Polyaspartic Acid (PAA) protects the kidney from experimental gentamicin nephrotoxicity despite large increases in renal cortical gentamicin content. In these experiments, prominent cytoplasmic vacuoles were noted in all animals that received PAA with or without gentamicin. The authors evaluated the intraocular penetration of gentamicin (50 mg/ml) into aphakic rabbit eyes following anodal iontophoresis (0.75 mA for 10 min). Gentamicin levels were determined at 0.5, 4, 8, 16, and 24 hrs after iontophoresis (n = 6 eyes for each time) using an agar diffusion bioassay. Flavobacterium odoratum was isolated from the stumps of the amputated toes of a debilitated patient. Gentamicin therapy and the poor state of health of the patient provided suitable conditions for this gentamicin-resistant species to multiply. When the gentamicin therapy was stopped, and as the patient's heal improved, F. odoratum gradually disappeared to be replaced by Staphylococcus aureus. This appears to be the first report of the isolation of F. odoratum in Australia, and it indicates the potential of this resistant species to act as a source of infection for groups at risk.

A 12-year-old boy developed renal wasting of magnesium, calcium, and potassium, with secondary hypomagnesaemia, hypocalcaemia, and hypokalaaemia (without hyperaldosteronism) after treatment with 14 400 mg gentamicin over 4 months. Gentamicin should not be given for prolonged courses if less toxic antibiotics are suitable. The incidence of bacterial endophthalmitis has been 3-6% after cataract extraction in the eye camps of South India from 1961-75. During this time 50 791 cases were operated using systemic and topical chloramphenicol prophylaxis. This study evaluated the prophylactic intracameral injection of 50 microng of gentamicin in eye camp cataract extractions. Only 6 of 1626 patients treated with intracameral gentamicin at operation developed bacterial endophthalmitis, an incidence of 0-37%.

We studied on the antibacterial activity of gentamicin against various pathogens isolated from clinical materials mainly isolated during 1974 and 1975, comparing with other antibiotics. Beta hemolytic streptococci, pneumococci and enterococci are less susceptible to gentamicin than staphylococci. Staph, aureus and Staph. epidermidis resistant to various antibiotics are very susceptible to gentamicin, and no resistant strain to this drug was found. Haemophilus influenzae, H. parainfluenzae and H. parahaemolyticus are very susceptible to gentamicin, and there is no resistant strain to this drug.

The prevalence of obligate anaerobes was studied prospectively in 60 patients with severe sepsis of intra-abdominal, soft tissue, female genital or oropulmonary origin. In addition, the efficacy of clindamycin (for anaerobes) plus gentamicin (for aerobic bacteria, especially coliforms) as initial empiric therapy in these patients was evaluated.
