO d
